The system stays in HCL all the time except when CGM data is not

temporary basal rates.

Do not override boluses to give more insulin than

Bolus before eating. If bolusing after a meal, the
user should reduce bolus as system has already been
increasing insulin for hyperglycemia.
Give correction boluses for hyperglycemia if
recommended by the bolus calculator.
OTHER EDUCATION
Treat hypoglycemia with 5-10 g CHO since insulin
may have been reduced/suspended for a period of time
before hypoglycemia occurs.
Disconnecting: If disconnected for more than 20
minutes, SUSPEND insulin so Control-IQ calculates IOB
accurately
Infusion set failure: Change infusion set if unexplained
persistent hyperglycemia. ( i.e. >300 mg/dL for >90 min)

AUTO-OFF: Consider setting “Auto o”
to “O”. Auto-o is a pump setting (not
specific to control-IQ). If set to “On”- pump
will suspend all insulin delivery IF the
user has not pressed any buttons in the
programmed time duration (i.e. 12 hours
default). This may cause unnecessary/
dangerous suspensions of insulin.
ALARM FATIGUE?
• Consider setting low alert only, low
snooze 20 min.
• Turn o all optional alerts that are not
useful to user
WEIGHT/TDI: Update “Weight” and
“Total Daily dose” on their insulin pump at

is using the Tandem t:slim X2 with Control-IQ Hybrid Closed Loop (HCL) system to manage his/her
diabetes (insulin pump + Dexcom G6 CGM + algorithm to maximize time in range). I reviewed the past 2
weeks of data. Overall, their CGM Time-In-Range (70-180 mg/dL) was %, Time Above Range (>180 mg/
dL) %, and Time Below Range (<70% mg/dL) was %. He/she is using the HCL feature % of the time, and is
wearing the Dexcom CGM 100% of the time. We reviewed settings including CGM alerts. I reviewed
hyperglycemia safety with insulin pumps, including checking ketones with prolonged hyperglycemia, giving
an insulin injection with pen or syringe if ketones elevated, and changing infusion set. reports giving meal
boluses per day.

Great job using Control-IQ for your diabetes care.
Using systems like this can help you achieve better glucose control. Aim for more than 70% of your CGM
glucose levels to be between 70-180 mg/dl. This is the goal for MOST people with type 1 diabetes. This is
about the same as having an HbA1c level of 7%

HYPERGLYCEMIA > 300 mg/dL for 1-2 hours?
Check ketones first! If ketones, give a syringe injection of insulin and turn o “Control-IQ”
feature for 4 hours. Change infusion set (“when in doubt, change it out!”)
Do not override boluses to give more insulin than the pump recommends
(may cause hypoglycemia due to automated insulin delivery).
Bolus before eating. If bolusing after a meal, the user should reduce bolus as
system has already been increasing insulin for hyperglycemia.
Read bolus prompts carefully. If it states “Your BG is Below Target.
Reduce Bolus Calculation?”, press “NO” (or r) to get full amount of insulin for
carbohydrates. Press “Yes” (ora) to subtract insulin.
Give correction boluses for hypergylcemia.
Try treating hypoglycemia with 5-10 g CHO since insulin may have been
reduced/suspended for a period of time before hypoglycemia occurs.
If disconnected for more than 20 minutes, SUSPEND insulin so the Control-IQ
system counts IOB correctly.
Check “Auto-o” settings. Turn o or extend to 16 hours.
CHANGE INFUSION SET every 2-3 days, or as needed for persistent hyperglycemia.
