Concerning Customer Service Stan ar s

AT&T places the highest priority on provi ing you with the best customer service. We have establishe general
U-verse TV service stan ar s for customer care that not only meet applicable legal requirements but are esigne
to excee customer expectations. Liste below are general customer service stan ar s we inten to meet.

Appointment Hours for Installations an Service Calls
The appointment window alternatives for installations, service calls, an other installation activities that we provi e
will be, at a maximum, a four-hour time block uring normal business hours. We may sche ule service calls an
other installation activities outsi e of normal business hours for your express convenience. We will not cancel an
appointment with you after 5 p.m. on the business ay before the sche ule appointment. If our representative is
running late for an appointment with you an will not be able to keep the appointment as sche ule , we will
contact you an you will be offere the next available two (2) hour appointment win ow convenient for you.
Installation Intervals
A stan ar installation ate is available within seven (7) business ays after the or er has been place . Customers
may also choose from available ates an times outsi e of the stan ar interval.
We will provi e all estimate costs for repairs or service prior to any work.
During your or er process, sales representatives will isclose applicable installation service charges an
promotional pricing. If any a itional service or repair work nee s to be performe once your technician is
on-site, he/she will state orally the estimate cost of any a itional service. You must authorize the charges
before service is elivere or work is performe .
We will han le service repairs promptly.
Exclu ing con itions beyon our control, we will begin working on U-verse TV service interruptions promptly, but
no later than 24 hours after the interruption is reporte by you or becomes known to us. Service interruption
means the loss of picture or soun on one or more channels. We will take action to correct other service issues
the next business ay after being notifie of the problem. We will interrupt your TV service only if necessary.
We will notify you of any changes in rates or eletions in programming.
When it is within our control, we will provi e you with a minimum of thirty (30) ays written notice before increasing
rates, changing channel position, or eleting programming. If an increase in rates, changing channel position, or
eletion of programming is outsi e of our control, we will make reasonable efforts to provi e as much notice
as possible.
We will clearly outline how eposits, bill isputes, an termination of service will be han le .
Deposits
Any eposits or a vance payments may be applie towar any unpai balance on your bill, or as otherwise set
forth in your Terms of Service. Interest will not be pai on eposits or a vance payments unless require by law.
Upon our sole etermination of satisfactory payment history, or as require by law, we may begin refun ing
eposits or a vance payments through bill cre its, cash payments, or as otherwise permitte by law.
Bill Inquiries & Disputes
If you believe you have been bille in error for services, please notify us within sixty (60) ays of the billing ate
by contacting Customer Care at 800.288.2020. We will respon within 30 (thirty) ays. Refun s an cre its will be
issue promptly. We will not, except as require by law, issue refun s or cre its if you fail to notify us with sixty
(60) ays of the billing ate.

Customer Service Standards

Termination of Service
You may terminate your service by calling us at 800.288.2020. If AT&T propose to terminate your service, we will
provi e you fifteen (15) ays notice an will terminate your service only on a ay you can contact us. We will
restore your service without charge if it was terminate in error.
Return of Equipment
Upon termination of services, for whatever reason, you must return any equipment provi e by AT&T, un amage ,
within twenty one (21) calen ar ays. If the equipment is not returne within twenty one (21) calen ar ays, or is
returne amage , you will be charge for the value of the equipment. If the equipment is returne within ninety
(90) ays of termination, any fees charge for the equipment will be refun e (other than fees for amages). No
refun s will be ma e for any equipment returne more than ninety (90) ays after termination.
We will provi e accurate, un erstan able bills.
We will provi e you with an itemize , complete monthly bill for your service that is accurate an easy to un er-
stan . You shoul remit payment by the ue ate in icate on your bill, which will be at least fifteen (15) ays
from the ate of billing. If payment is not receive by the in icate ue ate, a late fee may be assesse no earlier
than twenty-two (22) ays after billing. Payment may be ma e by mail, online at att.com, an at most AT&T stores
an Western Union locations.

AT&T U-VERSE® TV AND AT&T PHONE
GENERAL TERMS OF SERVICE
Effective March 2017
1. GENERAL AGREEMENT
The following Terms of Service (referre to as “TOS”) are between you, the customer, an one of the following
AT&T companies, epen ing upon your service a ress: Southwestern Bell Telephone Company; Pacific Bell
Telephone Company; Illinois Bell Telephone Company; In iana Bell Telephone Company, Incorporate ;
Michigan Bell Telephone Company; Neva a Bell Telephone Company; The Ohio Bell Telephone Company;
Wisconsin Bell, Inc.; or BellSouth Telecommunications, LLC (each in ivi ually an collectively referre to as
“AT&T”). The TOS constitute a legal ocument that etails your rights an obligations as a purchaser of AT&T
Phone service (f/k/a AT&T U-verse Voice) an /or AT&T U-verse TV service (in ivi ually an collectively referre
to as “Services”). If you purchase AT&T Phone service, your TOS inclu e the General Terms of Service set forth
herein, an the attache Sche ule 1. If you purchase AT&T U-verse TV service, your TOS inclu e the General
Terms of Service set forth herein an the attache Sche ule 2. Your TOS also inclu e the Acceptance Form
for Terms of Service for Purchase an Use of AT&T Phone service an /or AT&T U-verse TV provi e to you
when Services are installe . Your TOS will continue to apply to your Services when they are transferre from
one location to another.
PLEASE READ THIS AGREEMENT CAREFULLY TO ENSURE THAT YOU UNDERSTAND EACH PROVISION.
THIS AGREEMENT REQUIRES THE USE OF ARBITRATION ON AN INDIVIDUAL BASIS TO RESOLVE
DISPUTES, RATHER THAN JURY TRIALS OR CLASS ACTIONS, AND ALSO LIMITS THE REMEDIES
AVAILABLE TO YOU IN THE EVENT OF A DISPUTE.
You must accept this TOS as a con ition of receiving the Services. For purposes of these TOS, “you” an
“your” refer to the person purchasing the Services. “We,” “our,” “us,” refer to AT&T.
AT&T will comply with all applicable fe eral, state an local laws, to the extent that such laws apply to AT&T
an its obligations un er the TOS. If there is any conflict between the TOS an such applicable law, such
applicable law controls. These conflicts coul inclu e, but are not limite to, fees an charges for service,
billing an payments, notices, an your rights an reme ies.
Legal Authority. You must be at least 18 years of age to purchase the Services as an in ivi ual or to accept
these TOS as an authorize representative for the person or entity who purchases the Services. By accepting
these TOS, you confirm you are an a ult of at least 18 years of age. If you are an entity, by accepting these
TOS, you confirm (through your uly authorize representative) that you are a corporation, partnership, or
other legal entity uly forme (an incorporate if applicable) in goo stan ing where require to o business
with all legal authority an power to accept these TOS; an you are also confirming that these TOS constitute
a vali an bin ing obligation of yours. All use of the Services, whether or not authorize by you, shall be
eeme for your use. You are responsible for ensuring that all use of the Services complies with these TOS.
By enrolling in, activating, using, or paying for the Services, you agree to the terms an con itions in these
TOS, inclu ing but not limite to the prices, charges, an terms an con itions provi e to you in marketing
an informational materials associate with the Services an on the AT&T U-verse website, all of which are
incorporate herein by reference. If you o not agree to all of the aforementione terms an con itions, o
not use the Services, an cancel the Services imme iately by calling AT&T at 800.288.2020.
Updates. These TOS may be up ate or change from time to time. You can review the most current version
of these TOS at any time at: att.com/uversetermsofservice. If AT&T makes a change to these TOS an that
change has a material impact on the Services, you will be provi e notice of that change, an such notice
will be provi e consistent with Notice provisions of this TOS. Your continue use of the Services following
such notice constitutes your acceptance of those changes.
Special Arrangements. Some customers may receive the Services through a special arrangement with their
property owner or manager. If you have such an arrangement, these TOS shall apply to the Services, except
that AT&T may not irectly charge you for Services (inclu ing Equipment) provi e to you as part of the special
arrangement, an the Equipment return provisions may not apply to you even though Equipment remains
AT&T-owne . You will be responsible for fees an charges associate with a itional Service or ers. You may
have an a itional agreement or contract with your property owner or manager that covers any applicable
special arrangement. Any such a itional agreement or contract is outsi e these TOS an AT&T is not
responsible for nor boun by the terms of any agreement you may have with your property owner or manager.
If the special arrangement with your property owner or manager terminates, you will continue receiving
Service un er stan ar billing terms an these TOS unless you notify AT&T.

