BLOM-SINGER® SpeakFree™HME
DEVICE DESCRIPTION
The Blom-Singer SpeakFree™ HME is a single use device that oers the benets of a heat and
moisture exchange system as well as a hands-free valve to achieve tracheostoma (stoma)
closure for use of a voice prosthesis.
The device (diagram 1) is intended to be attached to either a Blom-Singer tracheostoma
adhesive housing or a Blom-Singer StomaSoft™ Laryngectomy Tube (diagram 2). The device
contains a foam lter for heat and moisture exchange and a detachable crossbar. The purpose of
the crossbar is to prevent unintended airway obstruction.
INDICATIONS (Reasons to prescribe the device or procedure)
The device is for individuals who have a permanent tracheostoma following total laryngectomy
surgery. The device provides humidied and warmed air. The device allows for hands-free
speech in patients who have undergone tracheoesophageal puncture (opening made by
a surgeon between trachea and esophagus for placement of a voice prosthesis) with voice
prosthesis insertion.
CONTRAINDICATIONS (Reasons that make it inadvisable to prescribe the device or procedure)
The device should not be placed over a tracheostoma if its presence obstructs respiration.
WARNINGS AND PRECAUTIONS
Wash hands with soap and water before and after handling the device. Only use a lint-free cloth
or tissue on the device to avoid getting lint into the airway. This device is not reusable and may
only be used by one patient. There is a risk of infection if this device is shared between patients
or reused. Inspect the device before use for physical damage, such as cracks or tears, and do not
use if damaged. If physical damage is found, contact InHealth Technologies Product Complaints.
Breathing
Breathing through the device should be comfortable and easy. If breathing is not easy, remove
the device and consult your medical professional for assistance. When you begin using the
device, a slight increase in airow resistance may be felt. This resistance is a normal function of
the device that may require some adjustment time. The device may need to be removed during
instances of extreme physical activity and heavy breathing.
Coughing
The device must be removed before coughing or clearing the airway.
Sleeping
The device must be removed before sleeping to avoid the potential for airway obstruction.
POTENTIAL COMPLICATIONS
The following may occur:
yLabored breathing, which may occur if the device is not changed regularly
yStoma contamination or infection that may require the use of appropriate antibiotics
yIrritation, including skin irritation around the stoma
INSTRUCTIONS FOR USE
Insertion, Removal and Care
Insertion and removal of any devices worn over the tracheostoma should only be done with the
user positioned in front of a mirror with a bright light focused directly on the stoma.
A Blom-Singer tracheostoma housing or StomaSoft™ Laryngectomy Tube must rst be placed
on/around the tracheostoma. Before inserting or removing the device, adjust it to its nearest-
closed position by rotating the outer rim clockwise (diagram 3).

