Indications for use
Accu-Chek Ultraex is an infusion set for the
subcutaneous infusion of insulin administered
with microdosage insulin pumps.

WARNINGS
Read the instructions carefully before use.
It is recommended that you consult your insulin
pump manual for information regarding pump
therapy.
For rst time use, setup must be performed in
the presence of a health care professional.
• This device remains sterile until the package
is opened or damaged. Do not use if the sterile
package has been opened or damaged.
• Protect the product from excessive humidity,
sunlight and heat. Store at room temperature.
• Discard the set after each use. Do not clean
or re-sterilize.
• The Accu-Chek Ultraex is for subcutaneous
use only.
• Always tighten the luer-lock rmly into the
pump adapter. If this is not done, danger of
leakage can result, due to a loose connection
between the infusion set and the pump. Avoid
using any tools to screw the luer-lock connec-
tor into place. Otherwise damage could result
to the luer-lock connector.
• Do not leave air bubbles in the Accu-Chek
Ultraex. Prime thoroughly.
• A small percentage of Accu-Chek Ultraex us-
ing soft cannulas can accidentally get crimped
during insertion or displaced while in use.

If this should occur you should immediately
use a new Accu-Chek Ultraex.
• Check the infusion site frequently to make
sure the soft cannula remains rmly in place.
Because the cannula is so soft, you will not
notice any pain if it is pulled out. The soft
cannula must always be completely inserted
in order to receive the full amount of insulin.
Replace the Accu-Chek Ultraex as soon as
the self-adhesive loosens.
• Do not reinsert the introducer needle into the
soft cannula. Reinsertion could cause tear-
ing of the soft cannula which would result in
unpredictable medication ow.
• If you have to disconnect your Accu-Chek
Ultraex pay particular attention to hygiene.
• Never use a soft cannula for more than 72
hours (3 days). Never use the tubing for more
than 6 days. When changing the cartridge,
make sure that the transfer-set is completely
primed before connecting to the head-set.
• lf the infusion site becomes inamed, replace
your Accu-Chek Ultraex and use a new site.
• Do not put disinfectants, perfumes, deodorants
or insect repellent on your Accu-Chek Ultraex
as these may affect the integrity of the set.
• Never prime the transfer-set or attempt to free
a clogged line while the transfer-set is con-

nected to the head-set. You may accidentally
infuse an uncontrolled quantity of insulin.
• Avoid mechanical stress on the infusion site
and on the Accu-Chek Ultraex. Pay attention
when carrying heavy weights.
• Prepare the infusion site before insertion ac-
cording to CDC (Centers for Disease Control
and Prevention) guidelines or institutional
protocol.
• Check blood glucose level 1–3 hours after
inserting your Accu-Chek Ultraex and inspect
the infusion site on a regular basis. Do not
insert a new soft cannula just prior to bedtime.
Check blood glucose frequently (check with
your physician).
• Check your infusion set at the luer-lock to
tube connection periodically (every 3 hours
during the day and before going to sleep) dur-
ing use. A break may result in leakage, causing
interruption of insulin delivery. If you notice a
break, change your infusion set immediately
and check your blood glucose.
• lf blood glucose level gets unexplainably too
high, or an «occlusion» alarm occurs, check
for clogs and leaks. lf in doubt, change your
Accu-Chek Ultraex.

Insert Accu-Chek Ultraex
Picture 1
Choose a site away from the waistline, bones,
scar tissue, belly button and recent infusion
sites. Clean hands and the site according to the
instructions from your health care professional.
Allow infusion site to dry before the soft cannula
is inserted.

While holding the Accu-Chek Ultraex at the
blue holder cap (M) with thumb and forenger,
remove the transparent protective cover (B) by
turning and pulling. Keep the protective cover for
later use in order to safely dispose the introducer
needle (C).

Remove both pieces of the adhesive backing (E)
from the self-adhesive (F).

Pinch the subcutaneous tissue. Insert the intro-
ducer needle (C) and soft cannula (D) quickly
and straight into the infusion site, at a 90° angle.
Watch the soft cannula while inserting to as-
sure that it goes in smoothly without kinking or
crimping.

WARNING
Ensure clean application. Do not touch the soft
cannula or introducer needle. Make sure that the
soft cannula is fully inserted into the subcutane-
ous tissue and not kinked at the skin surface.
Once the introducer needle punctures the skin,
the soft cannula must be quickly pushed in the
rest of the way, or it may kink upon insertion,
causing reduced or blocked insulin delivery.