2. ACCEPTABLE USE AND PRIVACY POLICIES
Use of the Services is subject to the AT&T Acceptable Use Policy (available at http://www.corp.att.com/aup),
which is incorporate herein by reference. Once you have purchase the Services you will have an account with
AT&T (“AT&T Account”). Your AT&T Account will inclu e information applicable to the Services inclu ing but not
limite to billing information an charges relate to the Services (whether recurring or one-time). If you have, or
later obtain, an AT&T Access ID, you are subject to the AT&T Access ID Terms an Con itions (available at
http://www.att.com/accessi terms), which are incorporate herein by reference. The AT&T Privacy Policy, which is

online at http://www.att.com/privacy, a resses AT&T’s use of account information an other information specific to
your use of AT&T Services.
AT&T U-verse Services are provi e for your non-commercial personal use only, an for your enjoyment in a
private resi ential welling/office unit. You agree not to repro uce, uplicate, copy, sell, transfer, tra e, resell or
exploit for any commercial purposes any portion of the Services, use of the Services, or access to the Services.
Further, you agree that the AT&T U-verse TV service will not be viewe in areas open to the public or in
commercial establishments an that your AT&T U-verse TV service may not be rebroa cast, nor performe , nor
may a mission be charge for listening to or viewing AT&T U-verse TV service.
3. INSTALLATION/SERVICE
You will be responsible for payment of service charges for visits by AT&T or its subcontractors to your premises
when a service request results from causes not attributable to AT&T or its subcontractors, inclu ing, but not
limite to, when you are unwilling to complete troubleshooting steps requeste by AT&T. You will provi e AT&T
an its subcontractors with reasonable access to your premises in or er to install, maintain, an repair the
Service an you authorize any other a ult resi ent or guest at your resi ence to grant access to your premises
for these purposes. You un erstan an agree that AT&T may rill, cut, an otherwise alter improvements on
the premises (inclu ing walls, flooring, an /or other surfaces) in or er to install, maintain, or repair the Service.
If you o not own your premises or your unit is part of a multi-tenant environment (e.g., apartment buil ing,
con ominium, private sub ivision), you warrant that you have obtaine permission from any necessary party,
inclu ing but not limite to the owner, lan lor , or buil ing manager, to allow AT&T an its subcontractors
reasonable access to install, maintain, an repair the Service an to make any alterations AT&T eems
appropriate for the work to be performe .
You acknowle ge that AT&T may use existing wiring, inclu ing altering the wiring an removing accessories,
locate within your unit (“Insi e Wiring”). You warrant that you own or control the Insi e Wiring, an give
AT&T permission to use, alter, an remove equipment from, such wiring. Without limiting any other provisions
of this TOS, you agree to in emnify AT&T from an against all claims by an owner, lan lor , buil ing manager,
or other party in connection with installation, maintenance, repair, or provision of the Services.
4. FEES AND CHARGES
Agreement to Pay. You agree to pay all fees an charges for the Services associate with your AT&T Account,
inclu ing recurring an nonrecurring charges, taxes, fees, surcharges, an assessments applicable to the
Services, associate equipment, installation an maintenance, an inclu ing all usage an other charges
associate with your account. For a list of a itional charges an fees that coul apply to the Service, please
see Sche ule 3 (Fee Sche ule), which is incorporate by reference. In or er to provi e you with the Services,
AT&T may pay taxes, fees, an surcharges to municipalities an other governmental entities, which AT&T may
pass on to you.
Late Payment Charge and Dishonored Chec or Other Instrument Fee. You agree that for each bill not pai
in full by the payment ue ate, a late payment charge of $9 will be assesse , an if your Service is suspen e
for non-payment, we may charge you a Restoral Fee to resume Service. You will be charge a fee of up to
$30 ( epen ing on applicable law) for any check or other instrument (inclu ing cre it car charge backs)
returne unpai for any reason.
Unpaid Past Due Charges and Consent to Contact. In the event you fail to pay AT&T or AT&T is unable to bill
charges to your cre it car , AT&T may assign unpai late balances to a collections agency. You expressly
authorize, an specifically consent to allowing, AT&T an /or its outsi e collection agencies, outsi e counsel,
or any other agents acting by or on behalf of AT&T to contact you with informational messages regar ing
your account, inclu ing but not limite to contact in connection with any an all matters relating to unpai
past ue charges bille by AT&T to you. You agree that such contact may be ma e to any mailing a ress,
telephone number, cellular phone number, e-mail a ress, or any other electronic a ress that you have
provi e , or may in the future provi e, to AT&T an to any an all telephone numbers bille on your account.
You expressly consent an agree that such contact may be ma e using, among other metho s, pre-recor e
or artificial voice messages elivere by an automatic telephone ialing system, text messages elivere by

an automate system, pre-set e-mail messages elivere by an automatic e-mailing system, or any other
pre-set electronic messages elivere by any other automatic electronic messaging system. You agree to
provi e true, accurate, current an complete contact information to AT&T an its authorize agents an to
promptly up ate your contact information to keep it true, accurate an complete.
Changes to Fees & Charges. If you signe up for Services for a specifie term, you agree that if you cancel
your plan before the en of the term, you will pay any applicable Early Termination Fee. At the conclusion of
your term, AT&T will automatically begin charging the applicable month-to-month fee. If you purchase the
Services as part of a bun le offering with one or more other pro ucts an are receiving a iscount base
upon that bun le offering, your iscount may cease an you may be bille the stan ar monthly rate for
the Services if you change or isconnect one or more of the services in the applicable bun le. AT&T may,
upon notice require by applicable laws, at any time change the amount of or basis for etermining any fee
or charge or institute new fees or charges.
Data Usage. Use of certain services, inclu ing but not limite to AT&T U-verse TV features an apps, AT&T
Digital Life, home security, home automation an me ical alarm systems, will count towar s your internet
usage allotment. For more information about the use of your resi ential High Spee Internet Service an the
ata plans that may apply to your service, how much ata you use, an management of your ata usage,
please refer to www.att.com/internet-usage.
5. BILLING AND PAYMENTS
Credit Card Authorization. You may be aske to provi e us with a vali email a ress an a cre it car
number from a car issuer that we accept in or er to activate your Services. You hereby authorize AT&T to
charge an /or place a hol on your cre it car with respect to any unpai charges relate to the Services.
You authorize the issuer of the cre it car to pay any amounts escribe herein without requiring a signe
receipt, an you agree that these TOS are to be accepte as authorization to the issuer of the cre it car to
pay all such amounts. You authorize AT&T an /or any other company who bills pro ucts or services, or acts
as billing agent for AT&T to continue to attempt to charge an /or place hol s with respect to all sums
escribe herein, or any portion thereof, to your cre it car until such amounts are pai in full.
You agree to provi e AT&T with up ate cre it car information upon AT&T's request an any time the
information you previously provi e is no longer vali . You are solely responsible for maintaining an up ating
the cre it car information. Without limiting the applicability of any other provisions of this TOS, you
acknowle ge an agree that neither AT&T nor any AT&T affiliate company will have any liability whatsoever
for any non-sufficient fun s or other charges incurre by you as a result of such attempts to charge, an /or
place hol s on, your cre it car . If you mistakenly provi e a ebit car number, instea of a cre it car
number, you authorize all charges escribe herein to be applie to such ebit car unless an until you
provi e a cre it car number. In the event you are enrolle , or later enroll, in an automatic payment or
electronic fun s transfer plan, you agree that all sums escribe herein may be charge , at AT&T's option,
to the account number provi e for such automatic payment or electronic fun s transfer plan.
Advance Payments, Deposits, Fees and Limits. We may require you to make eposits or a vance payments for
Services, which we may use to satisfy your initial bill for Services, to offset against any unpai balance on
your account, or as otherwise set forth in these TOS or permitte by law. Interest will not be pai on a vance
payments or eposits unless require by law. We may require a itional a vance payments or eposits if we
etermine that the initial payment was ina equate. Upon etermination solely by AT&T of satisfactory payment
history or as require by law, AT&T may begin refun ing of the eposit or a vance payment through bill cre its,
cash payments, or as otherwise etermine solely by AT&T. Base on your cre itworthiness, a non-refun able
fee may be require to establish service an we may require you to enroll, an remain enrolle , in an
automatic payment or electronic fun s transfer plan. We may establish a itional limits an restrict service
or features as we eem appropriate. If your account balance goes beyon the limit we set for you, we may
imme iately interrupt or suspen service until your balance is brought below the limit. Any charges you incur
in excess of your limit become imme iately ue.
Payment Cycle and Cancellation. Billing for the Services commences when AT&T has provisione the Services.
Recurring charges for each month's Services will be bille one month in a vance. Billing is base on a 30- ay
cycle. Non-recurring an usage-base charges for the Services generally will be bille in the billing cycle
following the transaction. Your first bill for Services may inclu e pro-rate charges for a partial monthly perio
prior to the beginning of your first monthly billing cycle. Upon termination you will be charge for the pro-rate
number of ays for which you ha Services in that billing cycle an , if applicable, you will receive a cre it for
any balance of payments for Services bille in a vance. A owngra e fee may apply if you make changes to
your Service within thirty (30) ays of Service provisioning or later programming or ers.

