This package contains small parts which, before assembly, present a choking hazard to
children under the age of three.
• Fall hazard. Children have suffered serious injuries after falling from changing tables.
Falls can happen quickly.
• STAY in arm’s reach of your child.
• ALWAYS secure this pad to the support surface by correctly using anchoring straps.

• ALWAYS use restraint system when using this product.
• Suffocation hazard. Babies have suffocated while sleeping on changing pads.
Changing Pads are not designed for safe sleep.
• NEVER allow child to sleep in changing area.
• Do not use with child over 30 lbs.

• This pad is for use on a level, stable and structurally sound surface with minimum
surface dimensions of 16 in. x 32 in.

Instructions for Assembly and Use:
Parts Included: Changing Pad, Screws (2), Washers (2)
Tool Needed: Phillips Screwdriver
Attach the changing pad to the changing surface by using the following instructions:
1. Place changing pad on top of changing table or dresser with the anchoring straps to the back
of the changing surface ensuring the pad does not hang over any edge. Make sure the back of
the changing table or dresser is clean and dry.
2. Slide one washer onto each screw and feed through the hole in the anchoring straps. With the
changing pad in position, use a screwdriver to attach the anchoring straps to the back of the
changing table or dresser or to the wall by screwing in the mounting screws. The anchoring
straps have been placed 16” apart so they can easily be attached to the wall’s studs.
3. Periodically check the anchoring straps to ensure they remain tight and securely attached to
the changing table or dresser.
4. To remove the pad from the changing table or dresser, unscrew the mounting screws and
store in a safe place.
How to properly use the restraint system:
Position the infant on their back and centered on the changing pad, bring the safety strap around from
both sides of the pad so they align over their abdomen and buckle securely. Adjust and tighten the
restraint strap to fit snugly around child’s waist by pulling the loose end of the safety strap away from
the buckle. To release the safety strap, pull the buckle off of the child’s skin and simply squeeze the
tabs together on both sides of the buckle. Never leave child unattended when using your changing pad,
even with all straps secured. Always keep one hand on the child to prevent falling. Always use enclosed
restraint system when using this pad.
Care & Maintenance
Wipe clean with a damp cloth and mild soap.
