INSTRUCTIONS:
1. Retain this record in the front of the document.
2. On receipt of temporary revisions, insert pages adjacent to the pages
affected, in accordance with filling instructions.
3. Insert temporary revision number, issue date, date inserted, name and sign.
4. When a normal revision or another temporary revision is received which
incorporates or supersedes a temporary revision, enter the new revision
number and the issue date, then remove the affected temporary revision
pages, enter date and sign.
5. If a temporary revision is removed, without being superseded by a normal
revision, write “NONE” in the Incorporated/Superseded column.