Carefully smooth out the self-adhesive (F) onto
the skin and run your ngers over the self-adhe-
sive to ensure good skin contact is achieved.

Hold the self-adhesive (F) in place, gently press
the side clips of the blue holder cap (M) and pull
it from the cannula housing (G).

To remove the introducer needle (C) hold the
cannula housing (G) rmly to your body. Place
one nger on the top of the cannula housing
and withdraw the introducer needle in a straight
manner using the introducer needle handle (A).
Remember to dispose the introducer needle (C)
safely (needle container). Discard your introduc-
er needle and infusion set after use according to
local regulations.

Attach the luer-lock (K) to the pump by tightening
it into the pump adapter. Follow the pump manu-
facturer’s user manual to prime the transfer-set.
Prime the transfer-set until medication drips
from the connector needle tip. Do not leave air
bubbles in either the cartridge or the transfer-set.
Connect the transfer-set to the newly inserted
head-set and ll the empty space of the cannula
housing with a bolus of 1.0 units (U100 insulin).
WARNING
You must ll the empty space of the newly in-
serted head-set after connecting the transfer-
set. Failure to do so results in missed insulin.
If the coupling does not click noticeably and au-
dibly together, use a new head-set and a new
transfer-set.
Do not use any tools to screw the luer-lock con-
nector into place.
Filling volume transfer-set:
12”/ 30 cm: approx. 6 units* approx. 60 μl
24”/ 60 cm: approx. 11 units* approx. 110 μl
31”/ 80 cm: approx. 14 units* approx. 140 μl
43”/110 cm: approx. 18 units* approx. 180 μl
Filling volume head-set:
approx. 1.0 units* approx. 10 μl
*U100 Insulin

WARNING
Change head-set every 2–3 days (max. 72
hours). Otherwise you risk insulin resistance or
inammation of the infusion site. Never use the
transfer-set for more than 6 days.
Disconnect Accu-Chek Ultraex
WARNING
Please consult with your health care profession-
al on how to compensate for any missed insulin
while you are disconnected. Carefully monitor
blood glucose level while disconnected from the
pump and after you reconnect.
Accu-Chek Ultraex allows you to temporar-
ily disconnect from your pump for shower-
ing, swimming, etc., without changing your
Accu-Chek Ultraex.
Hold the self-adhesive (F) in place, gently press
the side clips of the connector (I) and pull the
connector (H) from the cannula housing (G).
Put the disconnect cover (L) on the cannula
housing.
Reconnect Accu-Chek Ultraex
WARNING
Before reconnecting, ensure that a drop of medi-
cation appears at the tip of the connector needle.

Fill the Accu-Chek Ultraex until medication
comes out of the connector needle.
Hold the self-adhesive (F) in place and remove
the disconnect cover (L) from the cannula
housing (G). Reattach the connector (H) to the
cannula housing. An audible «click» ensures the
connector is locked.
WARNING
It is not necessary to give an additional bolus.

ADVERTENCIAS
Lea detenidamente las instrucciones antes de
usarlo. Es recomendable que consulte el manual
de la micro-infusora de insulina para obtener
información acerca de esta terapia.
Si es la primera vez que lo utiliza, deberá co-
locarlo bajo la supervisión de un profesional
sanitario.
• Este dispositivo se conserva estéril hasta que
se abre o se daña el envase. No utilice el pro-
ducto si el envase está abierto o ha sufrido
algún daño.
• Proteja el producto de la humedad excesiva,
de la luz solar y del calor. Almacénelo a tem-
peratura ambiente.
• Deseche el set de infusión después de haberlo
utilizado. No lo limpie ni vuelva a esterilizarlo.
• Accu-Chek Ultraex es sólo para uso subcu-
táneo.
• Ajuste siempre con rmeza el cierre luer con
el adaptador de la bomba. Si no procede de
esta forma, la conexión del set de infusión a
la bomba puede quedar holgada y ocasionar
alguna fuga. No utilice ninguna herramienta
para enroscar el conector de cierre luer. Si lo
hace, puede dañar el conector de cierre luer.

