Intended Use and Descriptions
Otto Bock’s family of wheelchair back cushions are used to improve the seated posture of
the user, as well as to provide pressure relief, by properly supporting the trunk region and
distributing existing pressure over the largest possible area.
Application
Improved thoracic lateral support if the user’s sitting posture requires additional support.
Support for an orthopedic deformity such as a lordosis, can be inserted in a compartment
between the solid base and the pad.
Using the integrated FLOAM® pads for Back 2, Back 3 and Back 4 in the vertebral region
can reduce risk of decubitus ulcers.
The Otto Bock warranty applies only when the product is used according to the specified
conditions and for the intended purposes, following all manufacturers’ recommendations
(see 2-Year Limited Warranty section for more details).
INSTRUCTION MANUAL
BEFORE YOU BEGIN:
Tools Needed:
Phillips head screwdriver.
Soft-headed hammer: optional width adjustment.
Drill with 1/4˝ (6 mm) bit: for use in attaching optional headrest.
Preparing the Wheelchair:
Remove the upholstery from the back of the wheelchair’s push canes per manufacturer’s
instructions.
Definitions as Used in this Manual:
1. B1 (Back 1)
2. B2 (Back 2)
3. B3 (Back 3)
4. B4 (Back 4)
Installing the Otto Bock Back Cushion to the Wheelchair
A. Connecting the Rigid Back Bracket to the Wheelchair Push Canes
1. Determine the diameter of the wheelchair push canes.
2. The rigid back bracket will accommodate 3/4˝, 7/8˝, and 1˝ (19-20 mm) push canes
(Figure 1). Hold the Back in the location it will be installed on the wheelchair and
place a mark on both push canes to indicate the location for the bracket tape. (This
location is about 1/4˝ (6 mm) below the rigid back bars).
3. Remove the rigid back bracket screw and attach the rigid back bracket around the
wheelchair push canes as shown in Figure 2.
4. Make sure that the left and right rigid back brackets are the same height on the
wheelchair push canes before tightening.
5. Re-insert the rigid back bracket screw and tighten as shown in Figure 2A, 2C, 2D.

NOTE: The rigid back bar notch can be positioned toward the posterior of the wheelchair
as shown. The rigid back bar notch can also be in the anterior position, although seat depth
will be affected.
NOTE: Avoid over-tightening the screws around the wheelchair push canes (See Figure 2B)
B. Adjusting the Back Width
A unique feature of the Back is its range of width adjustability. It is designed first to custom
fit the user, and second, to fit onto the wheelchair. This is accomplished by having two sets
of width adjustments.
When the proper width of the Back has been adjusted to the specific user, a second set of
adjustments within the attachment hardware allows the Back to properly fit the wheelchair.
The Back is offered in two standard sizes:

Note: Both sizes (standard and large) allow micro adjustments of 1/2” (12 mm) increments
within the range of adjustability.
Tips before you begin (Width adjustments)
1. The mounting hardware offers about 2˝ (50 mm) of total adjustability. This means that
the push canes of the wheelchair, as a general rule, need to be within 2˝ (50 mm) of the
width chosen for the user.
The foam pad (B1), FLOAM® Pad (B2 & B3) or Cel-Pak (B4) are designed to expand and
to accommodate all widths within the range of adjustability for each back.
B.1 Adjusting the Back Width to Fit the User
1. Measure the width of the user’s back. This measurement represents a starting width
setting. If this width differs from the factory settings, follow the adjustment Steps 2-8
below, otherwise skip ahead to Step 8.
2. Remove the upholstery by detaching all points of hook and loop fasteners and
unsnapping the two snaps on the top rear side of the Back.
Refer to section “D. Removal of the Upholstery”.
3. With a Phillips screwdriver, remove the “rigid back wing-to-spine” attachment screws.

NOTE: The rigid back wing will not slide in or out of the spine until all of the attachment
screws have been removed.
4. For exact width specifications, refer to Figure 3 and Table 2 for proper
hole configuration.

5. Once the proper hole pattern is chosen, insert the screws in the top and bottom holes
only, so that the width can be checked for proper patient fit.
6. Before you proceed check the Back width as follows:
a. Re-attach the upholstery as described in section “E. Installation of the Upholstery”.
b. Attach the Back to the wheelchair push canes. If the rigid back hooks do not fit
the width of the wheelchair and need adjustment, make adjustment as described in
section B.2.
c. With the new back in place, have the user sit in the wheelchair and check for
proper fit.