Insertion
To insert the device, grasp its outer rim and insert it partially into the lower rim of the previously
applied housing or laryngectomy tube. Gently push the device into the attachment housing or
tube until it snaps securely into place (diagram 4). Do not use the crossbar as a handle to insert
the device.
Care
The device is designed for daytime/awake use only and is not designed to be disassembled,
reassembled, or reused. If the device should become heavily soiled, remove it and replace with
a new device. If mucus is removed from the device, never use facial or toilet tissue as particles
of lint or fabric may be deposited onto the device that can be inhaled into the airway upon
reinsertion. Do not wash the device as this will result in loss of function.
Removal
To remove the device, hold the tracheostoma housing or laryngectomy tube in place with one
hand while pulling the device with the other hand until it releases (diagram 5). Do not use the
crossbar as a handle to remove the device.
Attaching/Removing Crossbar
To attach the optional crossbar, hold the device and press the crossbar onto the top of the
device until it snaps into place (diagram 6). To remove the crossbar, hold onto the device and
pull the crossbar away from the device until it releases (diagram 7). Do not attempt to place or
remove the crossbar while the device is attached to the tracheostoma.
Device Function
When properly adjusted, the valve remains in an open position during normal, quiet breathing
and routine physical activity. As the user exhales to speak, the airow pushes the valve into
the closed position to provide a seal for speech. When the user stops speaking or inhales,
the valve returns to its resting open position. This device can also be manually occluded to
produce speech by placing a nger or thumb over the opening at the top of the device when
the crossbar is detached. With the extremely high pressure of a cough, the valve may be forced
partially through the opening in the device. If this happens, remove the device and reposition
the valve with a clean ngertip.
Device Sensitivity Adjustment
The device is designed to accommodate the speech and breathing requirements of each
individual user by rotating the outer ring of the device clockwise or counterclockwise. Clockwise
rotation adjusts the valve to close more easily (diagram 3). Counterclockwise rotation adjusts
the valve to require greater eort (respiratory airow) to close (diagram 8). Set the device to
a more closed position during speech and adjust it to a more open position during increased
respiratory exertion (e.g. during physical exercise).
To adjust the device while seated in the tracheostoma housing, the user should rst stabilize the
base of the housing with one hand while simultaneously rotating the outer ring of the device
with the other hand (diagram 9).
SPECIAL STORAGE AND/OR HANDLING CONDITIONS
These devices have special storage and/or handling conditions:
yWash hands after handling the devices, as they contain substances that may be irritating to
skin and eyes.
yThe package containing the devices should be resealed after opening to preserve the
eectiveness of the devices’heat and moisture exchange capabilities.
DISPOSAL INSTRUCTIONS
This product is not biodegradable and can be considered contaminated when used. Carefully
dispose of this device per local guidelines.

设备功能
经过适当调节后，在正常且安静的呼吸和日常体力活动中，阀门将保持在打开位置。在使用者
呼气时，气流将阀门推至关闭位置，以为说话时提供密封。在使用者停止说话或吸气时，阀门
将返回其静止开启位置。当断开横杆时，也可以通过将手指或拇指放在设备顶部的开口，通
过手动阻塞此设备以发出语音。在咳嗽的极高压力下，阀门可能会部分地被迫通过设备上的开
口。如果发生这种情况，请取出设备，然后用干净的指尖重新复位阀门。
设备灵敏度调整
该设备设计为通过顺时针或逆时针旋转设备的外环来满足每个用户的语音和呼吸需求。顺时针
旋转可更轻松地关闭阀门（图3）。逆时针旋转可将阀门调节至需要更大的力气（呼吸气流）
才能关闭的位置（图8）。在说话过程中将设备设置为更加关闭的位置，并在增加呼吸运动时
（例如，在体育锻炼过程中）将其调整为更加开启的位置。
如需在气管造口外壳中时调整设备，用户应首先用一只手固定外壳底座，同时用另一只手旋转
设备的外环（图9）。

警告および使用上の注意

呼吸

使用説明
挿入、取り外し、ケア

Blom-Singer気管孔ハウジングまたはStomaSoft™ Laryngectomy Tube は、まず最初に気管孔

挿入

ケア

は設計されていません。装置がひどく汚れた場合は、取り外して新しい装置と交換します。
装置から粘液を取り除く場合、糸くずや布の粒子が装置に付着して再挿入時に気道に吸入さ
れる恐れがあるため、ティッシュペーパーやトイレットペーパーは絶対に使用しないでくだ
さい。機能が失われるため、装置を洗浄しないでください。

