Wozu dient die Pessarbehandlung?
Was bewirkt ein T-Pessar ?
T-Pessare werden eingesetzt zur Behandlung bestimmter Formen der Inkontinenz
(unfreiwilliger Abgang von Harn oder Stuhl), Beckenbodendefekte (oft eine
Kombination aus Cystozele, Enterozele und Rektozele) sowie bei Beschwerden,
die durch eine Senkung von Scheide, Gebärmutter, Blase oder Darm entstehen.
Eine Cystozele tritt auf, wenn die sehr dünne Wand zwischen Blase und Scheide
zu schwach geworden ist, und die Blase teilweise in die Scheide sinkt. Dadurch
ist eine komplette Entleerung der Blase nicht mehr möglich und in den meisten
Fällen liegt auch eine Stressinkontinenz vor. Senkungen können z.B. durch eine
angeborene Bindegewebsschwäche, durch Hormonmangel nach den Wechsel-
jahren oder durch Schwangerschaften und Geburten hervorgerufen werden.
Mit Hilfe der Pessarbehandlung – unterstützt durch östrogenhaltige Cremes
und Beckenbodentraining – gelingt es häug die Beschwerden so zu bessern,
dass auf ein kleineres Pessar gewechselt oder sogar ganz auf einen Pessar
verzichtet werden kann.
Die zylindrische Form des T-Pessars repositioniert die Organe in die anatomisch
richtige Lage und wirkt dem Vorfall bzw. der Senkung von Scheide, Gebärmutter,
Blase oder Darm entgegen. Die Pessartherapie ist eine Alternative zur Operation
gegen Deszensus uteri und Inkontinenz. Speziell bei körperlicher Belastung, wie
z.B. schwerer Arbeit, Sport usw. hält das T-Pessar, Blase und Darm in der
anatomisch richtigen Lage.

Welches Pessar ist das richtige für mich?
medesign T-Pessare Typ B und Typ R werden in verschiedenen Größen angeboten
(siehe Tabelle). Das Pessar sollte in der kleinsten Größe gewählt werden, die
gerade noch sicher hält. Die Auswahl der richtigen Größe ist die Voraussetzung
für eine optimale Behandlung und Beschwerdefreiheit. Ihr Arzt wird die für Sie
richtige Größe ermitteln. Er setzt das Pessar ein und Sie probieren in seinem
Beisein aus, ob das Pessar richtig sitzt. Dazu pressen, husten und bewegen Sie
sich. Anschließend wird der Arzt nochmals den Sitz des Pessars kontrollieren.
Es sollte immer das kleinste Pessar benutzt werden, das gerade noch hält. Achten
Sie auch darauf, dass Sie bei eingesetztem Pessar problemlos Wasser lassen
können.
Nach einigen Wochen bzw. Monaten wird der Arzt überprüfen, ob evtl. auf eine
kleinere Pessargröße gewechselt werden kann. Durch die verschiedenen Größen
ist eine individuelle Anpassung möglich. Die speziell gestaltete Oberfläche
vermeidet weitgehend Druckstellen im Schleimhautbereich. T-Pessare gibt es
perforiert mit Bohrungen (Typ B) oder mit Rillen (Typ R).
medesign Pessare werden aus einem speziell für den medizinischen Gebrauch
entwickelten, flexiblen und gewebefreundlichen und geprüftem Silikon hergestellt.
Sollten Sie dennoch eine Empndlichkeit bemerken verwenden Sie das T-Pessar
nicht mehr und Besprechen die Situation mit Ihrem Arzt. Die Pessare sind flexibel
und lassen sich gut falten bzw. zusammendrücken. Dies macht das Einführen
und Entfernen der Pessare besonders einfach. Das Pessar darf nur von einer
Patientin verwendet werden.
Wie führe ich das T-Pessar ein?
Die Pessare lassen sich gut selbst einführen. Kontrollieren Sie das T-Pessar auf
Unversehrtheit, Sauberkeit, Verfärbungen, Schäden und Abnutzung. Bei sichtbaren
Schäden oder Veränderungen der Oberfläche ist das Pessar durch ein neues zu