d. When the fit is proper, remove the foam pad upholstery and insert the remaining
attachment screws. Tighten securely. If the fit is not proper, it may be necessary to
repeat steps 2-6d.
7. Replace the upholstery as described in section “E. Installation of the Upholstery.”
8. The Back is ready for final installation.
B.2 Adjusting the Back to Properly Fit the Wheelchair
With the Back properly fitted for the user, it is time to make sure that it fits the wheelchair.
1. With the Back installed the rigid back bar should be extended to the outer edge of the
rigid back bracket notch as shown in Figure 3.
2. The Rigid Back Hook Assembly can be width adjusted as shown in Figure 4.
a. If the wheelchair push canes are less than 1˝ (25 mm) diameter, attach one layer of
adapter tape for 7/8˝ (22 mm) tubes or two layers of tape for 3/4˝ (19 mm) tubes
(see Figure 1).
b. Loosen the two rigid back offset wedges to wing adjustment screws and slide the
Rigid Back Hook Assembly in or out so that the rigid back hook matches the width
of the wheelchair as shown in Figure 5.
NOTE: Use a Phillips screwdriver for the 10/32 x 5/8 Phillips screws.
c. Once both widths are matched, tighten the adjustment screws.
NOTE: It is important that both the left and right rigid back hook assemblies are moved the
same distance as shown in Figure 5. If not, the Back will sit crooked in the wheelchair. The
definition of left and right side, is called out, as the user sits in the wheelchair. The user’s
left side is the Back left side and the wheelchair’s left side.
B.3 Attaching the Back to the Wheelchair
1. Insert the rigid back bars (Figure 7A, Pos. 1) into the receiving lip of the back bracket
(Figure 7A, Pos. 2). Pull the closing latch of the bracket securely over the back bars on
each side of the back. (Figure 7B)
2. With the rigid back hook latches (Figure 7B) lifted up, place the rigid back hooks (Figure
7B, Pos. 1) against the wheelchair push canes.
3. Lock the rigid back hook latches in place (down position) as shown in Figure 7C to
prevent the Back from rotating forward.
C. Adjusting the Recline or Incline
The Back is factory preset at a 90° back angle. When installed, the posterior surface will
be parallel to the push canes. It is possible to adjust the back angle from approximately
80° to just over 100°.
1. Each rigid back hook assembly is held together with two 10/32 x 5/8 flathead screws
and nut sliders (Figure 5).
2. To change the angle, remove both screws and nuts from each side of the rigid back
hook assembly and adjust as needed.
NOTE: Both screws must be used to ensure proper use.
3. Once the proper setting has been determined, insert the two screws into the proper
holes and firmly re-tighten.
D. Removal of the Upholstery
1. Unsnap the tabs and remove the foam pad (B1), FLOAM® Pad (B2 & B3) or
Cel-Pak (B4) completely from the Back as shown in Figure 8.
2. Separate all hook and loop fasteners and remove the wing covers (B2, B3 and B4 only)
from the wings as shown in Figure 8.

E. Installation of the Upholstery
1. Slip the wing sections over the appropriate wings and re-attach all fasteners as shown as
shown in Figure 8 (B2, B3, B4 only).
2. Align the top snaps with the snaps on The Back of the center spine section, and snap
into place (B1, B2, B3 and B4). Fold the expandable sections of the wings under the
FLOAM® Pad or Cel-Pak and secure the bottom (B2, B3, B4 only).
OPTIONAL EQUIPMENT
F. Adding Lumbar or Sacral Support (B2 and B3 only)
Otto Bock Pelvic Positioning/Support Kit
Caution: The selection, positioning and installation of these, and any other, seated
positioning products should be done by a qualified therapist, Rehabilitation Technology
Supplier or other trained seating specialist.
If determined by the seating specialist that the user of the Back requires additional pelvic
positioning or support, the lumbar roll or sacral wedge by Otto Bock HealthCare may be
added to the basic back configuration.
The lumbar roll is used primarily to increase the amount of lumbar lordosis. To avoid
potential skin breakdown and discomfort to the user, the lumbar roll should never be used
with a fixed or non-reducible lumbar flexion deformity.
The sacral wedge, with the wider edge toward the top and the flat surface toward the
back of the chair, is designed to apply anteriorly directed forces at the level of the P.S.I.S.
(posterior superior iliac spine). Its primary function is to position a flexible pelvis/lumbar
spine in a slightly anteriorly tilted position. This position helps maintain the natural
complementary curves of the spine.
The sacral wedge may also be used to support a fixed or non-reducible, posteriorly tilted
pelvis. In this application, the sacral wedge should be positioned with the wider edge toward
the bottom and the flat surface toward the back of the chair. This position supports the
pelvis to help prevent additional posterior pelvic tilt.
Please Note: The lumbar roll and sacral wedge will be most effective when used with a
well-positioned and properly adjusted pelvic positioning support, i.e., lap belt.
1. A compartment within the Back upholstery is accessible when the Back is in place, and
the user is in the wheelchair. This compartment allows a sacral or lumbar support to be
inserted into the Back to customize the shape of the Back.
2. There are three strips of hook and loop fasteners, running from the top to the bottom of
this compartment as shown in Figure 10, Pos. 1.
3. These fasteners will allow the placement of the Otto Bock Lumbar Roll or Sacral Wedge
(or various sizes or shapes of foam or FLOAM® bladders) to provide a wide range of
adjustments.
4. To customize the shape of the Back, obtain the desired shape of foam or bladder, or order
the Otto Bock Pelvic Positioning/Support Kit from Otto Bock Health Care. Insert the
lumbar roll or sacral support into the compartment at the desired location and press the
hook side (Figure 11, Pos. 1) to the loop side (Figure 11, Pos. 3) around the insert (Figure
11. Pos. 2) to hold in place.
5. Have the user lean back. The seating specialist should verify the position and function of
the lumbar roll or sacral support.
6. Make adjustments as needed.
7. Recheck fit and function.
