Intended Use
HeroTracker®sensor is a medication
reminder device intended for single-patient
use to assist physicians and patients in
recording and monitoring the actuations
of prescribed metered dose inhaler (MDI)
or Diskus®inhaler usage. The device is
designed and intended for use by children 5
years of age or older, as well as adults.
Contraindications
The HeroTracker sensor is not intended
for use as an MDI or Diskus inhaler dose
counter, nor is it intended to indicate the
quantity of medication remaining in an MDI
or Diskus.

or Rescue tracks your medication use
automatically. When you actuate your
medication, you trigger the sensor in
the HeroTracker device. The date and
time of your dose is then recorded by
the HeroTracker sensor and stored to its
memory.
Diskus®Inhaler
Your HeroTracker Sensor for Diskus tracks
your medication use each time you press
the colored button in the center. After you
actuate your medication, press the button
to alert the HeroTracker sensor of the dose.
The date and time of your dose is then
recorded by the HeroTracker sensor and
stored to its memory.
Bluetooth Pairing and Syncing
If a mobile device paired with the

BreatheSmart mobile app is within range,
the information from the MDI or the Diskus
will be transferred to the app. If not, the
HeroTracker sensor will search once an hour
for a mobile device with the BreatheSmart
mobile app until it is able to sync the data.
Installation
Metered Dose Inhaler (MDI)
Remove the plastic pull tab on the inside
of the HeroTracker Sensor for Control or
Rescue to enable the battery to connect.
To attach your HeroTracker sensor to your
inhaler, rst match the HeroTracker sensor
to the type of medication you will be using.
HeroTracker sensors come in two types, one
for control medication (purple sensor) and
one for rescue medication (teal sensor).
Make sure to match the appropriate
HeroTracker sensor with the type of

medication that matches. Next, unclip the
top strap of the sensor from the connection
pegs. Push the medication canister and
the top part of your MDI through the ring
formed by the HeroTracker sensor until the
HeroTracker sensor securely ts on the MDI.
The SYNC button should be facing away from
the MDI mouthpiece.
Pull down the top strap and connect the
strap to the two connection pegs. Adjust
the t of the HeroTracker sensor on the
medication, so that the trigger sits in the
middle of the top of the medication canister
(see image on the next page).

Your nger should comfortably sit in the
designated area on top of the HeroTracker
sensor, and the SYNC button should be
facing away from you when you take your
medication.

Diskus®Inhaler
Remove the plastic pull tab on the inside
of the HeroTracker sensor to enable
the battery to connect. Next, attach the
HeroTracker sensor to your inhaler by sliding
the HeroTracker sensor over the inhaler.
You will know that it is successfully attached
when you hear a small click.

Usage
Bluetooth Pairing
Follow the instructions in the BreatheSmart
mobile app to pair your HeroTracker
sensor. When you are asked to sync your
HeroTracker sensor, press the small SYNC
button on the back.
Metered Dose Inhaler (MDI)
Once the HeroTracker sensor is attached
to an MDI and paired to the BreatheSmart
app, when you actuate your MDI, you will
automatically engage the trigger underneath
the top strap. The HeroTracker sensor will
record the date and time of this dose. This
data will be automatically transferred to the
BreatheSmart app when a mobile device
with the paired BreatheSmart app is within
range.

Diskus®Inhaler
Once the HeroTracker sensor is attached to
an inhaler and paired to the BreatheSmart
app, you will be able to record your
medication adherence by pressing
the colored button on the front of the
HeroTracker sensor when you take your
medication. By doing this, the HeroTracker
sensor will record the date and time of
this dose. This data will be automatically
transferred to the BreatheSmart app when a
mobile device with the paired BreatheSmart
app is within range.
Transferring to a New Inhaler
Metered Dose Inhaler (MDI)
When you run out of medication in the MDI
that has a HeroTracker sensor, and you
have a new MDI, you can simply remove
the HeroTracker sensor by disconnecting

the top strap from the two pegs, and sliding
the HeroTracker sensor o the top of the
MDI. Then reinstall on new medication
following the instructions above. See also
usage instructions provided within the
BreatheSmart app.
Diskus®Inhaler
When you run out of medication and receive
a new inhaler, simply slide the HeroTracker
sensor o of the old inhaler and onto
the new one following the instructions in
the “Installation” section. See also usage
instructions provided with the BreatheSmart
app.

Precautions and Warnings
The HeroTracker sensor is not meant
to communicate emergencies. Call your
doctor right away if you need care.
HeroTracker sensor information can be
delayed and your care team may not be
aware of your condition. The HeroTracker
sensor can help patients and their care
teams understand patterns over time. The
HeroTracker sensor is not a real-time patient
monitoring system. If there is a serious
incident that has occurred in relation to the
device, please contact Cohero Health at
(833) 426-4376 or support@coherohealth.
com to report the incident.
Medication pu activation

interrupted by a number of conditions.

Conditions that could cause this include
excessive distance between the HeroTracker
and phone or radiated signals in the vicinity
of the phone. Other conditions that can
cause this is; the phone or Bluetooth being
turned o, or no internet connection. In any
event, the dose information is not lost and
is stored within the HeroTracker, and will be
sent either during the next successful dose
along with the present dose or by pressing
the “Sync” button on the HeroTracker.
If your HeroTracker sensor interferes in
any way with use of your inhaler, remove
your sensor. You can continue to enter your
doses manually.
Never delay using your inhaler in order to
attach your HeroTracker sensor.

The HeroTracker sensor is not a dose
counter. Please use the dose counter on
your inhaler, if it has one. If the sensor
obstructs the dose counter, frequently check
your dose counter to ensure that you do not
run out of medication.
The HeroTracker sensor should not be
immersed in liquid. Do not place the
sensor under water or put it in a dishwasher
or sterilizer. It could damage the sensor and
cause it to not function properly.
Choking hazard: Keep away from small
children.
Do not try to remove the battery from
the HeroTracker sensor or perform
servicing activities on the sensor. This
could damage the sensor and cause it to not
work properly. Changes or modications not

expressly approved by Cohero Health could
void the user’s authority to operate the
equipment.

interference in a residential installation.
This equipment generates, uses and can
radiate radio frequency energy and, if not
installed and used in accordance with the
instructions, may cause harmful interference
to radio communications.
However, there is no guarantee that
interference will not occur in a particular
installation.
If this equipment does cause harmful

which can be determined by turning
the equipment o and on, the user is

• Reorient or relocate the receiving antenna.

receiver is connected.
• Consult the dealer or an experienced