Terms of Service

Method of Billing/Payment. Fees an charges for the Services will be bille to your AT&T Account. You will
receive an online bill for the Service, unless you specifically notify us that you want to receive a paper bill for
the Services (at 800.288.2020). You must register online to establish a personal AT&T My Account an provi e
a billing email a ress. You will then be able to view an pay your bill online by logging on to your personal
AT&T My Account (username an passwor require ). You un erstan that you have sole responsibility for the
security of your passwor an you are solely responsible for notifying AT&T if your passwor is lost or stolen.
AT&T is not liable for any claims, costs, amages, or expenses arising from a lost, misplace , or stolen passwor .
If you forgot your passwor or want to change your passwor for any reason, you may request to reset your
passwor online. It is your responsibility to notify AT&T imme iately if your contact information changes.
Bill Inquiries and Refunds. If you believe you have been bille in error for the Services, please notify us within
60 ays of the billing ate by contacting Customer Service (800.288.2020). AT&T will not issue refun s or
cre its after the expiration of this 60- ay perio , except where require by law or regulation. Any amounts
refun e in the form of bill cre its, cash payments or any other form shall be inclusive of all applicable taxes,
fees an surcharges that were originally pai on such amounts. Cre it amounts, such as customer loyalty
rewar s, that o not represent a refun of, or a iscount to, the price pai for any goo or service will not
result in the refun of any tax, fee, or surcharge previously pai by the customer.
Refunds. You authorize AT&T to use outsi e payment processing agencies or other companies for purposes of
paying any refun owe to you, an you further authorize AT&T to sell, assign or otherwise transfer its refun
rights an obligations un er this Agreement to outsi e payment processing agencies or other companies. You
agree that we or the outsi e payment processing agency or other company that is responsible for your refun
may etermine in our or, if applicable, their sole an absolute iscretion the form of any refun that we issue
to you un er this Agreement, an such form may inclu e a cre it on your next statement, a check, or a prepai
ebit car that may be subject to monthly service fees not to excee $2.95 per month an that are e ucte
from the amount of the refun .
Promotions and Contingent Benefits. You may receive or be eligible for certain iscounts, features, promotions,
an other benefits associate with your purchase of the Services as offere to you in marketing an informational
materials, on the AT&T U-verse website, or in other materials (“Benefits”). Any an all such Benefits are provi e
to you so long as you continue to meet qualification requirements; provi e , however, such Benefits may be
mo ifie or terminate at any time as set forth in these TOS or if you change your Services after installation.
Unless otherwise set forth in Benefits materials, stan ar monthly rates will be charge at the conclusion of the
Benefits perio or when you no longer qualify for the Benefits.
6. EQUIPMENT
Equipment provi e by AT&T may be new or fully inspecte an teste . Any equipment or software that was
not provi e to you by AT&T, inclu ing batteries, is not the responsibility of AT&T, an AT&T will not provi e
support, or be responsible for ongoing maintenance of such equipment. Depen ing on your service a ress,
your Services will inclu e one of the following Equipment configurations:
a. A Wi-Fi® Gateway (“WG”) locate insi e your premises, an certain service-specific equipment set
forth in Sche ule 1 an /or Sche ule 2 that is require for the Services to function (the WG an service-
specific equipment herein collectively referre to as “Equipment”). If you o not purchase the Equipment
from AT&T, you agree to pay a monthly equipment fee for the Equipment as part of your purchase of
the Services for the uration of your receipt of the Services. Equipment fees may be inclu e in your
monthly charge for the Services or be charge separately ( ifferent taxes an surcharges may apply
to the equipment fees, Service fees, an /or the equipment fee portion of the Service fees). Equipment
fee/purchase options epen on the AT&T U-verse Services you or er an installation options you
choose. The Equipment requires electrical power from your premises to operate, which you are
responsible for provi ing.
b. If you have an WG insi e your premises, you may also have an Optical Network Terminal (“ONT”),
which is a box that may be locate insi e your premises, on the outsi e of your premises, in a central
location in a multi-tenant buil ing, or in your garage, where AT&T’s fiber network terminates. The ONT
also requires electrical power from your home to operate, which you are responsible for provi ing. AT&T
will install your ONT evice. The ONT power supply box converts the AC power in your home to the DC
power require by the ONT.
c. If you o not have an WG locate insi e your premises, your service is provi e by an Intelligent

therein collectively referre to as “Equipment”). If you o not purchase the Equipment from AT&T, you
agree to pay a monthly equipment fee for the Equipment as part of your purchase of the Services for
the uration of your receipt of the Services. Equipment fees may be inclu e in your monthly charge

for the Services or be charge separately. Equipment fee/purchase options epen on the AT&T
U-verse Services you or er an the installation options you choose. The iNID inclu es three components:
(1) a unit typically locate on the outsi e of your premises or in your garage where the AT&T network
terminates (the outsi e unit); (2) a home networking hub, which provi es wireless networking capability
an is locate insi e your premises, (the insi e unit); an , (3) a power supply unit, typically locate in a
sheltere area either insi e your premises or in an attache structure. You are responsible for provi ing
the electrical power for the iNID.
Battery Bac up for WG. It is your responsibility to provi e for your own battery backup. You may choose to
purchase battery backup for your WG from thir party manufacturers or retailers. For more information an
minimum specifications visit att.com/batterybackup also see Sche ule 1, section IV, for more information on
Power Outages an No Battery Backup.
Battery Bac up for ONT. It is your responsibility to provi e for your own battery backup. You may choose to
purchase battery backup for your ONT from thir party manufacturers or retailers. You agree to rea an
follow all manufacturer or ven or irections for the replacement an recycling of battery backup. For more

Battery Bac up for iNID. If there is an iNID at your premises, AT&T provi e the initial battery backup. All
subsequent battery backups are the responsibility of the customer at the premises. For more information an

Power Outages an No Battery Backup.
AT&T reserves the right to manage the AT&T installe Equipment uring the time you are an AT&T customer
an retains exclusive rights to ata generate by the Equipment. Neither you nor a thir party may change,
interfere with, or block access to the Equipment ata or settings. AT&T will repair or replace amage
Equipment as AT&T eems necessary. You un erstan that repair or replacement of the Equipment may
elete store content, reset personal settings, or otherwise alter the Equipment. If the Equipment was
amage ue to your intentional acts, negligence, or use inconsistent with the TOS as etermine by AT&T,
you will be responsible for the price of repair or replacement. Any tampering with the Equipment, inclu ing,
for example, opening an attempting to mo ify the Equipment, or attempting to connect the Equipment to
other har ware, will be treate as amage ue to your intentional acts or negligence. You agree that you will
use the Equipment only for its inten e resi ential use, an not for any other purpose (such as on another
AT&T network, or on another provi er’s (non-AT&T) network). You agree to use appropriate an reasonable
care in using any an all Equipment.
AT&T will not provi e support for, or be responsible for, ongoing maintenance or management of, customer-
owne equipment, inclu ing the battery backup equipment use by AT&T customers. For more information
an minimum specifications visit att.com/batterybackup.
Return of Equipment. Upon termination of the Services for whatever reason, you must return the Equipment,
un amage , within 21 calen ar ays to AT&T. If the Equipment is not returne within 21 calen ar ays, or is
returne amage , you will be charge for the value of the Equipment. We may retain any a vance payment
or eposit, or portion thereof that previously ha not been refun e , if you fail to return the Equipment
within this time perio . If the Equipment is returne within 90 ays of termination, any fees charge for the
Equipment will be refun e (other than fees for amages). No refun s will be ma e for any Equipment
returne more than 90 ays after termination. In a ition to termination of service, these return of equipment
provisions apply if your existing equipment is replace or upgra e for any reason.
7. INDEMNITY
You agree to in emnify an hol AT&T an its subsi iaries, affiliates, irectors, officers, agents, an employees
harmless from any claim, eman , action, citation, or legal procee ing, inclu ing, but not limite to, those
arising out of or resulting from the eath or bo ily injury of any person, or the amage, loss, or estruction
of any real or tangible personal property, or for reasonable attorneys’ fees (except as provi e in paragraph
11(e) below), ma e by any party against AT&T, its subsi iaries, affiliates, irectors, officers, agents, an
employees arising out of or relate to your use of or inability to use the Services, your connection to the
Services, the provisioning or allege failure to provision the Services, a violation of any provision of this TOS,
or your violation of any rights of another.
8. INTERRUPTIONS, LIMITATIONS, AND MODIFICATIONS TO SERVICE
Service may be temporarily interrupte or otherwise limite for a variety of reasons; some beyon the control
of AT&T. AT&T reserves the right to refuse cre it allowances for interruptions of Service. AT&T also reserves
the right to mo ify or iscontinue, temporarily or permanently, at any time an from time to time, the Services

