Wozu dient die Pessarbehandlung?
Was bewirkt ein Keulen-Pessar?
Keulen-Pessare werden eingesetzt zur Behandlung einer ausgeprägten Senkung
bzw. einem Vorfall von Scheide und Gebärmutter. Z.B. wenn andere Pessare
diesem hohen Druck nicht standhalten und es bei der Verwendung eines Würfel-
Pessars zu vermehrten unfreiwilligem Abgang von Urin (Inkontinenz) kommt.
Diese Senkungen können z.B. durch eine angeborene Bindegewebsschwäche,
durch Hormonmangel nach den Wechseljahren oder durch Schwangerschaften
und Geburten hervorgerufen werden.
Der medesign Keulen-Pessar repositioniert die Gebärmutter in die anatomisch
richtigen Lage und wirkt dem Vorfall bzw. der Senkung von Scheide oder
Gebärmutter (Totalprolaps) entgegen. Der verdickte Pessarstiel wird hinter der
Symphyse positioniert und verhindert das Herausrutschen des Pessars und
gibt so größeren Halt.
medesign Keulen-Pessare werden in verschiedenen Größen angeboten. Die
Auswahl der richtigen Größe ist die Voraussetzung für eine optimale Behandlung
und Beschwerdefreiheit. Ihr Arzt wird die für Sie richtige Größe ermitteln. Das
Pessar sollte so gewählt werden, dass es auch beim Husten und Pressen noch
gut im Scheidengewölbe haftet. Nach einigen Wochen bzw. Monaten wird der
Arzt überprüfen, ob evtl. auf eine kleinere Pessargröße gewechselt werden kann.
Welches Pessar ist das richtige für mich?

medesign Pessare werden aus einem speziell für den medizinischen Gebrauch
entwickelten, flexiblen und gewebefreundlichen Silikon hergestellt. Sollten Sie
dennoch eine Überempndlichkeit bemerkten, verwenden Sie das Pessar nicht
mehr und besprechen die Situation mit Ihrem Arzt. Das Keulen-Pessar ist relativ
flexibel und lässt sich problemlos einführen. Das Pessar darf nur von einer Patientin
verwendet werden.
Die Pessare lassen sich gut selbst einführen. Kontrollieren Sie das Keulen-Pessar
auf Unversehrtheit, Sauberkeit, Verfärbungen, Schäden und Abnutzung. Bei
sichtbaren Schäden oder Veränderungen der Oberfläche ist das Pessar durch
ein neues zu ersetzen.
Sie sollten das Keulen-Pessar morgens in die Scheide einführen und abends wieder
entfernen. So kann sich das empndliche Scheidengewebe über Nacht erholen.
In besonderen Fällen kann das Pessar auch über Nacht eingesetzt bleiben, bzw.
nachts durch ein evtl. kleineres Pessar ersetzt werden. Statt das Keulen-Pessar
selbst zu wechseln, überlassen viele ältere Patientinnen den Wechsel Ihrem Arzt
oder der Pflege . Dieser ist alle ein bis zwei Wochen nötig. Halten Sie sich dabei
an die Anweisungen Ihres Arztes.
Bevor Sie mit dem Einsetzen des Pessars beginnen, sollten Sie sich die Hände
waschen. Um das Einführen zu erleichtern hat sich die Verwendung eines
Gleitmittels wie z.B. Ladysoft bewährt. Östrogenhaltige Creme bzw. Gel tragen
Sie nach Bedarf auf die Schale des Keulen-Pessars auf.
Zuerst müssen alle vorgefallenen Organe wieder an die richtige Stelle
zurückgeschoben werden (Lassen Sie sich bitte die dazu nötigen Handgriffe
von Ihrem Arzt genau erklären). Nun führen Sie das Pessar mit dem Griff nach

