MicroCO Meter
Operating anual
I. Overview
The MicroCO eter is a hand held battery operated device used to
easure the concentration of carbon onoxide, CO, on the breath and
calculates the percentage of carboxyhae oglobin, %COHb, in the
blood.
It is accurate, easy to use, and has any features designed to si plify
its operation
.
These include: Auto zero function
S oking level colour light indicators
Poison level alar
Si ple calibration
Serial interface to PC
Please note: It is reco ended that this unit be calibrated
upon receipt
The device should only be used by trained and qualified personnel.

Introduction – English
The MicroCO eter is based on an electroche ical fuel cell sensor,
which works through the reaction of carbon onoxide (CO) with an
electrolyte at one electrode, and oxygen (fro a bient air) at the
other. This reaction generates an electrical current proportional to CO
concentration. Output fro the sensor is onitored by a
icroprocessor, which detects peak expired concentrations of alveolar
gas. This is then converted to % carboxyhae oglobin (%COHb) using
the athe atical relationships described by Jarvis et al

, for
concentrations below 90 pp , and by Stewart et al
2
for higher levels.
Raised levels of carboxyhae oglobin are ost co only caused by
accidental s oke inhalation/CO poisoning or cigarette s oking. The
MicroCO includes a countdown ti er as an aid to ti ing the breath
holding period prior to exhalation results are displayed on a clear LCD
display. Warning lights are provided to give an instant indication of
the s oking level.
The countdown ti er, warning light levels, and the alar level are
user adjustable when the unit is connected to a PC running COBRA
software.
Note: the countdown ti er, warning light levels, and the alar
level used throughout this anual are the factory settings and
ay have been changed.

S oke Inhalation/CO Poisoning
CO poisoning usually occurs as a result of s oke inhalation fro fires
or exposure to CO fro car exhausts or faulty heating syste s. CO
binds with hae oglobin to for carboxyhae oglobin (COHb) which
reduces the capacity of the blood to carry oxygen. Acute poisoning
ay cause sy pto s ranging fro headache and breathlessness (at
COHb levels of 10% to 30%) through confusion to co a and death
(COHb usually greater than 60%). A particularly insidious feature of
CO poisoning is the develop ent of neurological proble s such as
ove ent disorders (often rese bling Parkinson’s disease), e ory
loss and altered personality. Such proble s ay develop weeks after
apparent recovery fro acute poisoning.
Chronic exposure to relatively low levels of CO ay result in a variety
of sy pto s including headache, fatigue, poor concentration,
dizziness, palpitations, chest pain, visual disturbance, nausea,
diarrhoea and abdo inal pain.
Chronic CO poisoning as a cause of such sy pto s is often issed.
The MicroCO eter allows i ediate assess ent of patients at risk of
CO poisoning who can then be rapidly referred for expert assess ent.
Pro pt treat ent with oxygen (in a hyperbaric cha ber, if severe) is
often life saving. Screening for CO exposure ay also reveal the
cause of non-specific sy pto s relating to low level CO exposure
(usually as the result of faulty gas appliances).

Cigarette Consu ption and CO Measure ents
The MicroCO eter provides a si ple screening test for cigarette
consu ption for use in anti-s oking clinics and all s oking cessation
progra s.
Measure ent of carboxyhae oglobin has been well validated as an
indirect easure of cigarette consu ption and is widely used in
s oking cessation progra s.
Typical values for carboxyhae oglobin and expired CO in s okers and
poisoning victi s, together with the alar light status, are given
below:

Please note that so e urban areas ay have high environ ental
levels of CO. This can cause a rise in exhaled CO of a few pp above
that which is nor ally present on the breath. In these cases it is
possible for a non-s oker to appear at the botto of the ‘light
s oker’ range (7 – 10 pp ).

