Intended Use
The EyeQue Personal Vision Tracker (PVT) is a self-administered, mobile application-
driven refraction device that provides spherical, cylindrical and axis measurements
of the eye. The PVT allows the tracking of changes over time and generate estimated
refractive corrections.
Description
The EyeQue Personal Vision Tracker (PVT) is a self-administered refraction
measurement device that uses the light from your smartphone screen to determine
EyeQue EyeGlass Numbers: lens power needed to correct nearsightedness or
farsightedness as well as lens power and axis needed to correct for astigmatism. Your
EyeQue results are safely stored in the cloud.
EyeGlass Numbers are not prescriptions and personal vision testing does not replace
the need for an annual eye health exam.

Instructions
Download the app and complete your account activation process
1. Download the myEyeQue App from Google Play or Apple
app store
2. Launch the app and register for an account. If you already
have an EyeQue account, sign in to your existing account
3. You’ll be asked for the Serial number. You will fi nd a serial
activation number inside the box for registration
4. Complete your account activation process
How to get started and attach your EyeQue miniscope
1.Turn up your smartphone screen’s brightness, as well
as the volume. There will be voice prompts to help you
through your test.
2. Click on the “test” tab in the app. If this is your fi rst time
using the app, please review the tutorial and complete the
practice test before attempting a full test.
3. For either the practice test or full test, you will be prompted
to remove the miniscope from its case. Rotate the
eyepiece clockwise several times to get a feel for the
movement.
4 You will see a rectangular area with a dotted outline. This
is where you attach the miniscope. Remove the cover on
the base of the miniscope and attach it directly onto your
smartphone screen, with the EyeQue logo facing the plus
(+) and minus (-) buttons. The miniscope should not slide
easily once attached to your phone.
5. Secure the miniscope by attaching silicone elastic band
with a snug fi t.
6. Make sure the rotating eyepiece is set to position 1. Click next, and select whether
you will be taking the test with or without glasses or contact lenses.

How to take your fi rst vision test
1. Place your elbows on a fl at surface to hold your smartphone as steady as possible
2. Hold the smartphone at reading distance, and slowly move it closer until the eye
cup touches your right eye
3. You should see two lines, one red, and one green. Take a few seconds to fi nd
these lines. If you do not see them, try gently moving the miniscope left and right,
up and down. It takes some practice and is an important part of getting accurate
test results. NOTE: Your fi rst time taking the test will be the hardest. Once you
fi nd the lines and successfully completely your fi rst test, all subsequent tests will
become much easier to perform.
4. Once you see the red and green lines, open and relax your left eye (non-testing
eye) during the test. Most people fi nd it helpful to focus on a large object at least 6
feet away.
5. Tap on the plus (+) or the minus (-) button
and overlap the red and green lines
completely, until they form a solid yellow
line. You may hold down the plus (+) or
minus (-) buttons for faster movement.
6. Once you are satisfi ed that the lines are
overlapped, click “next”.
7. You will be prompted to rotate your miniscope to position 2. Turn the rotating eye
piece in the direction of the arrow until the number “2” is just above the white dot.
8. Look into the miniscope. The red and green lines are now at a different angle.
Overlap them as you’ve done for the previous test; click “next” when you see only
one solid yellow line.
9. Rotate your eye piece to position 3 and repeat. Complete all 9 measurements for
your right eye. Follow voice prompts.
10. Rotate your eye piece to position 1 and test your left eye. Complete all 9
measurements for your left eye. Follow voice prompts.
11. Congratulations! The test completion screen appears with the fi rst indication of
your vision. You may choose to save or discard this test result.