unten in die Scheide ein. Dazu kann es hilfreich sein, ein Bein z.B. auf die Bettkante
zu stellen. Manchen Frauen fällt es leichter, die Keule in der Hocke oder im
Liegen einzuführen. Sie können sich auch mit leicht gespreizten Beinen an eine
Wand lehnen und dann das Pessar einführen. Probieren Sie ein bisschen, so
nden Sie am besten die für Sie vorteilhafteste Methode. Fassen Sie dazu die mit
Ladysoft bestrichene Schale mit der rechten Hand und drücken sie im schrägen
Durchmesser vorsichtig, dem geringsten Widerstand folgend nach hinten unten in
die Scheide. Achten Sie dabei auf den empndlichen Wulst der Harnröhre. Ist der
Ring an der Harnröhre vorbeigerutscht, drehen Sie die Schale in die Waagerechte
und schieben Sie soweit wie möglich nach oben. Das Ende der Keule ist meist
eben noch sichtbar. Schieben Sie das Keulen-Pessar so weit wie möglich in die
Scheide bzw. in die Position, die Ihnen Ihr Arzt gezeigt hat. Um Druckstellen zu
vermeiden sollte der Stil des Keulen-Pessars immer an einer anderen Stelle der
Symphyse platziert werden.
Auch vor dem Entfernen des Pessars sollten Sie sich die Hände waschen. Benetzen
Sie den inneren Scheidenbereich mit viel Ladysoft Gleitgel. Dann ziehen Sie
am Griff der Keule das Pessar in der Scheidenachse bis zum Anschlag an das
Schambein (Symphyse). Dazu gehen Sie am besten in die Hocke. Dabei bildet sich
aus dem schlaffen, überdehnten Gewebe oft ein ngerdicker Wulst als Hindernis.
Mit der freien Hand schieben Sie diesen Wulst langsam um den äußeren Rand
des Pessars herum. Danach können Sie das Keulen-Pessar leicht entfernen.
Verwenden Sie keine Hilfsmittel bei der Entfernung des Keulen-Pessars.
Anschließend waschen Sie sich nochmals die Hände.
Wie entferne ich das Keulen-Pessar?

Nachdem Sie das Pessar entfernt haben reinigen Sie es mit lauwarmem Wasser
und Seifenlösung oder einer milden Waschlotion wie z.B. Femsoft. Anschließend
klar reinspülen und trocknen lassen. Eine Desinfektion ist normalerweise nicht
nötig. Nach Reinigung des Pessars waschen Sie sich nochmal die Hände.
Zur unterstützenden Reinigung und Pflege des Intimbereiches verwenden Sie z.B.
Femsoft Intim-Waschlotion. Eine speziell entwickelte Waschlotion auf der Basis
von Molke und Brennnessel mit einem niedrigen pH-Wert (3,8 bis 4). Anschließend
waschen Sie sich nochmals die Hände.
Reinigung des Pessars
Nicht mit chemischen Mitteln reinigen.
Keine Haushaltsreiniger oder Spülmittel verwenden.
Es ist nicht erforderlich dass Pessar auszukochen.
Legen Sie es nicht in die Mikrowelle.
Beim Stuhlgang kann es vorkommen, dass das Pessar ganz oder teilweise
herausrutscht. Reinigen Sie das Pessar (wie unter Reinigung beschrieben) und
setzen es erneut wie oben beschrieben ein. Das Herausrutschen des Pessars
können Sie verhindern, indem Sie den Stiel des Pessars ertasten und ihn ggf.
beim Pressen in der Scheide festhalten.

Kontraindikatoren zur Pessaranwendung sind
Infektionen von Vagina und Zervix.
Nicht anwenden bei einer Überempndlichkeit
von Silikon. Bisher sind keine allergischen
Reaktionen bekannt.
medesign Pessare sind aus geprüftem medizinischem Silikon hergestellt und
können im Hausmüll entsorgt werden Das Silikon ist nicht biologisch abbaubar,
daher sollte es nicht über die Toilette entsorgt werden.

verwenden oder mit ärztlicher Rücksprache.
·Nicht verwenden wenn Sie schwanger sind oder glauben, schwanger zu sein.
·Nicht bei Blasen-, Harnweg-, oder Scheidenentzündung verwenden.

When is therapy with a Club Pessary useful?
How does the Club Pessary work?
Club Pessaries are used for treating severe descensus or prolapse of the vagina
and the uterus e.g. when other pessaries succumb the large pressure and when
the use of a Cube Pessary increases involuntary leakage of urine (incontinence).
Descents like this can be caused by e.g. hereditary weakness of the connective
tissue, postmenopausal hormone deciency or pregnancy and giving birth.
medesign’s Club Pessary repositions the uterus and brings it into the anatomically
correct position – counteracting a prolapse or descensus of the vagina or the
uterus (total prolapse). The thickened stem of the pessary is placed behind the
symphysis – stabilising the pessary and preventing it from slipping out.
Which Club Pessary is the right one for me?
medesign’s Club Pessaries are offered in various sizes. Selecting the correct size is a
prerequisite for ideal treatment and freedom from symptoms. Your gynaecologist will
determine the right size for you. Select a pessary, which securely stays in the vaginal
vault even when coughing and squeezing. Your gynaecologist will assess the t of your
pessary after a few weeks or months to see whether you can switch to a different size.
medesign pessaries are made from flexible, tissue tolerant silicone, which is
especially developed for medical use. If you should, despite this, notice a reaction,
stop the use of the pessary and discuss the situation with your physician. The
Club Pessary is rather flexible and can thus be inserted without difculties. The
pessary may be used by one patient only.