(or any function or feature of the Services or any part thereof) without liability. You acknowle ge that AT&T
may establish general practices an limits concerning use of the Services, inclu ing without limitation, the
limits set forth in the attache Sche ule 1 an Sche ule 2.
IP Networ Interruptions. You acknowle ge an un erstan that the Services will not function in the event of
an IP network interruption.
9. ACCOUNT SECURITY
Customer Duty. You agree to keep confi ential all passwor s, user IDs, IP a resses, an other account
i entifiers an are solely responsible for any liability or amages resulting from your failure to maintain that
confi entiality. You are solely an fully responsible an liable for all activities that occur un er your AT&T
Account, passwor , user ID, or IP a ress. You agree to: (a) imme iately notify AT&T if you suspect any breach
of security such as loss, theft, public use (unrestricte , open, communal or share use by thir parties
unrelate an /or not affiliate with the Customer either for profit or not for profit) or unauthorize isclosure
or use of your AT&T Account, passwor , user ID, or any cre it or charge car number provi e to AT&T by
calling 800.288.2020; (b) ensure you exit from your account as applicable at the en of each session; an (c)
perio ically change your passwor .
Account Access.
You authorize AT&T to provi e information about an to make changes to your AT&T Account,
inclu ing a ing new service, upon the irection of any person able to provi e information we eem sufficient
to i entify you.
Assumption of Ris . There is a risk that other users may attempt to access your Services, such as through the
Internet or connecte networks. You acknowle ge this risk as inherent to the share nature of the Services
an you agree to take full responsibility for taking a equate security precautions an safeguar ing your ata.
Theft of AT&T Equipment or Service. You agree to notify AT&T imme iately, in writing or by calling the AT&T
customer support line, if the Equipment is stolen or if you become aware at any time that Services are being
stolen or frau ulently use . When you call or write, you must provi e your AT&T Account number an a
etaile escription of the circumstances of the Equipment theft, inclu ing ocumentation of theft (e.g., a
copy of a police report) or stolen or frau ulent use of the Services. You will be responsible for all charges
incurre on your AT&T Account until you report the theft or frau ulent use of the Services. You will be
responsible for stolen Equipment, however, AT&T may in its sole iscretion waive or re uce charges for stolen
Equipment upon submission of ocumentation of theft or other circumstances. Failure to provi e notice to
AT&T of theft in a timely manner may result in the termination of your Services an a itional charges to you.
Unless notifie otherwise by AT&T, after you report the theft or frau ulent use of the Services, you will remain
responsible for paying your monthly fees for Services not stolen or frau ulently use .
10. SUSPENSION AND TERMINATION
Reduction/Suspension/Termination by AT&T. Your Services may be re uce , suspen e or terminate if your
payment is past ue. AT&T may also re uce, suspen or terminate your Services if it is etermine that there
is previously unpai , un ispute an outstan ing ebt for U-verse service. Such re uction, suspension or
termination may continue until satisfactory arrangements have been ma e for the payment of all past unpai
charges. When your Service is suspen e for non-payment, you will be place in a minimum service package
with re uce programming, for a one-time flat fee of $6.00. While your Services are suspen e you will not
receive automatic cre it balances (if any are ue) an billing will continue for your monthly charges, an any
applicable promotional offers may be iscontinue an revoke as etermine solely by AT&T. If AT&T
re uces or suspen s your Service for non-payment, you must pay all past ue amounts in or er to resume
Service at any level above the minimum service package. In a ition, to resume Service at any level above
the minimum service package you must also pay an account Restoral Fee of $49. The Restoral Fee will be
assesse on the next monthly bill you receive following the resumption of Service from the minimum service
package. In a ition, AT&T may imme iately terminate all or a portion of your Service or re uce or suspen
Service, without notice, for con uct that AT&T believes (a) is illegal, frau ulent, harassing, abusive, or inten e
to intimi ate or threaten; (b) constitutes a violation of any law, regulation, or tariff (inclu ing, without applicable
policies or gui elines (inclu ing the Acceptable Use Policy), an AT&T may refer such use to law enforcement
authorities without notice to you. Termination of suspension or re uction by AT&T of the Services also
constitutes termination or suspension (as applicable) of your license to use any Software, if applicable.
Contacts to Terminate Service. You may terminate the Services at any time by calling 800.288.2020. You must
pay service fees an other charges incurre through the termination ate, inclu ing any early termination
fees that apply. If you elect not to purchase your Equipment, you may also be charge the value of any
Equipment that is not returne in accor ance with Section 6.

11. DISPUTE RESOLUTION WITH AT&T BY BINDING ARBITRATION
PLEASE READ THIS CAREFULLY. IT AFFECTS YOUR RIGHTS.
Most customer concerns can be resolve quickly an to the customer's satisfaction by calling AT&T at
800.288.2020. In the unlikely event that AT&T’s customer service epartment is unable to resolve a
complaint you may have to your satisfaction (or if AT&T has not been able to resolve a ispute it
has with you after attempting to o so informally), we each agree to resolve those isputes through
bin ing arbitration or small claims court instea of in courts of general juris iction. Arbitration is
more informal than a lawsuit in court. Arbitration uses a neutral arbitrator instea of a ju ge or jury, allows
for more limite iscovery than in court, an is subject to very limite review by courts. Arbitrators can awar
the same amages an relief that a court can awar . Any arbitration un er this Agreement will take place on
an in ivi ual basis; class arbitrations an class actions are not permitte . For any nonfrivolous claim that
oes not excee $75,000, AT&T will pay all costs of the arbitration. Moreover, in arbitration you are entitle
to recover attorneys' fees from AT&T to at least the same extent as you woul be in court.
In a ition, un er certain circumstances (as explaine below), AT&T will pay you more than the amount of the
arbitrator’s awar an will pay your attorney (if any) twice his or her reasonable attorneys’ fees if the arbitrator
awar s you an amount that is greater than what AT&T has offere you to settle the ispute.
Arbitration Agreement:

• claims arising out of or relating to any aspect of the relationship between us, whether
base in contract, tort, statute, frau , misrepresentation or any other legal theory;
• claims that arose before this or any prior Agreement (inclu ing, but not limite to,
claims relating to a vertising);
• claims that are currently the subject of purporte class action litigation in which you
are not a member of a certifie class; an
• claims that may arise after the termination of this Agreement.
References to “AT&T”, “you,” an “us” inclu e our respective subsi iaries, affiliates, agents, employees,
pre ecessors in interest, successors, an assigns, as well as all authorize or unauthorize users or
beneficiaries of Services un er this or prior Agreements between us. Notwithstan ing the foregoing,
either party may bring an in ivi ual action in small claims court. This arbitration agreement oes not
preclu e you from bringing issues to the attention of fe eral, state, or local agencies. Such agencies
can, if the law allows, seek relief against us on your behalf. YOU AGREE THAT, BY ENTERING INTO
THIS AGREEMENT, YOU AND AT&T ARE EACH WAIVING THE RIGHT TO A TRIAL BY JURY OR TO
PARTICIPATE IN A CLASS ACTION. This Agreement evi ences a transaction in interstate commerce,
an thus the Fe eral Arbitration Act governs the interpretation an enforcement of this provision. This
arbitration provision shall survive termination of this Agreement.
b. A party who inten s to seek arbitration must first sen to the other, by certifie mail, a written Notice
of Dispute (“Notice”). The Notice to AT&T shoul be a resse to: Office for Dispute Resolution, AT&T,
1025 Lenox Park Blv ., Atlanta, GA 30319 (“Notice A ress”). The Notice must (1) escribe the nature
an basis of the claim or ispute; an (2) set forth the specific relief sought (“Deman ”). If AT&T an you
o not reach an agreement to resolve the claim within 30 ays after the Notice is receive , you or AT&T
may commence an arbitration procee ing. During the arbitration, the amount of any settlement offer
ma e by AT&T or you shall not be isclose to the arbitrator until after the arbitrator etermines the
amount, if any, to which you or AT&T is entitle .

c. After AT&T receives notice at the Notice A ress that you have commence arbitration, it will promptly

fee, AT&T will pay it irectly upon receiving a written request at the Notice A ress.) The arbitration will
be governe by the Commercial Arbitration Rules an the Supplementary Proce ures for Consumer
Relate Disputes (collectively, “AAA Rules”) of the American Arbitration Association (“AAA”), as mo ifie
by this Agreement, an will be a ministere by the AAA. The AAA Rules are available online at
http://www.a r.org, by calling the AAA at 800.778.7879, or by writing to the Notice A ress. (You may
obtain information that is esigne for non-lawyers about the arbitration process at
att.com/arbitration-information.) The arbitrator is boun by the terms of this agreement. All issues are

provision are for the court to eci e. Unless AT&T an you agree otherwise, any arbitration hearings will

that you may choose whether the arbitration will be con ucte solely on the basis of ocuments
submitte to the arbitrator, through a telephonic hearing, or by an in-person hearing as establishe by
the AAA Rules. If your claim excee s $10,000, the right to a hearing will be etermine by the AAA
Rules. Regar less of the manner in which the arbitration is con ucte , the arbitrator shall issue a
reasone written ecision sufficient to explain the essential fin ings an conclusions on which the
awar is base . Except as otherwise provi e for herein, AT&T will pay all AAA filing, a ministration an
arbitrator fees for any arbitration initiate in accor ance with the notice requirements above. If, however,
the arbitrator fin s that either the substance of your claim or the relief sought in the Deman is frivolous
or brought for an improper purpose (as measure by the stan ar s set forth in Fe eral Rule of Civil
Proce ure 11(b)), then the payment of all such fees will be governe by the AAA Rules. In such case,
you agree to reimburse AT&T for all monies previously isburse by it that are otherwise your obligation
to pay un er the AAA Rules. In a ition, if you initiate an arbitration in which you seek more than
$75,000 in amages, the payment of these fees will be governe by the AAA rules.
. If, after fin ing in your favor in any respect on the merits of your claim, the arbitrator issues you an
awar that is greater than the value of AT&T’s last written settlement offer ma e before an arbitrator
was selecte , then AT&T will:
• pay you the amount of the awar or $10,000 (“the alternative payment”), whichever is
greater; an
• pay your attorney, if any, twice the amount of attorneys’ fees, an reimburse any
expenses (inclu ing expert witness fees an costs) that your attorney reasonably
accrues for investigating, preparing, an pursuing your claim in arbitration (“the
attorney premium”).
If AT&T i not make a written offer to settle the ispute before an arbitrator was selecte , you an your
attorney will be entitle to receive the alternative payment an the attorney premium, respectively, if
the arbitrator awar s you any relief on the merits. The arbitrator may make rulings an resolve isputes
as to the payment an reimbursement of fees, expenses, an the alternative payment an the attorney
premium at any time uring the procee ing an upon request from either party ma e within fourteen
(14) ays of the arbitrator's ruling on the merits.
e. The right to attorneys’ fees an expenses iscusse in paragraph ( ) supplements any right to
attorneys' fees an expenses you may have un er applicable law. Thus, if you woul be entitle to a
larger amount un er the applicable law, this provision oes not preclu e the arbitrator from awar ing
you that amount. However, you may not recover uplicative awar s of attorneys' fees or costs.
Although un er some laws AT&T may have a right to an awar of attorneys' fees an expenses if it
prevails in an arbitration, AT&T agrees that it will not seek such an awar .
f. The arbitrator may awar eclaratory or injunctive relief only in favor of the in ivi ual party seeking
relief an only to the extent necessary to provi e relief warrante by that party's in ivi ual claim. YOU
AND AT&T AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR
ITS INDIVIDUAL CAPACITY, AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED
CLASS OR REPRESENTATIVE PROCEEDING. Further, unless both you an AT&T agree otherwise, the
arbitrator may not consoli ate more than one person’s claims, an may not otherwise presi e over any
form of a representative or class procee ing. If this specific provision is foun to be unenforceable, then
the entirety of this arbitration provision shall be null an voi .
g. Notwithstan ing any provision in this Agreement to the contrary, we agree that if AT&T makes any
future change to this arbitration provision (other than a change to the Notice A ress) uring the
perio of time that you are receiving Services, you may reject any such change by sen ing us written
notice within 30 ays of the change to the Arbitration Notice A ress provi e above. By rejecting any
future change, you are agreeing that you will arbitrate any ispute between us in accor ance with the
language of this provision.
12. SOFTWARE
The Services use an inclu e certain software an /or firmware (the “Software”). Some Software resi es on
the Equipment.
End User License Agreement. If you ownloa e or installe Software, your use of that Software is subject to
the En User License Agreement that accompanie that Software. With regar to any Software (inclu ing

Software upgra es, changes, or supplements) which is not accompanie by an En User License Agreement,
AT&T, or its applicable thir party licensors, grants you a limite , personal, nontransferable, an nonexclusive
right an license to use the object co e of its Software on the Equipment; provi e that you o not (an o not
allow any thir party to) copy, mo ify, create a erivative work of, reverse engineer, reverse assemble, or
otherwise attempt to iscover any source co e or structure, sequence an organization of, sell, assign,
sublicense, istribute, rent, lease, grant a security interest in, or otherwise transfer any right in the Software. You
acknowle ge that this license is not a sale of intellectual property an that AT&T or its thir -party licensors,
provi ers, or suppliers continue to own all right, title, an interest to the Software an relate ocumentation.
The Software is protecte by the copyright laws of the Unite States an international copyright treaties.
Export Limits.
You shall comply with all export laws an restrictions an regulations of the Department of
Commerce, the Unite States Department of Treasury Office of Foreign Assets Control (“OFAC”), or other Unite
States or foreign agency or authority, an shall not export, or allow the export or re-export of the Software in
violation of any such restrictions, laws or regulations. By ownloa ing or using the Software, you agree to the
foregoing an represent an warrant that you are not locate in, un er the control of, or a national or resi ent
of any restricte country or on any such list.
Restricted Rights. The Software is provi e with RESTRICTED RIGHTS. Use, uplication, or isclosure by the
Government is subject to restrictions as set forth in subparagraphs (a) through ( ) of the Commercial
Computer Restricte Rights clause at 48 CFR 52.227-19 when applicable, or in subparagraph (c)(1)(ii) of The
Rights in Technical Data an Computer Software clause of DFARS 252.227-7013 an in similar clauses in the
NASA FAR Supplement.
Non-AT&T Software, Services or Applications. Your use of the Services may also inclu e access to an use of
software, services an /or applications which interact with the Services an which are provi e by non-AT&T
thir parties, an , when applicable, those thir -parties terms an con itions apply to your access to an use
of such non-AT&T software, services an /or applications. AT&T is not liable to you for any loss or injury arising
out of or cause , in whole or in part, by your use of any such software, services, an /or applications accesse
through, or in conjunction with, the Service.
NOTICE ABOUT AUTOMATIC SOFTWARE UPGRADES. AT&T, or its applicable thir -party licensors may provi e
Software upgra es, up ates, or supplements (such as, but not limite to, a ing or removing features or
up ating security components). You un erstan an agree that AT&T, or the applicable thir -party licensor,
have the unrestricte right, but not the obligation, to upgra e, up ate, or supplement the Software on the
Equipment at any time. Although unlikely, Software upgra es, up ates, or supplements coul reset your
Equipment an erase save preferences an store content.
13. DISCLAIMER OF WARRANTIES
YOU EXPRESSLY UNDERSTAND AND AGREE THAT:
1. YOUR USE OF THE SERVICES IS AT YOUR SOLE RISK. THE SERVICES ARE PROVIDED ON AN “AS IS” AND “AS
AVAILABLE” BASIS. AT&T EXPRESSLY DISCLAIMS ALL WARRANTIES OF ANY KIND, WHETHER EXPRESS OR
IMPLIED, INCLUDING BUT NOT LIMITED TO THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR
A PARTICULAR PURPOSE AND NONINFRINGEMENT.
2. AT&T MAKES NO WARRANTY THAT (i) THE SERVICES WILL MEET YOUR REQUIREMENTS, (ii) THE SERVICES
WILL BE UNINTERRUPTED, TIMELY, SECURE, OR ERROR-FREE, (iii) THE RESULTS THAT MAY BE OBTAINED
FROM THE USE OF THE SERVICES WILL BE ACCURATE OR RELIABLE, (iv) THE QUALITY OF ANY PRODUCTS,
SERVICES, INFORMATION, OR OTHER MATERIAL PURCHASED OR OBTAINED BY YOU THROUGH THE
SERVICES WILL MEET YOUR EXPECTATIONS, OR (v) THE SERVICES WILL NOT CONFLICT OR INTERFERE WITH
OTHER SERVICES FROM AT&T OR THIRD PARTIES THAT YOU RECEIVE AT YOUR PREMISES.
3. NO ADVICE OR INFORMATION, WHETHER ORAL OR WRITTEN, OBTAINED BY YOU FROM AT&T OR THROUGH
OR FROM THE SERVICES WILL CREATE ANY WARRANTY NOT EXPRESSLY STATED IN THESE TOS.
14. LIMITATION OF LIABILITY
YOU EXPRESSLY UNDERSTAND AND AGREE THAT, UNLESS PROHIBITED BY LAW, AT&T SHALL NOT BE LIABLE
TO YOU FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, PUNITIVE OR EXEMPLARY DAMAGES,
INCLUDING, BUT NOT LIMITED TO, DAMAGES FOR PERSONAL INJURY, PROPERTY DAMAGE, LOSS OF REVENUE
OR PROFITS, BUSINESS OR GOODWILL, USE, DATA, OR OTHER INTANGIBLE LOSSES (EVEN IF AT&T HAS BEEN
ADVISED OF THE POSSIBILITY OF SUCH DAMAGES), RESULTING FROM: (a) USE OF THE SERVICES (WHICH
INCLUDES EQUIPMENT, SOFTWARE, AND INSIDE OR OUTSIDE WIRING), (b) THE PERFORMANCE OR NONPER-
FORMANCE OF THE SERVICES, (c) THE INSTALLATION, MAINTENANCE, REMOVAL, OR TECHNICAL SUPPORT OF
THE SERVICES, EVEN IF SUCH DAMAGE RESULTS FROM THE NEGLIGENCE OR GROSS NEGLIGENCE OF AN
AT&T INSTALLER, TECHNICIAN, OR OTHER REPRESENTATIVE, ( ) ANY INABILITY TO REACH 911 EMERGENCY
SERVICES, ANY ALLEGED INTERFERENCE WITH ALARM OR MEDICAL MONITORING SIGNALS, OR ANY FAILURE

OF ALARM OR MEDICAL MONITORING SIGNALS TO REACH THEIR INTENDED MONITORING STATIONS ALLEGEDLY

IN ANY EVENT, YOUR SOLE AND EXCLUSIVE REMEDY FOR ANY DISPUTE WITH AT&T IN CONNECTION WITH
THE SERVICE IS A REFUND NOT TO EXCEED THE TOTAL AMOUNT OF SERVICE FEES PAID DURING THE
IMMEDIATELY PRECEEDING TWELVE MONTH PERIOD.
15. EXCLUSIONS AND LIMITATIONS

All portions of the Services an Equipment an any firmware or software use to provi e the Services or
provi e to you in conjunction with provi ing the Services, or embe e in the Equipment, an all Services,
information, ocuments, an materials on relate website(s) are the property of AT&T or thir -party provi ers
an are protecte by tra emark, copyright, or other intellectual property laws an international treaty provisions.
All websites, corporate names, service marks, tra emarks, tra e names, logos, an omain names (collectively
“Marks”) of AT&T or thir -party provi ers are an shall remain the exclusive property of AT&T or thir -party
provi ers, an nothing in this Agreement shall grant you the right or license to use such Marks.
18. CREDIT REPORTING AUTHORIZATION
As permitte un er applicable laws an without limitation to other rights provi e in these TOS or other
applicable policies, you authorize AT&T to (a) isclose your account information, inclu ing your payment
history an confi ential information, to cre it reporting agencies or private cre it reporting associations, an
(b) perio ically obtain an use your cre it report an other cre it information from any source in connection
with AT&T's offering of the Services an other services. You un erstan that if you fail to fulfill the terms of
your obligations un er these TOS, AT&T may report your failure to a cre it reporting agency.
19. ASSIGNMENT
AT&T may assign these TOS an its rights an obligations pertaining to the provision of the Services, or parts
thereof, to a parent or affiliate company without notice to you. You may not assign these TOS or your rights
or obligations pertaining to the Services or any parts thereof without the written consent of AT&T.
20. MISCELLANEOUS PROVISIONS
1. These TOS o not provi e any thir party with a reme y, claim, or right of reimbursement.
2. These TOS, any policies, gui elines, or other ocuments reference herein, the provisions set forth in any
marketing an informational materials or promotional offers for the Services, an the terms an con itions
poste on the AT&T U-verse website constitute the entire agreement between AT&T an you an
superse e any prior agreements between you or AT&T with respect to the subject matter of these TOS.
3. These TOS an the relationship between you an AT&T will be governe by the law of the state of your
billing a ress except to the extent such law is preempte by or inconsistent with applicable fe eral law.
In the event of a ispute between us, the law of the state of your billing a ress at the time the ispute
is commence , whether in litigation or arbitration, shall govern except to the extent that such law is
preempte by or inconsistent with applicable fe eral law.
4. The failure of AT&T to exercise or enforce any right or provision of these TOS will not constitute a waiver
of such right or provision.
5. If any provision of these TOS is foun by a court or agency of competent juris iction to be unenforceable,
the parties nevertheless agree that the remaining provisions of these TOS shall remain in full force an
effect. The foregoing oes not apply to the prohibition against class or representative actions that is part
of the arbitration clause; if that provision is foun to be unenforceable, the arbitration clause (but only

AT&T Phone is a resi ential enhance voice communication service that converts voice communications into
Internet Protocol (IP) packets that are carrie over AT&T's IP network. It may be generically referre to as “voice
over IP” or “VoIP.” The Service inclu es irect- iale calling an certain calling an call management features or
a vance features associate with the Service, inclu ing a itional features or a vance features which may be
offere at a itional costs, all of which AT&T, in its sole iscretion, may a , mo ify, or elete from time to time.
The Service also inclu es a telephone number or numbers that will be inclu e in printe irectories an /or
irectory assistance atabases, an options, available at a itional costs, to have numbers withhel from
printe irectories an /or irectory assistance atabases. The Service is not available for business use. It is not
mobile or noma ic an will function only in your home.
When you accept these TOS, you become the main account hol er for each telephone number assigne to
the AT&T Phone service an all plans, features, an functionalities associate with each telephone number,
whether those telephone numbers, plans, features, an functionalities are purchase initially or are a e
subsequently. These TOS apply to all such telephone numbers, an to the Service an the plans, features, an
functionalities associate with the Service, for both the main account an all sub accounts. You will be aske
to choose a unique name for the main account (your main account ID).
Subaccounts. You may create up to ten subaccounts un er your main account, for others in your househol
(each subaccount will have a separate passwor an ID). Main account hol ers are responsible for all activity
on their main account an on any an all subaccounts. Violations of these TOS in a main account or in a
subaccount can result in suspension or termination of the main account an all associate subaccounts. Call
histories (call logs for outgoing, answere , an misse calls) for each telephone number are accessible in the
main account an in each subaccount create un er the telephone number. Main account hol ers can reset
subaccount passwor s an IDs by contacting Customer Service an can elete an recreate subaccounts.
You agree to a vise all subaccount hol ers that the main account hol er can have access to all aspects of
their subaccount, inclu ing, but not limite to, feature settings, voicemail messages, an a ress books. All
subaccount hol ers can therefore have no expectation of privacy vis-à-vis the main account hol er with
regar to any aspect of the subaccount.
II. Billing An Payments
For AT&T Phone service, nonrecurring an usage-base charges generally bille in the billing cycle following
the transaction inclu e, but are not limite to, international calling (inclu ing surcharges for international
termination to a wireless phone number), Operator Services, Directory Assistance (411 or xxx.555.1212), call
trace, an overage minutes associate with efine minutes-of-use plans (e.g. Phone200 plan). Partial
minutes are roun e up for per-minute usage charges.
III. Service-Specific Equipment
AT&T Phone service requires a regular touchtone lan line telephone, which you must supply an which must
be connecte to the WG or iNID, either irectly or through your home’s insi e wiring. (Rotary an pulse phones
will not work). The WG or iNID will support up to two AT&T Phone lines (telephone numbers use for inboun
an outboun calling).
You agree that neither you nor a thir party will move Equipment use for AT&T Phone service within your
premises or to any other physical location outsi e of the premises where it was installe by AT&T. AT&T Phone
service is not esigne to be noma ic an will not function properly if the WG is move or altere by a non-
AT&T employee. If you require the WG to be move , you must contact AT&T. Failure to o so may result in a
failure of the Service an /or in AT&T’s termination of your Service.
IV. Interruptions, Limitations, An Mo ifications to Service
Since voice over IP is epen ent on the IP network, the availability of an a equate power source, an correct
Equipment configuration, AT&T oes not guarantee that AT&T Phone service will be continuous or error-free.
You acknowle ge an un erstan that AT&T cannot guarantee that voice over IP communication is
completely secure.
You also acknowle ge that AT&T may establish general practices an limits concerning use of the AT&T
Phone service, inclu ing without limiting other limitations set forth in these TOS or otherwise, AT&T Phone
service cannot be use to make operator-assiste collect or thir -party billing calls (Note: a AT&T Phone

customer can make a Collect call through a thir party Collect Call company that separately han les an
bills for the Collect call), nor can AT&T Phone service be use to make 900/976 calls; area co e 500, 700,
an 710 calls; 10-10-XXX ial-aroun calls; or international operator or irectory assistance calls. Also, the
ability to call certain N11 services (211, 311, 511) may not be available.
AT&T also limits the maximum number of ays that messages will be retaine ; the maximum number messages
that will be retaine by the Service; the maximum size of any message; an the maximum isk space that will
be allotte on AT&T's servers on your behalf. You agree that AT&T will have no responsibility or liability for the
eletion, for failure to store or to eliver any messages an other communications, for the mo ification or
malformation of communications over the AT&T Phone service, or for other content maintaine or transmitte
by AT&T Phone service. You acknowle ge that AT&T reserves the right to log off accounts or isconnect
sessions that are inactive for an exten e perio of time. You further acknowle ge that AT&T reserves the right
to change these general practices an limits at any time without a vance notice.
If you also purchase AT&T U-verse TV call history, information for all misse an answere calls can be
isplaye on your TV screen an cannot be PIN protecte . Call history for iale calls cannot be isplaye
on your TV screen. Also, Caller ID information may be isplaye on your TV screen at the time you purchase
AT&T Phone an AT&T U-verse TV or in later iterations of the Services.
Power Outages and No Battery Bac up.
YOU ACKNOWLEDGE AND UNDERSTAND THAT AT&T PHONE REQUIRES
ELECTRICAL POWER TO FUNCTION. AT&T DOES NOT PROVIDE BATTERY BACK UP FOR YOUR SERVICE. YOU MAY
CHOOSE TO PURCHASE BATTERY BACKUP FOR YOUR iNID, WG, IAD* AND YOUR ONT (IF YOU HAVE ONE) FROM
THIRD PARTY MANUFACTURERS OR RETAILERS. YOU ACKNOWLEDGE AND UNDERSTAND THAT IT WILL TAKE TIME
TO CHARGE AN INITIAL BATTERY BACKUP AFTER AT&T PHONE IS INSTALLED AND/OR AFTER A REPLACEMENT
BATTERY IS INSTALLED. YOU ACKNOWLEDGE AND UNDERSTAND THAT TO CONSERVE BATTERY POWER DURING
A POWER OUTAGE, YOU SHOULD NOT ATTEMPT TO USE THE BATTERY BACKUP FOR ANY PURPOSE OTHER THAN
TO POWER YOUR AT&T PHONE SERVICE (OR TO POWER YOUR INTERNET CONNECTION, FOR THE PURPOSE OF
POWERING YOUR PREMISES ALARM, IF YOU HAVE AN IP-BASED PREMISES ALARM THAT USES AT&T INTERNET).
YOU ACKNOWLEDGE AND UNDERSTAND THAT THE BATTERY BACKUP DOES NOT PROVIDE POWER FOR
CORDLESS PHONES AND THAT, IF YOU ARE USING A CORDLESS PHONE WITH YOUR AT&T PHONE SERVICE, A
SEPARATE BATTERY BACKUP OR OTHER POWER SOURCE MAY BE REQUIRED IF THERE IS A POWER OUTAGE. YOU
ACKNOWLEDGE AND UNDERSTAND THAT YOU ARE SOLELY RESPONSIBLE FOR DETERMINING WHEN THE iNID,
WG, IAD* AND ONT BATTERY BACKUP REQUIRES REPLACEMENT AND FOR REPLACING AND RECYCLING USED
BATTERIES IN ACCORDANCE WITH MANUFACTURER OR VENDOR DIRECTIONS. YOU ALSO ACKNOWLEDGE AND

OF 911 SERVICE OVER AT&T PHONE SERVICE AND THE DISTINCTIONS BETWEEN 911 SERVICE OVER AT&T PHONE
SERVICE AND 911 SERVICE OVER TRADITIONAL WIRELINE TELEPHONE SERVICE. YOU AGREE TO ADVISE ALL
INDIVIDUALS WHO MAY PLACE CALLS OVER AT&T PHONE SERVICE OF THE 911 LIMITATIONS DESCRIBED BELOW.
AT&T MAKES NO WARRANTY THAT AT&T PHONE SERVICE FOR ACCESS TO 911 WILL BE UNINTERRUPTED,
TIMELY, SECURE, OR ERROR-FREE OR BATTERY BACKUP POWER WILL BE SUFFICIENT TO MAINTAIN THE
SERVICE THROUGHOUT ANY AND/OR ALL POWER OUTAGES.
911 SERVICE OVER AT&PHONE SERVICE IS ONLY AVAILABLE AT YOUR SERVICE ADDRESS, WHILE CONNECTED
TO A PROPERLY POWERED iNID, OR WG (AND A PROPERLY POWERED ONT, IF APPLICABLE) AND AFTER AT&T
PHONE HAS BEEN ACTIVATED.
911 SERVICE OVER AT&T PHONE SERVICE WILL NOT FUNCTION IF YOUR iNID OR WG FAILS OR IS NOT
CONFIGURED CORRECTLY OR IF YOUR AT&T PHONE SERVICE IS INTERRUPTED OR NOT FUNCTIONING FOR
ANY REASON, INCLUDING, BUT NOT LIMITED TO, IN THE EVENT OF A POWER OUTAGE (UNLESS YOU HAVE
WORKING BATTERY BACKUP POWER), NETWORK OUTAGE, BROADBAND CONNECTION FAILURE, OR
DISCONNECTION OF YOUR SERVICE BECAUSE OF BILLING ISSUES. IF THERE IS A POWER OUTAGE, YOU MAY
BE REQUIRED TO RESET OR RECONFIGURE THE EQUIPMENT PRIOR TO BEING ABLE TO USE YOUR SERVICES,
INCLUDING USE FOR 911 CALLING. YOU ACKNOWLEDGE THAT AT&T STRONGLY RECOMMENDS YOU MAINTAIN AT
ALL TIMES AN ALTERNATIVE MEANS OF ACCESSING 911 SERVICES, SUCH AS VIA CELLULAR TELEPHONE SERVICE.
YOU ACKNOWLEDGE AND UNDERSTAND THAT AT&T WILL NOT BE LIABLE FOR ANY LOSSES INCURRED
DIRECTLY OR INDIRECTLY AS A RESULT OF SERVICE OUTAGE AND/OR INABILITY TO DIAL 911 USING YOUR

WITHOUT LIMITING ANY PROVISIONS OF THE TOS, YOU AGREE TO DEFEND, INDEMNIFY, AND HOLD HARMLESS
AT&T, ITS SUBSIDIARIES, AFFILIATES, OFFICERS, AGENTS, DIRECTORS, EMPLOYEES, AND ANY OTHER SERVICE
PROVIDER WHO FURNISHES SERVICES TO YOU IN CONNECTION WITH THE AT&T PHONE SERVICE, FROM ANY
AND ALL CLAIMS, LOSSES (INCLUDING LOSS OF PROFITS OR REVENUE), LIABILITIES, DAMAGES, FINES, PENALTIES,
DEMANDS, ACTIONS, COSTS, AND EXPENSES (INCLUDING, WITHOUT LIMITATION, REASONABLE ATTORNEY
FEES) BY, OR ON BEHALF OF YOU OR ANY THIRD PARTY OR USER OF THE AT&T PHONE SERVICE, REGARDLESS
OF THE NATURE OF THE CLAIM, INCLUDING WITHOUT LIMITATION CLAIMS RELATED TO 911 DIALING, ARISING
FROM OR IN CONNECTION WITH ANY FAILURE OR OUTAGE OF AT&PHONE SERVICE OR ANY FAILURE OR
OUTAGE OF THE 911 NETWORK ITSELF.
VI. Premise Alarm Systems an Other Device Compatibility
AT&T MAKES NO WARRANTY THAT (i) AT&T PHONE SERVICE USED AS A COMMUNICATIONS PATHWAY FOR
MONITORED PREMISE ALARM SYSTEMS, (INCLUDING BURGLAR ALARMS, MONITORED FIRE ALARMS, AND/OR
MEDICAL MONITORING SYSTEMS OR DEVICES), WILL BE UNINTERRUPTED, TIMELY, SECURE, OR ERROR-FREE,
(ii) THE SERVICE WILL BE COMPATIBLE WITH ANY PARTICULAR OR ALL MONITORED PREMISE ALARM SYSTEMS
(INCLUDING BURGLAR ALARMS, MONITORED FIRE ALARMS, AND/OR MEDICAL MONITORING SYSTEMS OR
DEVIDES OR (iii) ANY BATTERY BACKUP POWER EQUIPMENT YOU MAY PURCHASE WILL BE SUFFICIENT TO
MAINTAIN YOUR AT&T PHONE SERVICE THROUGHOUT ANY AND/OR ALL POWER OUTAGES. SEE MANUFACTURER
INFORMATION FOR BATTERY LIFE DURING A POWER OUTAGE. YOU ARE RESPONSIBLE FOR PURCHASING
AND MAINTAINING ANY DESIRED BATTERY BACKUP EQUIPMENT.