What to do with your vision test results
1. Instant results after each test only gives you
your spherical reading (nearsightedness or
farsightedness) and an estimated range of your
astigmatism (low, moderate, high).
2. After taking three sets of saved vision tests
with the EyeQue Personal Vision Tracker, the
myEyeQue app will generate an accurate set of
EyeGlass Numbers that you can use to check
whether your vision has changed since your last
prescription, or order new eyeglasses online.
3. More vision tests will result in better, more
accurate EyeGlass Numbers.
Multiple Users
One device can be shared with several users.
Since user results are saved and EGN calculations are based upon the results of
several saved tests, each user must have a separate EyeQue subscription account.
Note
Clean the miniscope eye cup with rubbing alcohol between users. Remove and/or allow
the rubbing alcohol residue to evaporate before bringing the miniscope eye cup to the
eye.
Contraindications
The miniscope should not be shared with users that are known to have, or exhibit signs
or symptoms of, an eye infection.
If used on other devices
Users will need the device serial activation number to unlock the app on other devices.

What Can You Do With EyeGlass Numbers?
We recommend using it for general tracking and assessment of your eyesight over time
and providing it to your eye care provider before your annual eye exam for evaluation.
EGN’s can be used to order eyeglasses from online retailers that do not require an eye
care provider’s prescription certifi cation.

1. Create an account

b. Confi rm your email address and login to your account
2. Purchase a subscription
a. Click on “Subscription” on the left-hand side
b. Select the Buy button

3. Download the MyEyeQue app for the PVT (or MyEyeQueVA app for the EyeQue
Insight.)
4. Login using the app
5. You may be prompted for the serial activation number from the EyeQue device.
Via the App
1. Download the MyEyeQue app for the PVT (or MyEyeQueVA app for the EyeQue
Insight.)
2. Open the app and on the login screen click on “Register” button
3. Complete the information
4. Confi rm your email address
5. Login and enter the device serial number
6. A pop-up will appear indicating a subscription is required.
7. Select the “Buy” button
8. Complete the purchase process.

With a single active subscription, a user can use the EyeQue PVT and/or EyeQue
Insight device. You do not need to purchase a different subscription to use the other
device. You may be prompted for the serial activation number from the EyeQue device
when launching the product-specifi c device for the fi rst time. This activation number
validates you are using a genuine EyeQue product.

The accuracy and precision of the results can vary among individuals. Not all users
will get accurate or consistent results because of, but not limited to, effects such as
accommodation, other eye conditions, insuffi cient manual dexterity, or improper use.

Safety Warning

detecting changes between regular eye exams, it does not replace a comprehensive

Pupillary Distance (PD) Ruler
Some EyeQue PVT’s are shipped with a PD Ruler. If your purchase came with one,
follow the directions below to get proper measurements.
What is Pupillary Distance?
Pupillary Distance is the distance between the centers of the pupils of the eyes,
measured in millimeters. PD is one of the measurements necessary to order glasses.
Depending on the ruler you have, there may be two methods to measure your PD.
Method 1
Applicable for most PD ruler styles.
1. Stand approximately 8 inches in front of a mirror or ask someone else to measure
for you.
2. Without touching the eye, align the “0” with the center of the pupil of one eye and
while looking straight ahead, measure the distance from the center of one pupil to
the center of your other pupil.
3. Note your results. (Range of Adult PD: 54-74 mm, Range of Child PD: 43-54 mm)
4. Repeat several times for accuracy and use an average for your fi nal measurement.
Method 2
Applicable if your ruler has eye-slits and you wish to use them.
Note: Method 1 may be applicable too; either method can be used.
1. Stand approximately 8 inches in front of a mirror or ask someone else to measure
for you.
2. Without touching the eye and while looking straight ahead, look through the eye
slits and note the number that is aligned with the center of one pupil (numbers will
range between 20 and 40mm).
3. Without moving the ruler, note the number that is aligned with the center of the
other pupil (numbers will range between 20 and 40mm).
4. Add the two results.
5. Note your results. (Range of Adult PD: 54-74 mm, Range of Child PD: 43-54 mm)
6. Repeat several times for accuracy and use an average for your fi nal measurement.