ersetzen. Sie sollten das T-Pessar morgens in die Scheide einführen und abends
wieder entfernen. So kann sich das empndliche Scheidengewebe über Nacht
erholen. In besonderen Fällen kann das Pessar auch über Nacht eingesetzt
bleiben, bzw. nachts durch eine evtl. kleinere Größe ersetzt werden. Halten Sie
sich dabei an die Anweisungen Ihres Arztes.
Bevor Sie mit Einsetzen des Pessars beginnen, sollten Sie sich die Hände waschen.
Um das Einführen zu erleichtern hat sich die Verwendung eines Gleitmittels wie
z.B. Ladysoft bewährt. Östrogenhaltige Creme bzw. Gel tragen Sie längs des
Pessars nach Bedarf auf.
Nun führen Sie das Pessar wie ein Tampon mit dem Faden nach unten in die
Scheide ein. Dazu kann es hilfreich sein, ein Bein z.B. auf die Bettkante zu stellen.
Manche Frauen fällt es leichter, das Pessar in der Hocke oder im Liegen einzuführen.
Sie können sich auch mit leicht gespreizten Beinen an eine Wand lehnen und
dann das Pessar einführen. Probieren Sie ein bisschen, so nden Sie am besten
die für Sie vorteilhafteste Methode.
Schieben Sie das T-Pessar so weit wie möglich in die Scheide bzw. in die Position,
die Ihnen Ihr Arzt gezeigt hat. Das Pessar sitzt richtig, wenn es nicht drückt.
Wie entferne ich das T-Pessar?
Auch vor dem Entfernen des Pessars sollten Sie sich die Hände waschen. Benetzen
Sie den Scheidenbereich mit Ladysoft Gleitgel. Ziehen Sie dann zunächst das
T-Pessar mit Hilfe des Fadens soweit nach unten bis Sie einen leichten Widerstand
der Beckenbodenmuskulatur spüren. Dazu gehen Sie im besten in die Hocke
und ziehen Sie leicht bis mäßig am Faden. Mit Hilfe des Fadens kann das T-Pessar
ganz aus der Scheide entfernt werden (wie bei einem Tampon). Verwenden Sie
keine anderen Hilfsmittel bei der Entfernung des T-Pessars.

Was tun, wenn der Faden reißt?
Reinigung des Pessars?
Sollte der Faden ausnahmsweise reißen, können Sie das Pessar nur mit den
Fingern entfernen. Sollte dies nicht möglich sein, lassen Sie das Pessar in den
nächsten Tagen von Ihrem Arzt entfernen.
Nachdem Sie das T-Pessar entfernt haben reinigen Sie es mit lauwarmen Wasser
und Seifenlösung oder einer milden Waschlotion wie z.B. Femsoft. Anschließend
klar reinspülen und trocknen lassen. Eine Desinfektion ist normalerweise nicht
nötig. Nach Reinigung des Pessars waschen Sie sich nochmal die Hände.
Zur unterstützenden Reinigung und Pflege des Intimbereiches verwenden Sie
z.B. Femsoft Intim Waschlotion. Eine speziell entwickelte Waschlotion auf der
Basis von Molke und Brennnessel mit einem niedrigen pH-Wert (3,8 bis 4).
Anschließend waschen Sie sich nochmals die Hände.
Nicht mit chemischen Mitteln reinigen.
Keine Haushaltsreiniger oder Spülmittel verwenden.
Es ist nicht erforderlich dass Pessar auszukochen.
Legen Sie es nicht in die Mikrowelle.
Entsorgung
medesign Pessare sind aus geprüftem medizinischem Silikon hergestellt und
können im Hausmüll entsorgt werden Das Silikon ist nicht biologisch abbaubar,
daher sollte es nicht über die Toilette entsorgt werden.

