Warning
1. Product is for single person use only.
2. Do not use this chair unless all bolts and knobs are securely tightened.
3. Check that all bolts and knobs are tightened every six months
or as needed.
4. Dispose of packing material properly. Do not use plastic cover as a head
covering. Suocation may result.
5. Do not use this chair as a stepladder.
* Arm Pad Lock Kit (L & M) & Arm Cap Kit (N, & O) are included.

Instructions Arm Pad Lock Kit

5) Remove four screws that
secure the part for sliding
4) Remove the arm pad
2) Slide the arm pad back
and take a screwdriver
and pop o the cover
1) Bracket A & Bracket B
7) Make sure the angle
adjustment is straight,
then remove center screw

9) Place sliding part
back on

11) Reinstall the arm pad
using the same two
screws and secure
the pad.

12) Pop cover back on

3) Slide the arm pad
forward and remove
screws
6) Remove part

8) Place Bracket A
and use the same
screw to secure it

10) Pick any of the screw
holes to install Bracket B
and use the same screw
to secure it

Assembly Instructions Slip Cover

3) Unzip old slip cover and remove
from foam
2) Remove old slip cover and foam
from seat pan by unfastening
the velcro fasteners on the back
and front of seat. Remove the
back fasteners first
4) Install new slip cover onto
the foam
6) Align 2 front corners and
install using velcro fasteners.
Li back of seat and zip up
slip cover. Align 2 back
corners and install using
velcro fasteners
5) Make sure all four slip cover
corners match to the foam
corners, top & bottom.
Close zipper approximately 2”
1) Assemble chair
2”

Installation tip for installing adjustable arm. Bolted Cap (Item L) must be aligned with the Long Bolt (Item M).
(Armrests for future shipment will be fully assembled.)

To ensure the bolted cap does not rotate out of alignment, place a thin piece of tape over bolted cap prior to installation.
(Armrests for future shipment will be fully assembled.)

Installation tip for installing slip cover.
Model 402
2) Remove old slip cover and foam
from seat pan by unfastening
the velcro fasteners on the back
and front of seat. Remove the
back fasteners first.
3) Unzip old slip cover and remove
from foam.
4) Install new slip cover onto
the foam.
5) Make sure all four slip cover
corners match to the foam
corners, top & bottom.
Close zipper approximately 2”.
5) Align 2 front corners and
install using velcro fasteners.
Li back of seat and zip
up slip cover. Align 2 back
corners and install using
velcro fasteners.
2) Assemble chair.
