Patient Financial Obligations and Billing Information
Our Notice of Privacy Practices is available for your review at www.preventicesolutions.com
On behalf of your physician, you are being provided with a heart monitor from Preventice
Services. This test will be billed in two parts:
1. Your medical professional will bill your insurance for the in-office hook-up and the final
reading of the test results.
2. Preventice Services will bill your insurance for the use of the monitor along with providing
the physician with 24/7 monitoring center service and all requested data and reports.
All equipment is the sole property of Preventice Services. To avoid financial liability and to ensure
that you do not receive a bill for the value of the equipment in your possession (up to $2,400),
please return the equipment immediately after your service ends. Any request for an extension of
your study beyond your prescribed study needs to come from your physician through a new order
sent to Preventice Services. Be aware that the second study may not be covered by all insurance
providers and you may be responsible for those charges.
Your insurance company will usually send you an Explanation Of Benefits (EOB) describing the
amount paid and the amount you owe Preventice. An EOB is not a bill. Our goal is to help your
physician diagnose your heart condition without delay or concerns about billing. We offer patient-
friendly payment options, including a discounted rate at the time of service or a reduced
rate when your insurance indicates a large fee payable by you for our services or the physician’s
services.

Welcome to the BodyGuardian® Verité
Your physician has prescribed you the BodyGuardian®Verité monitoring service provided by
Preventice Services. The monitor continuously records all heartbeats, detecting irregular rhythms.
You must wear the monitor for the length of time your doctor has prescribed.
Resources and Assistance
Important
Contact the Monitoring Center if you plan to travel outside the United States. The monitor does not
operate outside the United States and cannot be worn while traveling on an aircraft.
Keep the monitor away from water. Remove the monitor and the lead wires when you bathe, swim
or perform other similar activities then re-attach lead wires afterwards.
DO NOT remove the battery from the monitor unless instructed by a Preventice technician.
Preventice Services is not an emergency response service. Contact your doctor immediately if
your symptoms worsen. If you are experiencing a medical emergency, dial 911.

Your Equipment
Ensure that the following equipment is included in the box. Your equipment may vary from the components
pictured. Preventice reserves the right to modify, change, or discontinue products or accessories at any time.
1. Lead wires
Keep lead wires plugged into monitor at all
times, even when charging the battery and
returning the monitor.
2. Monitor display screen
3. Menu navigation buttons
4. Selection button
5. Event button
6. Device serial number
Serial number is next to the letters S N.
7. Battery
The battery is pre-installed in the monitor,
DO NOT REMOVE.
8. Belt Clip
Attach lead wires to monitor first. Insert rounded
edge of monitor into case. Slide the monitor
down into the bottom of the case with the display
screen facing out. Attach clip to pants.

Step 1: Preparing your skin
All leads should be put on BEFORE the plastic PULL Tab is removed from the monitor. The order of lead
placement is important and should be done in the correct order every time the leads are attached.
• If hair is present, remove the hair where you will place the electrodes.
• Thoroughly clean the area with soap and water. Dry the area completely.
• Do not apply lotions or oils.
Snap the electrodes onto the
lead wires.

Ensure there are no air
bubbles or creases once the
electrode is placed on skin.

Run the lead wires from the
electrodes to the monitor
neatly under your clothing.
Remove the adhesive from
the electrode.

Warning:
Do not apply the electrodes over open
wounds, lesions, infected or inflamed
areas. Apply the electrodes only to
intact, clean skin.
Patients with known skin allergies or
hypersensitivities to adhesives should
consult their doctor to select the most
appropriate option for their needs.

Step 2: Attaching the electrodes
1. Attach the black wire aligned with the left side of your neck, two finger widths below the left collarbone.
2. Attach the red wire below and to the left of the black wire, under the breast and to the outer ribcage
(bone part of the ribs).
3. Attach the white wire aligned with the right side of your neck, two finger widths below the right
collarbone.
4. Lastly, attach the green wire below and to the right of the white wire, under the breast and to the outer
ribcage (bone part of the ribs).
Changing the Electrodes
Change the electrodes every 3 days or when they no longer adhere to your skin.
• Place new electrodes about one inch away from the existing electrodes within
the placement zone indicated in diagram.
• Remove the electrodes while showering or use a warm, damp cloth to blot
while removing.

Step 3: Charging the Monitor
Plug the charger into the wall
outlet using only the charger
provided.
Connect the wall charger to
the charger port on top of the
monitor.
The monitor will vibrate twice to
indicate the charger is connected
properly and the device is
charging.

Important: The monitor should be charged daily for at least two hours or as needed using only the
monitor charger provided in the box. Upon receiving the monitor, you may need to charge the monitor if
the battery gauge is less than half.
NOTE: It is safe to keep the monitor connected to the electrodes while charging.

Step 4: Activating Your Monitor
Once all four leads are properly connected,
sit down.
Remove the PULL Tab from the monitor.
Remain still for the first 60 seconds.
Once the plastic PULL Tab is removed, the
monitor emits a single beep and powers on.
Important:
• If you have accidentally removed the PULL Tab before attaching the electrodes, contact Preventice
Services at 888.500.3522 (press 1, 1, 4).
• You should NOT push the Event button unless you feel a symptom.
You will hear a ring tone to confirm the
monitor is powered on correctly.
The monitor will automatically proceed to
the “Monitoring” screen indicating the
beginning of your study.
If the screen displays “Battery low Please
recharge”, charge immediately.