Kontraindikatoren zur Pessaranwendung sind
Infektionen von Vagina und Zervix.
Anwendungshinweise
· Lesen und beachten Sie die Gebrauchsanweisung.
· Anwendung nach Anleitung durch medizinisches Fachpersonal.
· Ununterbrochener Anwendungszeitraum ohne ärztliche Kontrolle:
maximal 29 Tage.
· Max. Anwendungszeitraum mit ärztlicher Kontrolle:
2 bis 3 Zyklen à maximal 29 Tage.
· Mehrfachverwendung bei gleicher Patientin ist möglich.
· Die Verträglichkeit und Richtigkeit der Pessarlage ist durch Ultraschall
und / oder Befragung der Patientin sicherzustellen.
· Vor jeder Anwendung ist das Produkt auf Unversehrtheit und Sauberkeit
der Oberfläche zu prüfen. Bei Veränderungen der Oberfläche ist das Pessar
durch ein neues zu ersetzen.
· Nach jeder Anwendung ist das Pessar mechanisch mit lauwarmem
Wasser und Seifenlösung zu reinigen und anschließend klar reinspülen.
· Die Anwendungszyklen sind vom Arzt zu dokumentieren.
· Nicht während der Menstruation und nie zusammen mit einem Tampon
verwenden oder mit ärztlicher Rücksprache.
· Nicht verwenden wenn Sie schwanger sind oder glauben, schwanger zu
sein.
· Nicht bei Blasen-, Harnweg-, oder Scheidenentzündung verwenden.
· medesign Pessare sind latexfrei.

When is therapy with a pessary useful?
How does the T-Pessary work?
T-Pessaries are used for treating certain types of incontinence (inability to control
urination or defecation), pelvic floor disorders (often a combination of cystocele,
enterocele and rectocele) as well as problems caused by descensus of the vagina,
the uterus, the bladder or the intestine. A cystocele appears when the very thin
wall between the bladder and the vagina becomes too weak and the bladder
falls down into the vagina. This makes the complete emptying of the bladder
impossible and mostly presents itself together with stress incontinence. Descents
like this can be caused by e.g. hereditary weakness of the connective tissue,
postmenopausal hormone deciency or pregnancy and giving birth. Treatment
with pessaries - combined with pelvic floor exercise and, if necessary, oestrogen
enriched creams - can permanently improve conditions and allow changing to
a smaller sized pessary or even totally obviate the need for a pessary.
The cylindric shape of the T-Pessary repositions the organs and brings them
into the anatomically correct position – counteracting a prolapse or descensus
of the vagina, the uterus, the bladder or the intestine. Therapy with pessaries is
an alternative to surgical intervention for the treatment of descensus uteri and
incontinence. The T-Pessary keeps the bladder and the intestine in their anato-
mically correct position - particularly during physical strain such as hard work,
doing sports, etc.

Which pessary is the right one for me?
medesign’s T-Pessaries Type B and Type R are offered in various sizes (see
table). The pessary, which is smallest in size, yet resting securely should be
chosen. Selecting the correct size is a prerequisite for ideal treatment and freedom
from symptoms. Your gynaecologist will determine the right size for you.
The pessary is inserted and you can check its correct position by moving, coughing
and squeezing, whereupon the gynaecologist will check the t of the pessary
once again. The pessary, which is smallest in size, yet resting securely should
be chosen. You should also make sure that you are able to urinate without any
problems.
Your gynaecologist will assess the t of your pessary after a few weeks or months
to see whether you can switch to a different size. The various sizes allow for
individual tting. The specically designed surface prevents pressure marks on
the mucous membranes to the greatest extent. T-Pessaries are available perforated
(Type B) or corrugated (Type R).
medesign pessaries are made from flexible, tissue tolerant silicone, which is
especially developed for medical use. If you should, despite this, notice a reaction,
stop the use of the T-Pessary and discuss the situation with your physician. The
pessaries are flexible and can thus be folded and squeezed, making insertion
and removal especially easy. The pessary may be used by one patient only.
How do I insert the T-Pessary?
Pessaries can easily be inserted on your own. Check the T-Pessary for intactness,
cleanliness, discolouration, damage and wear marks. The pessary has to be
replaced by a new one should damage or changes on the surface be visible.
You should insert the T-Pessary into the vagina in the mornings and remove it
in the evenings in order for the sensitive vaginal tissue to be able to recover
overnight. In certain cases the pessary may remain inserted during the night or

