THEINFORMATIONINFORMINGTHEUSEROFHIS/HERANDTHETELEPHONE
COMPANY'SRIGHTANDOBLIGATIONISOUTLINEDBELOW.
a)ThisequipmentcomplieswithPart68oftheFCCrulesandtherequirementsadopted
bytheACTA.ONthebottomofthisequipmentisa labelthatcontains,amongother
information,a productidentifierintheformatUS:AAAEQ##TXXXX.Ifrequested,
thisnumbermustbeprovidedtothetelephonecompany.
b)AllapplicablecertificationjacksUniversalServiceOrderCodes(USOC)forthe
equipmentisprovided(i.e.RJ11C)inthepackagewitheachpieceofapproved
terminalequipment.
c)A plugandjackusedtoconnectthisequipmenttothepremiseswiringandtele-
phonenetworkmustcomplywiththeapplicableFCCPart68rulesandrequirement
adoptedbytheACTA.A complianttelephonecordandmodularplugisprovidedwith
thisproduct.Itisdesignedtobeconnectedtoa compatiblemodularjackthatisalso
compliant.Seeinstallationinstructionsfordetails.
d)TheRENisusedtodeterminethenumberofdevicesthatmaybeconnectedtothe
telephoneline.ExcessiveRENsona telephonelinemayresultinthedevicesnot
ringinginresponsetoanincomingcall.Inmostbutnotallareas,thesumofRENs
shouldnotexceedfive(5.00).Tobecertainofthenumberofdevicesthatmaybe
connectedtoa line,asdeterminedbythetotalRENs,contactthelocaltelephone
company.[ForproductsapprovedafterJuly23,2001,theRENforthisproductis
partoftheproductidentifierthathastheformatUS:AAAEQ##TXXXX.Thedigits
representedby##aretheRENwithouta decimalpoint(e.g.,03isa RENof0.3).

e)Ifthisequipmentcausesharmtothetelephonenetwork,thetelephonecompanywill
notifyyouinadvancethattemporarydiscontinuanceofservicemayberequired.But
ifadvancenoticeisn'tpractical,thetelephonecompanywillnotifythecustomeras
soonaspossible.Also,youwillbeadvisedofyourrightstofilea complaintwiththe
FCCifyoubelieveitisnecessary.
f)Thetelephonecompanymaymakechangesinitsfacilities,equipment,operation
orproceduresthatcouldaffecttheoperationoftheequipment.Ifthishappensthe
telephonecompanywillprovideadvancenoticeinorderforyoutomakenecessary
modificationstomaintainuninterruptedservice.