Step: 5 Navigating the On-Screen Menu
NOTE: The blue lights will blink in a clockwise pattern every 10 seconds all the time to indicate the
device is recording properly.
The BodyGuardian Verité monitor does NOT have a touch-screen. Do not attempt to press the screen to
make selections. To wake up the monitor, tap the event button. Do NOT press hard.
The monitor has a series of
touch-sensitive buttons to the

Step 6: Recording Your Symptoms
Available Symptoms:
1. None or Accidental Push
2. Chest Pain or Pressure
3. Dizziness
4. Flutter or Skipped Beats
5. Tired or Fatigued
6. Rapid or Fast Heartbeat
7. Passed Out
8. Shortness of Breath
9. Light Headedness
10. Irregular Heartbeat
Event
button
If you begin to feel symptoms related to why your monitor was prescribed, such as dizziness, chest
pain or shortness of breath, you can manually record these events. You have 20 seconds to select the
appropriate symptom and 20 seconds to select the appropriate activity.
Push and hold the Event button until the monitor begins to vibrate.
Release the button when the symptom screen displays on the screen.
Touch the up or down arrows to scroll through the selections until
the symptom of your choosing is highlighted in the CENTER of the
screen.
Push the center button to select the highlighted entry.
The activity screen will display next.

Step 7: Recording Your Activities
Touch the up or down arrows to scroll through the selections until
the activity of your choosing is highlighted in the CENTER of the
screen
Push the center button to select the highlighted entry.
The “Event recorded successfully” screen will display briefly, then
the “Monitoring” screen will appear.

1. Pending Data – Displays the number of items
waiting to be transmitted.
• This number represents the events that
have been captured and other informational
items related to your study such as lead
status, recording status and charging
status.
• A large number, like 15, does not
necessarily mean that 15 cardiac events
are waiting to be transmitted.
2. Transmission Strength – The “E” symbol
indicates the monitor can transmit data.
3. Date and Time – Provided by cellular service
based on the nearest tower in your area.
4. Battery Gauge – Indicates the charge status
of the battery.
• When the battery is charging, a lightning
bolt symbol will appear.
• When the battery is full, the lightning bolt
will disappear.

Frequently Asked Questions
Why does the screen display “Connect electrodes to begin
monitoring”?
1. The monitor is not in study mode and has not recognized your heart
rhythm.
2. Verify the leads are properly connected to the electrodes.
3. Verify the electrodes are securely adhered to your skin without
wrinkles or bubbles. Replace electrodes if necessary.
4. If the display does not proceed to “Monitoring”, disconnect and
reconnect the lead wires in the proper order: black, red, white and
lastly, green.
5. Press and hold each of the snaps, one by one starting with the
green, to determine if one of the electrodes is not making contact.
−− If pressing on the snap allowed the monitor to display
“Monitoring” replace the electrode and verify the placement
location.
−− Ensure the electrode for the green lead is placed as shown in
the diagram, not too far onto your side or back.

Frequently Asked Questions
Why is the screen blank and the blue arrow lights circle around
the center button?
The monitor is currently recording. The screen has been turned off to extend
the battery life. To turn on the display, press the event button once (do not
hold). The screen will turn off after a few seconds.
Why does the screen display “Something has gone wrong” and
a 4 digit error code?
Monitor requires reset. Push Target/Select button for 3 seconds then quickly
push the event button (do not push and hold). If the problem persists, contact
888.500.3522 (press 1, 1, 4).
Why is the monitor vibrating or buzzing? (Can be a short or
continuous buzz)
Short buzz: Verify the electrode pads are properly attached to your body.
Replace pads if necessary.
Continuous long buzz: The battery needs to be charged. Charge the monitor
for a minimum of 1 hour. The battery can be safely charged for several hours.

Why does the screen display “Please Reconnect Electrodes”
or Monitoring Suspended”?
1. One or more leads is not adhered properly. Ensure all leads are
securely fastened to the electrodes on your skin.
2. Verify the electrodes are securely adhered to your skin without
wrinkles or bubbles. If necessary, replace the electrodes.
3. Press and hold each of the snaps, one by one starting with the
green, to determine if one of the electrodes is not making contact.
−− If pressing on the snaps allows the monitor to display
“Monitoring,” replace the electrode by placing a new one first
and removing the original. Verify the placement location.
−− Ensure the electrode for the green lead is placed as shown in
the diagram and not too far onto the patient’s side or back.
4. Verify the electrodes are positioned properly on the chest. Allow the
electrode to conform to your body. This can take 2 minutes.
5. If the problem persists, contact 888.500.3522 (press 1, 1, 4).

Frequently Asked Questions
Where can I view the reports provided by my monitor?
Contact your doctor to obtain all your reports. You should sign a medical
release form submitted to Preventice legal department to receive your reports
at the conclusion of your monitoring study. For additional information, contact
888.500.3522 (press 1, 8).
Why is my monitor not picking up a wireless signal?
1. Power on the monitor and check the signal bars in each room, find the
room with the strongest signal.
2. If this is not possible, once you outside, the device will begin to transmit.
3. If the problem persists, contact 888.500.3522 (press 1, 1, 4).
What is the best way to remove the electrode’s glue residue
from my skin?
Use warm, soapy water to ease removal of electrodes and glue residue.

Step 8: Returning the Equipment Using UPS®
Return the BodyGuardian®Verité equipment to
Preventice immediately after your service ends.
1. Place the monitor and attached lead wires into
the white padded pouch, then place the pouch
inside the box. Do NOT remove the battery
from the monitor.
2. Place the monitor, charger, unopened
electrodes and all other accessories into the
box. See p.4 for the list of equipment.
3. Remove the adhesive strip from flap on the
shipping box and seal securely. The prepaid
UPS return label is already affixed to the
bottom of the box.
4. Drop package off at any UPS op Box or The
UPS Store®. For assistance, call Preventice at
888.500.3522.
Note: If you have lost or damaged the equipment,
Preventice will mail you an invoice for the cost of
the item.