be replaced by a smaller pessary at night. Please follow your doctor’s instructions
for this.
Wash your hands prior to inserting the pessary. To ease insertion it is recommended
that a lubricant, such as Ladysoft, be used. If needed, the T-Pessary may be
coated lengthwise with a cream or gel containing oestrogen.
Now insert the pessary into the vagina like a tampon with the string hanging
down. It may be helpful to place one leg e.g. on the edge of your bed. Some
women nd it easier to insert the pessary in a crouching or lying position. You
could also lean against a wall with your legs slightly spread and then insert the
pessary. Try out which method is best for you.
Push the T-Pessary as far into the vagina as possible or into the position shown
to you by your physician. The pessary is placed correctly if you cannot feel any
pressure.
How do I remove the T-Pessary?
You should also wash your hands prior to removal. Thoroughly lubricate your
vaginal area with Ladysoft Lubricant. Use the attached string to pull the T-Pessary
downwards until you feel a slight resistance from the pelvic floor muscles. It is
best to crouch down and gently pull the string into different directions. Using
the string (like with a tampon), the T-Pessary can be removed from the vagina.
Do not use any other equipment for removing the T- Pessary.

What do I do if the string tears?
Pessary Care
You can use your ngers to remove the pessary, in the exceptional case of a torn
string. If you are not able to remove the pessary, let your physician remove it
within the next few days.
Clean the T-Pessary with lukewarm water and soap or a mild washing lotion
such as Femsoft after removal. Subsequently rinse and leave to dry. Disinfection
is usually not necessary. Wash your hands again after cleaning the pessary.
For added support for the cleaning and care of you intimate area, use e.g. Femsoft
Intimate Wash – a specially developed washing lotion with whey and stinging
nettle as a basis and a low pH-value (3.8 – 4). Subsequently wash your hands
again.
Do not use chemicals for cleaning.
Do not use household or dish detergents.
It is not necessary to boil the pessary.
Do not put it in the microwave.
Disposal
medesign pessaries are made from certied medical silicone and can be disposed
of in household waste. The silicone is not biodegradable and should therefore
not be disposed of in the toilet.

Infections of the vagina and the cervix are
contraindications for pessary treatment.

· Uninterrupted period of application without medical supervision:

· Maximum period of application with medical supervision:
2 – 3 cycles of 29 days maximum.
· Repeated use with the same patient possible.
· Tolerance and correct placement of the pessary have to be secured
through ultrasound and / or consultation.
· The product has to be checked for intactness and cleanliness of the
surface prior to every application. The pessary has to be replaced by a new
one should changes on the surface appear.
· The pessary has to be cleaned after every application – using lukewarm
water and soap and subsequently rinsing the pessary with clear water.
· The periods of application have to be documented by a physician.
· Do not use during menstruation and never use the pessary together with a
tampon or without medical consultation.
· Do not use during pregnancy or if you think you might be pregnant.
· Do not use if you have a bladder or urinary tract infection or inflammation
of the vagina.

A quoi sert le traitement avec pessaire?
Comment agit un T-pessaire?
Les T-pessaires sont introduits pour traiter certaines formes d’incontinence
(fuites involontaires d’urine our de selles), défauts du plancher pelvien (souvent,
une combinaison de cystocèle, d’entérocèle et rectocèle) et des douleurs qui
proviennent d’un affaissement du vagin, de l’utérus, de la vessie ou de l’intestin.
Une cystocèle intervient lorsque la très ne paroi entre la vessie et l’utérus est
trop affaiblie et la vessie s’affaisse partiellement dans le vagin. Une vidange
complète de la vessie n’est ainsi plus possible et il y a aussi alors dans la plupart
des cas une incontinence d’effort. Les affaissements peuvent être provoqués
par exemple par une faiblesse congénitale du tissu conjonctif, par un manque
d’hormones après la ménopause ou par des grossesses et des accouchements.
A l’aide du traitement avec pessaire, renforcé par des crème oestrogéniques et
une rééducation du plancher pelvien, on parvient souvent à améliorer les
symptômes de telle sorte qu’on peut ensuite utiliser un pessaire plus petit ou
même y renoncer complètement.
La forme cylindrique du T-pessaire repositionne les organes dans la bonne position
anatomique et corrige la pathologie c’est à dire l’affaissement du vagin, de l’utérus,
de la vessie ou de l’intestin. La thérapie par le pessaire est une alternative à
l’opération pour corriger une descente d’utérus et l’incontinence. En particulier
en cas d’effort physique comme par exemple un dur travail, du sport etc…, le
T-pessaire maintient la vessie et l’intestin dans la bonne position anatomique.