There are no known contraindications for using the MicroCO eter.
Warnings and Cautions

CAUTION: Read the anual before use
CAUTION: Do not atte pt to charge the batteries, connect
i properly, or dispose of in a fire as there is possibility of leakage or
explosion. Follow anufacturer’s reco endation for proper
disposal.
WARNING: The instru ent is not suitable for use in the presence
of explosive or fla able gases, fla able anaesthetic ixtures or
in oxygen rich environ ents.
CAUTION: Mouthpieces are single patient use. If used on ore than
one patient there is a risk of cross-infection. Repeat use ay
degrade aterials and lead to an incorrect easure ent.
PLEASE NOTE: The product and the battery you have
purchased should not be disposed of as unsorted waste.

PLEASE NOTE: Degree of protection against Ingress of Water is

CAUTION: When you connect the MicroCO to other equip ent,
always ake sure that the whole co bination co plies with the
international safety standard IEC 60601-1 for edical electrical
syste s. During easure ents, connect the MicroCO only to
co puters that co ply with IEC 60601-1 / ANSI/AAMI ES60601-
1:2005 / CAN/CSA-C22.2 No. 60601-1:14.
Intended Use
The MicroCO is intended to easure the a ount of Carbon Monoxide
(CO) that can be expired fro the breath. The expired CO is
easured in parts per illion and is used to calculate the percentage
of carboxyhae oglobin, %COHb, in the blood.
The MicroCO is used in s oking cessation clinics, GP’s surgeries,
accident and e ergency depart ents, and by the fire-fighting
services. In s oking cessation clinics it is used for instructional
purposes and to check on the client’s progress and co pliance.
General practitioners, A&E clinicians and the fire-fighting services can
use the instru ent to quickly assess the level of suspected CO
poisoning.
The MicroCO is designed for use by clinicians and health care
professionals.
Operation
For accurate results the CO eter should be used at roo
te perature. If the instru ent has been stored in cool or hot
conditions then allow ti e to reach roo te perature prior to use
Install the PP3 battery by sliding open the battery cover, clipping the
battery in place and replacing the cover. Insert the outhpiece

adapter into the MicroCO eter and then insert a disposable
outhpiece into the adapter. Turn the unit on by selecting the upper
or iddle position on the central slide switch and the display will show
the software version nu ber.
This will appear o entarily whilst correction for a bient levels is
executed. During this ti e the unit ust not be exposed to raised
concentrations of CO. The buzzer will then sound and the display will
change to:
Upon hearing the buzzer instruct the subject to inspire fully and hold
their breath for 20 seconds. The display will count down fro 20 to 0
as an aid to ti ing the breath holding period. The green indicator will
then illu inate and the display will change to:
BLOW
At this ti e the subject should seal their lips around the outhpiece
and exhale slowly and fully. The 20 second breath holding period is
reco ended to allow ti e for equilibration of alveolar gas.
If, however, the subject is unable to hold their breath for this period,
the unit ay still be used before 20 seconds have elapsed.

The unit ust not be used, however, for one second after the unit is
first switched on i.e. before the countdown co ences. Expired
alveolar gas is then entrapped between sensor and outhpiece valve.
The display reading will rise to a plateau over the course of a several
seconds. The final value will be held until the unit is turned off and
represents parts per illion CO or %COHb depending on the slide
switch position.
The lights will co e on according to the table on page 12 and the red
light will flash and an alar will sound if the easure ent rises above
72pp (12%COHb).
If this occurs then the possibility of CO poisoning should be
investigated as this level of CO is unlikely to be produced by cigarette
s oking.
I portant note:
Before repeating a easure ent the unit ust be turned off,
and the outhpiece and adapter re oved for at least 1 inute.
This is to allow re-equilibration with a bient air and to dry the
surface of the sensor. Visually inspect that all oisture has
evaporated fro the surface of the sensor before reuse.
If the unit is switched on again too quickly after use there ay be a
response to residual expired carbon onoxide fro the previous test.
In this case the display will show:
