WARNING: e chair weighs approximately 300 lbs. Use assistance
to remove the chair from the pallet. Failure to do so may result in

Forest Dental Equipment
1. Use a 7/32” hex key to remove the four bolts holding the chair to the
pallet. e bolts are located on the underside of the pallet (Figure 1).
2. Remove the outer strapping and plastic, then remove the screws
from the cardboard box, one from each side and two from each end.
3. Remove the outer box top and set aside.
4. Remove the cardboard supports.
5. Remove all plastic wrapping and foam.
6. Use wire cutters to cut the support straps/zip ties located on both
sides of the chair. Two ties at either side secure the chair to the
pallet. e other ties secure the chair to itself (Figure 2).
7. To aid in pallet egress, remove the two corner supports held in
place by 10 screws (Figure 3).
8. Remove the chair from the pallet and place it in the desired
location before doing any other installation.
9. Connect the chair base power cord to a power outlet.
NOTE: e chair base will automatically li to the highest position.
Support straps/zip ties
NOTE: Taking into consideration the chair’s specications
and dimensions position the chair in its permanent location
on a smooth hard and level oor making sure nothing will
interfere with its movement.

Armrest Upholstery
1. Place each upholstered armrest onto the chrome-plated arm supports.
2. Apply one drop Loctite 242 to hex screw threads and install underside of
each arm using a 5/32” hex key. (Figure 12).

Back Upholstery
1. Align the two upper and two lower studs on the chair back casting with
the slide holes on the back of the upholstered chair back.
2. Firmly press the upholstered chair back over the studs and slide the
upholstered chair back down until all four studs are locked into place
(Figure 11).

NOTE: Installation of the delivery unit is recommended before
upholstery installation. Follow the instructions supplied with the
delivery unit.
Bracket holes with inserted connector pins
1. Loosen (but do not remove) the two set screws with a 3/32” hex
key and remove both connector pins (Figure 4 and 5).
2. Li the back rest to its upright position.
3. Line up the linkage holes and reinsert the connector pins
(Figure 4 and 5).
4. Apply Loctite 242 to the set screw located on the end of the backrest.
5. Using a 3/32” hex key, tighten the set screw against the at surface
of the pin to hold the internal arm.
1. Remove cotter, clevis pins and set aside (Figure 6).
2. Place a nylon washer onto each clevis pin and slide into seat frame
hole. Place the second nylon washer onto the clevis base pins and
slide onto chair base hole. Replace cotter pin to hold frame in place
(Figure 7).

CAUTION: If the chair is part of an operatory, wait until entire
installation is complete before installing upholstery. Failure to do so may
result in damaging the upholstery.
Seat Upholstery
1. Align the studs on the underside of the upholstered seat with the
holes in the seat frame.
2. To install the upholstered seat to the seat frame, use a Phillips-head
screwdriver to install screws, install the upholstered seat to the seat frame

1. Tighten the tension adjustment screws as necessary until
the desired slide tension is reached (Figure 10).

1. Install the top screw to hold the headrest pad in place by using a
3/32” hex key (Figure 9A).
2. Press the locking mechanism (Figure 9B).
3. Flip the headrest up. Install the bottom hex screw (Figure 9C).
4. Press the locking mechanism to return headrest back into its
normal working position (Figure 9D).