WITH MONITORING STATIONS DURING A POWER OUTAGE UNLESS YOU MAINTAIN BATTERY BACKUP POWER
FOR AT&T PHONE AS DESCRIBED IN THIS TOS. IF YOU PURCHASE A MONITORED BURGLAR ALARM OR
MONITORED FIRE ALARM SYSTEM AFTER AT&T PHONE SERVICE HAS BEEN INSTALLED, YOU ALSO AGREE TO
CALL AT&T PRIOR TO INSTALLATION OF ANY SUCH SYSTEM. SUBSEQUENT INSTALLATION OF THESE SYSTEMS
MAY REQUIRE RE-WIRING OF AT&T PHONE SERVICE, WHICH MAY ALSO RESULT IN TIME AND MATERIAL
CHARGES. (AT&T DOES NOT PROVIDE SUPPORT FOR, OR RE-WIRING OF AT&T PHONE IN SUPPORT OF,

ONCE AT&T PHONE SERVICE HAS BEEN INSTALLED FOR USE WITH A MONITORED FIRE ALARM OR MONITORED
BURGLAR ALARM SYSTEM, YOU AGREE THAT YOU WILL NOT CHANGE OR MODIFY THE INSIDE WIRING OF YOUR
HOME OR MOVE OR RECONFIGURE YOUR RG IN ANY WAY WITHOUT CONTACTING AT&T AND YOUR ALARM
SERVICE PROVIDER. YOU ALSO AGREE THAT YOU WILL NOT PLUG ANY TELEPHONE EQUIPMENT INTO THE BACK
OF THE WG. YOU ACKNOWLEDGE AND UNDERSTAND THAT IF YOU CHANGE OR MODIFY YOUR INSIDE WIRING,
MOVE OR RECONFIGURE YOUR WG IN ANY WAY, OR PLUG ANY TELEPHONE EQUIPMENT INTO THE BACK OF THE
WG IT COULD RESULT IN A FAILURE OF YOUR MONITORED BURGLAR ALARM OR MONITORED FIRE ALARM SYSTEM.
BY ACCEPTING THESE TOS, YOU USE AT&T PHONE SERVICE AT YOUR OWN RISK AND WAIVE ANY CLAIM AGAINST
AT&T FOR INTERFERENCE WITH OR DISRUPTION OF A MONITORED FIRE ALARM OR BURGLAR ALARM SYSTEM, A
MEDICAL MONITORING DEVICE, OR OTHER SUCH SYSTEMS OR DEVICES DUE TO THE AT&T PHONE SERVICE.
VII. Local Number Portability
In the event you are transferring an existing phone number for your AT&T Phone service (i.e., porting a
number to AT&T Phone service), you hereby authorize AT&T to process your or er for AT&T Phone an to
notify your existing provi er of your ecision to switch your local, local toll, an long istance services to
AT&T Phone service, an you represent that you are authorize to take this action. Not all telephone
numbers are eligible for porting to AT&T Phone service.
VIII. Voicemail
AT&T Phone service inclu es Voicemail, a full-feature voicemail service. If you access your Voicemail voice

mailbox from outsi e your local calling area, you may incur applicable local toll or long istance charges.
In a ition, the Voicemail service allows you the option to integrate your AT&T wireless service voice mailbox
with your AT&T Phone Voicemail mailbox. (Wireless service from AT&T must be separately purchase .) Calls
forwar e to your Voicemail voice mailbox from your wireless phone will not incur airtime charges. However,
airtime charges may apply when using your wireless han set to retrieve messages. Pager notification allows
your pager to notify you when a message is receive in your Voicemail voice mailbox. Your pager can have
either an email a ress or your pager can have a telephone number associate with it an must be set up
through the Voicemail mailbox. Paging service an equipment must be purchase separately. Other
restrictions may apply.
Voicemail may inclu e a Voicemail-to-Text (VMTT) feature that provi es automate transcription of your
voicemail. AT&T is not responsible nor liable for: 1) errors in the conversion of or its inability to transcribe
voicemail messages to text/email; 2) lost or mis irecte messages; or 3) content that is unlawful, harmful,
threatening, abusive, obscene, tortious, or otherwise objectionable. We o not filter, e it or control voice,
text, or email messages, or guarantee the security of messages. We can interrupt, restrict or terminate VMTT
without notice, if your use of VMTT a versely impacts AT&T's network, for example that coul occur from
abnormal calling patterns or an unusually large number of repeate calls an messages; or if your use is
otherwise abusive, frau ulent, or oes not comply with the law.
You are solely responsible for an will comply with all applicable laws as to the content of any text messages
or emails you receive from VMTT that you forwar or inclu e in a reply to any other person. You authorize
AT&T or a thir party working on AT&T's behalf to listen to, an transcribe all or part of a voicemail message
an to convert such voicemail message into text/email, an to use voicemail messages an transcriptions to
enhance, train an improve AT&T’s speech recognition an transcription services, software an equipment.
You agree that the results of benchmarking VMTT against competing pro ucts or services is AT&T confi ential
information requiring AT&T written consent to isclose in accor ance with the BSA or RSA as may be applicable
to you. A itional charges may apply to receiving email on your wireless evice from VMTT, as well as, replying
to or forwar ing VMTT messages via SMS (text) or email, epen ing on your plan. Transcription times cannot
be guarantee . You are responsible for provi ing a correct email a ress an up ating the email a ress
when changes to the email account are ma e.
IX. Prohibite Uses of AT&T Phone Service
You agree that you will NOT use AT&T Phone service:
1. As a business service or for a business purpose.
2. To engage in auto- ialing, continuous or extensive call forwar ing, telemarketing, fax broa casting or fax

or mo ify the Service, or to change your call plan to a ifferent offer on a prospective basis, an in a ition,
to assess a itional charges for each month in which excessive usage occurre . If you subscribe to a
calling plan which inclu es unlimite calling of any type, unless otherwise specifie by your specific plan
in marketing materials associate therewith, consistent monthly use in excess of 5,000 aggregate minutes
per month, taking into account all types of calling in your plan which are provi e on an unlimite basis,
shall be presume to be inconsistent with these restrictions an shall be subject to the con itions above.

of AT&T Phone service an is esigne as a voice-mail, not an announcement, service. Use of Voicemail
service as an announcement service an /or other improper or excessive use may impair AT&T’s ability to
provi e reasonable service to other customers. AT&T reserves the right to cancel your AT&T Phone service at
any time, with or without notice, if as etermine solely by AT&T base on its network/service esign an
usage experience, your messaging service is (1) being use in an improper manner inclu ing, but not limite
to, using it as an announcement service or for unlawful purposes, (2) consistently generating excessive usage,
(3) affecting AT&T’s ability to provi e reasonable service to other customers, or (4) being use to interfere with
another’s use of the voicemail system.
X. Suspension/Termination
Without limiting other rights set forth in these TOS, AT&T may either terminate your AT&T phone service or transfer
your AT&T Phone service to a ifferent telephone number without penalty, upon reasonable notice, if AT&T stops
provi ing AT&T Phone service either generally or in your area, if at any time 911 service over AT&T Phone service
is not available, or for other reasons associate with the provisioning of 911 service to your premises. AT&T also
may terminate your or er for Service if you o not activate the Service or if you o not formally acknowle ge
information about 911 service limitations in accor ance with irectives from AT&T.

service. Suspension (but not termination) of AT&T Phone service still allows (assuming working battery backup
uring power outage) for the following ialing privileges: outboun calling to 911, outboun calling to Operator
Services (for emergency assistance only), an inboun calling (inten e to maintain a call back path for
emergency service provi ers). AT&T has no responsibility for retaining or elivering messages that are locate in
any voice mailbox at the time of the suspension or termination, or that are a resse to any main account hol er
or subaccount hol er thereafter.

a “remote access” tool which allows you to access portions of your AT&T U-verse TV service from a website or
other me ium).
II. Billing An Payments
For AT&T U-verse TV service, nonrecurring an usage-base charges generally bille in the billing cycle following
the transaction inclu e, but are not limite to, Vi eo on Deman an Pay Per View. As long as payments are
current, you will have a limit (up to a maximum of $150) per bill cycle on such one-time or ers bille to your
account. This limit will vary base on cre itworthiness or for other reasons. A owngra e fee may apply if you
make changes to your Service within 30 ays of Service provisioning or later programming or ers.
III. Service-Specific Equipment
Equipment for AT&T U-verse TV service inclu es an AT&T U-verse TV Receiver (“Receiver”), which is value at
$10 per month an inclu e in the service fee on your monthly invoice, an , if applicable, a Wireless Access
Point (“WAP”) to provi e connection for a wireless Receiver. You may request a itional Receivers an limits
on the number of available Receivers for a househol may apply. A itional equipment fees or other fees
may apply to all AT&T U-verse TV equipment, inclu ing, but not limite to, the Receiver(s). Receivers are
subject to all applicable taxes, fees an surcharges.

Some programming may not be available in certain areas ue to legal, regulatory, an contractual prohibitions,
inclu ing restrictions of the Fe eral Communications Commission an sports blackouts. If you also purchase
AT&T Phone service, Caller ID information for AT&T Phone calls can be isplaye on your TV screen. In a ition,

As permitte un er applicable law, in a ition to other rights provi e for in this TOS, in the event a payment
is past ue, AT&T may restrict your account to prevent access to Vi eo on Deman , Pay Per View, an other
usage-base services an content.
V. Disclaimer of Warranties
YOU EXPRESSLY UNDERSTAND AND AGREE THAT AT&T MAKES NO WARRANTY THAT THE SERVICE WILL
ALLOW YOU TO RECORD, VIEW, OR TRANSFER ANY PARTICULAR PROGRAM OR CONTENT.
VI. Intellectual Property
AT&T U-verse TV is provi e for your non-commercial personal viewing, use, an enjoyment in a private
resi ential welling/office unit. You agree that the AT&T U-verse TV service will not be viewe in areas open
to the public or in commercial establishments, an that a mission will not be charge for listening to or
viewing the Service. Your AT&T U-verse TV Service may not be copie , transmitte , repro uce , publishe ,
broa cast, rewritten, re istribute , or performe except as permitte by the “fair use” provisions of the U.S.
copyright laws.