取り外し
装置を取り外すには、片手で気管孔ハウジングまたは気管切開チューブを所定位置に保持し
ながら、もう片方の手で装置をリリースするまで引っ張ります (図5)。装置を取り外す際、
クロスバーをハンドルとして使用しないでください。
クロスバーの取付け/取外し
オプションのクロスバーを取り付けるには、装置を保持し、装置上部の所定位置にカチッ
とはまるまでクロスバーを押し込みます (図6)。クロスバーを取り外すには、装置をギュッ
と握って、クロスバーが外れるまで装置から引き離します(図7)。気管孔に装置を取り付けた
状態で、クロスバーを取り付けたり取り外したりしないでください。
装置の機能
適切に調整されると、通常の安静時の呼吸と日常の身体活動の間、バルブは開放位置のまま
になります。ユーザーが話すために息を吐くと、空気の流れがバルブを閉位置に押し込み、
発話のために密封されます。ユーザーが話すのをやめたり息を吸い込むと、バルブは再び
開き、開の静止位置に戻ります。また、この装置は、クロスバーが取り外されているとき
に、装置上部の開口部に指または親指を当てることにより、手動で閉塞させて音声を生成す
ることができます。咳の極端に高い圧力により、バルブが部分的に装置の開口部を押し通
されることがあります。このような場合は、装置を取り外して清浄な指先でバルブを再配
置します。
装置の感度調整
この装置は、装置の外輪を時計回りまたは反時計回りに回転させることにより、個々のユー
ザーの発話および呼吸の要件に対応するように設計されています。時計回りに回転させる
と、バルブはより簡単に閉じるように調整されます (図3)。反時計回りに回転させると、バ
ルブを閉じるのにより大きな力（呼吸流量）が必要になるように調整されます(図8)。発話中
は装置をより閉位置に設定し、呼吸活動が増えたとき（例：身体運動中など）にはより開位
置に調整します。
気管孔ハウジングに着座した状態で装置を調整するには、ユーザーはまず、片手でハウジン
グの基部を安定させながら、同時に他方の手で装置の外輪を回転させます (図9)。
特別な保管/取り扱い条件
これらの装置には、特別な保管/取り扱い条件があります。
y皮膚や目を刺激する可能性のある物質が含まれているため、装置の取り扱い後は手を
洗ってください。
y装置を含むパッケージは、装置の熱・水分交換能力の有効性を維持するために、開封
後は再び封じる必要があります。
廃棄方法
本製品は生分解性ではなく、使用後は汚染されているとみなすことができます。この装置

PËRSHKRIMI I PAJISJES
HME SpeakFree™ i Blom-Singer është një pajisje njëpërdorimëshe që ofron përtimet e një
sistemi shkëmbimi nxehtësie dhe lagështie, si dhe një valvulë me komandim pa duar për të
kryer mbylljen e trakeostomës (stomës) për përdorimin e një proteze zëri.
Pajisja (diagrami 1) është synuar për t’u montuar qoftë në një mbajtëse ngjitëse të trakeostomës
të Blom-Singer ose në një tub laringektomie StomaSoft™ të Blom-Singer (diagrami 2). Kjo pajisje
përmban një ltër shkume për shkëmbimin e nxehtësisë dhe lagështisë dhe një shufër tërthore
të heqshme. Qëllimi i shufrës tërthore është parandalimi i bllokimit të paqëllimshëm të rrugëve
të frymëmarrjes.
INDIKACIONE (Arsyet pse rekomandohet pajisja ose procedura)
Pajisja shërben për individë që kanë një trakeostomë të përhershme pas një ndërhyrjeje
kirurgjikale të laringektomisë totale. Pajisja ofron ajër të lagështuar dhe të ngrohur.
Pajisja mundëson të folurin pa komandim me duar te pacientët që kanë kryer punkturë
trakeoezofageale (vrimë e hapur nga kirurgu ndërmjet trakesë dhe ezofagut për vendosjen e një
proteze zëri) me vendosje të protezës së zërit.
KUNDËRINDIKACIONE (Arsyet që e bëjnë të pakëshillueshëm rekomandimin e pajisjes ose
procedurës)
Pajisja nuk duhet të vendoset mbi trakeostomë nëse prania e saj bllokon frymëmarrjen.
PARALAJMËRIME DHE MASA PARAPRAKE
Lani duart me ujë dhe sapun para dhe pasi të prekni pajisjen. Përdorni vetëm pecetë ose leckë
pa push te pajisja, në mënyrë që të shmangni hyrjen e pushit në rrugët e frymëmarrjes. Kjo
pajisje nuk është e ripërdorshme dhe mund të përdoret vetëm nga një pacient. Ekziston rrezik
infeksioni nëse kjo pajisje ndahet mes pacientëve ose ripërdoret. Kontrolloni pajisjen para çdo
përdorimi për dëmtime zike, si për shembull plasaritje ose çarje, dhe mos e përdorni nëse
është e dëmtuar. Nëse evidentohet dëmtim zik, kontaktoni Departamentin e Ankesave për
Produktet të “InHealth Technologies”.
Frymëmarrja
Frymëmarrja me anë të pajisjes duhet të jetë komode dhe e lehtë. Nëse frymëmarrja nuk është
e lehtë, hiqni pajisjen dhe konsultohuni me mjekun profesionist për ndihmë. Kur të lloni të
përdorni pajisjen, mund të ndihet një rritje e lehtë e rezistencës ndaj qarkullimit të ajrit. Kjo

rezistencë është një funksion i zakonshëm i pajisjes që mund të kërkojë një periudhë kohore për
t’u rregulluar. Mund të nevojitet heqja e kësaj pajisje gjatë rasteve të aktivitetit zik ekstrem dhe
frymëmarrjes së rënduar.
Kollitja
Kjo pajisje duhet të hiqet para se të kolliteni ose pastroni rrugët e frymëmarrjes.
Gjumi
Pajisja duhet të hiqet përpara gjumit për të shmangur bllokimin e mundshëm të rrugëve të
frymëmarrjes.
KOMPLIKACIONE TË MUNDSHME
Mund të vihen re sa më poshtë:
yVështirësi në frymëmarrje, e cila mund të shfaqet nëse pajisja nuk është ndërruar rregullisht
yKontaminim ose infeksion i stomës, i cili mund të kërkojë përdorimin e antibiotikëve të
përshtatshëm
yIrritim, duke përfshirë irritimin e lëkurës rreth stomës
UDHËZIME PËRDORIMI
Vendosja, heqja dhe kujdesi
Heqja dhe vendosja e çdo pajisjeje në trakeostomë duhet të kryhet vetëm pasi përdoruesi të
jetë pozicionuar para një pasqyre me një dritë të fortë të përqendruar direkt te stoma.
Mbajtësja e trakeostomës e Blom-Singer ose tubi i laringektomisë StomaSoft™ duhet të
vendoset llimisht mbi/rreth trakeostomës. Para se të vendosni ose hiqni pajisjen, rregullojeni
në pozicionin më të afërt të mbyllur, duke rrotulluar buzën e jashtme në drejtimin orar
(diagrami 3).
Vendosja
Për të vendosur pajisjen, kapni buzën e saj të jashtme dhe futeni pjesërisht në buzën e poshtme
të mbajtëses së paravendosur ose tubin e laringektomisë. Shtyni me kujdes pajisjen brenda
mbajtëses së ksuar ose tubit derisa të puthitet mirë në vend (diagrami 4). Mos e përdorni
shufrën tërthore si dorezë për të vendosur pajisjen.
Kujdesi
Kjo pajisje është parashikuar për përdorim gjatë ditës/kohës zgjuar dhe nuk është
parashikuar për t’u çmontuar, rimontuar ose ripërdorur. Nëse pajisja ndotet shumë, hiqeni
dhe zëvendësojeni me një pajisje të re. Mos përdorni asnjëherë peceta për fytyrën ose letër
higjienike për të hequr mukusin nga pajisja, pasi grimcat e pushit ose të leckës mund të
depozitohen në pajisje dhe mund të thithen në rrugët e frymëmarrjes gjatë rivendosjes. Mos e
lani pajisjen pasi kjo do të shkaktojë humbje të funksionit të saj.
Heqja
Për të hequr pajisjen, mbajeni mbajtësen e trakeostomës ose tubin e laringektomisë të palëvizur
me një dorë, ndërkohë që me dorën tjetër tërhiqni pajisjen derisa ajo të shkëputet (diagrami 5).
Mos e përdorni shufrën tërthore si dorezë për të hequr pajisjen.
Vendosja/heqja e shufrës tërthore
Për të vendosur shufrën tërthore opsionale, mbani pajisjen dhe shtypni shufrën tërthore
mbi pjesën e sipërme të pajisjes derisa të kërcasë në vend (diagrami 6). Për të hequr shufrën
tërthore, mbani pajisjen dhe tërhiqni shufrën tërthore nga pajisja derisa të shkëputet
(diagrami 7). Mos provoni të vendosni ose hiqni shufrën tërthore ndërkohë që pajisja është e
ksuar në trakeostomë.

Udhëzimet e përdorimit të HME SpeakFree™ të Blom-Singer®
Funksioni i pajisjes
Kur rregullohet saktë, valvula mbetet në pozicion të hapur gjatë frymëmarrjes së qetë normale
dhe gjatë aktivitetit zik rutinë. Ndërkohë që përdoruesi nxjerr frymë për të folur, qarkullimi
i ajrit e shtyn valvulën në pozicion të mbyllur për të siguruar izolimin për të folurin. Kur
përdoruesi ndalon së foluri ose merr frymë, valvula kalon në pozicionin e hapur të pushimit.
Kjo pajisje mund të bllokohet edhe në mënyrë manuale për të prodhuar të folur duke vendosur
gishtin e madh të dorës ose cilindo gisht tjetër mbi vrimën në pjesën e sipërme të pajisjes kur
shufra tërthore është e shkëputur. Me presionin tepër të lartë të kollës, valvula mund të shtyhet
pjesërisht përmes vrimës së pajisjes. Nëse ndodh kjo, hiqni pajisjen dhe ripozicionojeni valvulën
me majën e pastër të gishtit.
Rregullimi i ndjeshmërisë së pajisjes
Pajisja është parashikuar për të lehtësuar nevojat për të folur dhe frymëmarrje të çdo përdoruesi
individual duke rrotulluar unazën e jashtme të pajisjes në drejtim orar ose kundërorar. Rrotullimi
në drejtim orar rregullon valvulën që të mbyllet më lehtë (diagrami 3). Rrotullimi në drejtim
kundërorar rregullon valvulën duke kërkuar mundim më të madh (qarkullim i ajrit respirator)
që të mbyllet (diagrami 8). Vendosni pajisjen në pozicion më të mbyllur gjatë të folurit dhe
rregullojeni në pozicion më të hapur gjatë rritjes së përpjekjeve respiratore (p.sh. gjatë
ushtrimeve zike).
Për të rregulluar pajisjen ndërkohë që është e vendosur te mbajtësja e trakeostomës, përdoruesi
duhet llimisht të mbajë të ksuar bazën e mbajtëses me një dorë, ndërkohë që njëkohësisht
rrotullon unazën e jashtme me dorën tjetër (diagrami 9).
KUSHTE TËVEÇANTA PËR RUAJTJEN DHE/OSE TRAJTIMIN
Këto pajisje kanë kushte të veçanta për ruajtjen dhe/ose trajtimin:
yLani duart pas trajtimit të pajisjeve, pasi ato përmbajnë substanca që mund të shkaktojnë
irritim të lëkurës dhe syve.
yPaketimi ku është vendosur pajisja duhet të rivuloset pas hapjes për të ruajtur ekasitetin e
aftësive të shkëmbimit të nxehtësisë dhe lagështisë të pajisjes.
UDHËZIME PËR ASGJËSIMIN
Ky produkt nuk është i biodegradueshëm dhe duhet të konsiderohet i kontaminuar pas
përdorimit. Asgjësojeni me kujdes këtë pajisje duke ndjekur udhëzimet vendore.