Pessaries can easily be inserted on your own. Check the Club Pessary for intact-
ness, cleanliness, discolouration, damage and wear marks. The pessary has to
be replaced by a new one should damage or changes on the surface be visible.
You should insert the Club Pessary into the vagina in the mornings and remove it in
the evenings in order for the sensitive vaginal tissue to be able to recover overnight.
In certain cases the pessary may remain inserted during the night or be replaced by
a smaller pessary at night. Instead of changing the Club Pessary themselves, many
older patients leave this to their physician. Removal and reinsertion is necessary
every, or every other week. Please follow your doctor’s instructions for this.
Wash your hands prior to inserting the pessary. To ease insertion it is recommended
that a lubricant, such as Ladysoft, be used. If needed, the Club Pessary may be
coated with a cream or gel containing oestrogen.
To begin with, all prolapsed organs have to be pushed back into their correct
position (please have your physician exactly explain all necessary movements for
this). Now insert the pessary into the vagina with the handle facing downward. It
may be helpful to place one leg e.g. on the edge of your bed. Some women nd it
easier to insert the Club Pessary in a crouching or lying position. You could also
lean against a wall with your legs slightly spread and then insert the pessary.
Try out which method is best for you. Use your right hand to gently push the
cup - coated with Ladysoft – towards the back and bottom of your vagina. Pay
attention to the sensitive bulge of the urethra. Once the ring has slipped past
the urethra, turn the cup horizontally and push it as far up as possible. Usually
the tip of the stem is still visible.
Push the Club Pessary as far into the vagina as possible or into the position
shown to you by your physician. The stem of the Club Pessary should always be
placed in a different position on the symphysis in order to avoid pressure marks.
How do I insert the Club Pessary?

You should also wash your hands prior to removal. Thoroughly lubricate your
vaginal area with a large amount of Ladysoft Lubricant. Then pull the stem of
the Club Pessary up until it stops at the symphysis. An obstacle in form of a
bulge as thick as a nger may form from the loose, overstretched tissue. This
bulge must slowly be pushed past the edge of the pessary with your free hand.
Following this you will easily be able to remove the Club Pessary. Do not use any
assistive equipment for removing the Club Pessary.
How do I remove the Club Pessary?
It is possible that the pessary slips out partially or totally during defecation.
Clean the pessary (as described below) and insert it again as explained above.
You can avoid the movement of the pessary by locating the stem of the pessary
and holding it in place inside your vagina while pushing.
If the Club Pessary slips out when having bowel movement?

Clean the pessary with lukewarm water and soap or a mild washing lotion such
as Femsoft after removal. Subsequently rinse and leave to dry. Disinfection
is usually not necessary. Wash your hands again after cleaning the pessary.
For added support for the cleaning and care of you intimate area, use e.g. Femsoft
Intimate Wash – a specially developed washing lotion with whey and stinging nettle
as a basis and a low pH-value (3.8 – 4). Subsequently wash your hands again.
Pessary Care
Do not use chemicals for cleaning.
Do not use household or dish detergents.
It is not necessary to boil the pessary.
Do not put it in the microwave.

medesign pessaries are made from certied medical silicone and can be disposed
of in household waste. The silicone is not biodegradable and should therefore
not be disposed of in the toilet.
Disposal
Infections of the vagina and the cervix are
contraindications for pessary treatment.
Do not use if you are oversensitive to silicone.
So far no allergic reactions known.

A quoi sert le traitement avec pessaire ?
Comment agit un pessaire club ?
Les pessaires club sont utilisés pour traiter un prolapsus prononcé ou un prolapsus
du vagin et de l’utérus. Par exemple, lorsque d’autres pessaires ne peuvent pas
supporter cette pression élevée et que l’utilisation d’un pessaire en cube entraîne
une augmentation des fuites involontaires d’urine (incontinence). Ces gouttes
peuvent être provoquées, par exemple, par une faiblesse congénitale du tissu
conjonctif, par une carence hormonale après la ménopause ou par la grossesse
et l’accouchement.
Le pessaire club de medesign permet de repositionner l’utérus dans la bonne
position anatomique et de contrer le prolapsus du vagin ou de l’utérus (prolapsus
total). La tige épaissie du pessaire est positionnée derrière la symphyse, ce qui
empêche le pessaire de glisser et lui procure ainsi un meilleur soutien.
Quel est le pessaire club qui me convient ?
Les pessaires club de medesign sont proposés dans différentes tailles. Le choix
de la bonne taille est la condition préalable à un traitement optimal et à l’absence
de symptômes. Votre médecin déterminera la taille qui vous convient le mieux.
Le pessaire doit être choisi de manière à ce qu’il adhère encore bien à la voûte
vaginale lorsque vous toussez et poussez. Au bout de quelques semaines ou de
mois, le médecin vériera s’il peut éventuellement passer à une taille plus petite.
