Important Safety Instructions
When using your PVC High Back Foldable Shower/Commode Chair, always take basic safety precautions to
reduce the risk of injury to persons.
1. Read and understand all instructions.
2. Determine, with a physician or therapist, the suitability of this product for the intended user.
3. Do not leave user unattended unless you have determined that their abilities and requirements are
such that it poses no safety risk to leave them unsupervised.
4. Place your PVC High Back Foldable Shower/Commode Chair on a level surface; do not use near steps
or other hazards.
5. Keep your PVC High Back Foldable Shower/Commode Chair clean and sanitary.
6. Check regularly that all bolts, casters, caster locks engage properly, footrest and other parts are
tight and in good working order.
7. Be sure that all caster locks are engaged before transferring a patient in or out of unit. It is further
suggested that all transfers be performed by at least two (2) care givers to ensure safety.

Assembly of PVC High Back Foldable Shower/Commode Chair
1. Place the back frame into the Shower/Commode Chair.
2. Install Backrest Cover.
3. Install the two (2) set screws provided with unit; make sure they are tightened down. Do Not Over Tighten.

Folding Your PVC High Back Foldable Shower/Commode Chair
1. Remove both (2) Quick-Release pins behind Backrest assembly.
2. Remove upper Cross-Brace.
3. Remove both (2) Quick-Release pins from lower Cross-Brace under commode cushion.
4. Lift Commode Cushion from rear of Chair.
5. Standing at rear of chair, give a twisting motion and chair will fold.
6. Lift Commode Cushion; place Cross-Brace on one end of assembly for storage.
7. Unfold and re-assemble in reverse order.

Caster Lock Operation
Your PVC High Back Foldable Shower/Commode Chair has four (4) 4” Deluxe Casters. Two (2) Deluxe
Casters have a locking mechanism.
1. To engage Caster Lock, place foot on Caster lock lever and push down.
2. To disengage Caster Lock, push up on Caster lock lever.
3. Be sure that all Caster Locks are engaged before transferring a patient in or out of unit. It is further
suggested that all transfers be performed by at least two (2) care givers to insure safety.

COLUMBIA MEDICAL MFG., LLC warrants this product to be free of defects in material and
workmanship. Our obligation under this warranty is limited to repair or replacement of any part or entire
unit at our option for a period of one year from date of delivery to the original purchaser.
The warranty does not include cost of inconvenience, property damage, misuse, abuse, accident or similar
incidents. The warranty will not apply if product has been tampered with or repaired by unauthorized
individuals. This warranty gives you specific rights which vary from state to state.