• No deje burbujas de aire dentro del Accu-Chek
Ultraex. Cébelo completamente.
• Un pequeño porcentaje de los dispositivos
Accu-Chek Ultraex que contienen cánulas
exibles pueden doblarse durante la introduc-
ción o moverse durante su uso. En caso de
que esto ocurra, debe utilizar de inmediato un
nuevo dispositivo Accu-Chek Ultraex.
• Compruebe con frecuencia el sitio de inserción
para asegurarse de que la cánula exible per-
manece rmemente en su lugar. Dado que es
una cánula exible, no notaría ningún dolor si
se saliera. La cánula exible debe permane-
cer siempre totalmente insertada para recibir
la dosis completa de insulina. Sustituya el
dispositivo Accu-Chek Ultraex cuando vea
que se despega el adhesivo.
• No vuelva a introducir la aguja guía en la cánu-
la exible. Si lo hace, podría perforarse y cau-
sar una salida impredecible de medicamento.
• Si tiene que desconectar el Accu-Chek Ultra-
ex, hágalo de la forma más higiénica posible.
• No utilice la misma cánula exible durante
más de 72 horas (3 días). No utilice el catéter
durante más de 6 días. Cuando cambie el car-
tucho, asegúrese de purgar completamente el
catéter antes de conectarlo al cabezal.
• Si la zona de inserción se inama, retire la cá-
nula y utilice un nuevo Accu-Chek Ultraex en
una zona de aplicación distinta.

• No permita que Accu-Chek Ultraex entre
en contacto con desinfectantes, perfumes o
desodorantes, ya que estos productos pue-
den afectar a la integridad del dispositivo.
• Nunca purgue el catéter ni intente desob-
struirlo mientras se encuentre conectado al
cabezal. De lo contrario, podría introducir ac-
cidentalmente una cantidad no deseada de
insulina.
• Evite la presión mecánica en el sitio de inser-
ción y en el dispositivo Accu-Chek Ultraex.
Tenga cuidado cuando cargue objetos pesados.
• Limpie el sitio de inserción antes de introducir
la cánula, según las indicaciones de su médico
para mantener una higiene adecuada.
• Compruebe el nivel de glucemia de 1 a 3 horas
después de haber introducido el dispositivo
Accu-Chek Ultraex y revise con regularidad
el sitio de inserción. No introduzca una cánula
exible nueva antes de acostarse. Vigile con
frecuencia la glucemia y comente los resulta-
dos con su médico.
• Durante el uso, compruebe periódicamente el
set de infusión en la conexión del cierre luer al
tubo, cada tres horas durante el día y antes de
acostarse. Una rotura en este punto, podría dar
lugar a fugas, lo que causaría la interrupción
de la administración de insulina. Si detecta
una rotura, cambie inmediatamente el set de
infusión y compruebe el nivel de glucemia.

• Si la glucemia se eleva de forma inexplicable o
se presenta una alarma de obstrucción, com-
pruebe si el catéter está obstruido o si hay una
fuga de medicamento. En caso de duda, cam-
bie el dispositivo Accu-Chek Ultraex.
Cómo colocar Accu-Chek Ultraex

Elija un punto alejado de la cintura, los huesos,
las cicatrices, el ombligo y otros sitios de inser-
ción que haya utilizado recientemente. Lávese
las manos y desinfecte la zona de inserción
siguiendo las instrucciones del profesional sani-
tario. Deje secar la zona de aplicación antes de
introducir la cánula exible.

Sujete el Accu-Chek Ultraex por el tapón de
soporte azul (M) con el pulgar y el índice y retire
la cubierta protectora transparente (B) girando y
tirando. Guarde la cubierta protectora. La uti-
lizará posteriormente para desechar de forma
segura la aguja guía (C).

Pellizce el tejido subcutáneo. Introduzca la aguja
guía (C) y la cánula exible (D) de forma rápida
y recta en el sitio de inserción, con un ángulo
de 90°. Asegúrese de que la cánula exible entra
suavemente, sin arrugarse ni doblarse.
ADVERTENCIA
Asegúrese de que lo aplica de forma higiénica.
No toque la cánula exible ni la aguja guía.
Cerciórese de que la cánula exible haya sido
introducida por completo en el tejido subcutáneo
y no se haya doblado en la supercie de la piel.
Cuando la aguja guía se introduce en la piel, con
la cánula exible debe hacerlo también rápida-
mente hasta el fondo para evitar que se doble.
Si la cánula se doblara, el ujo de insulina podría
disminuirse o detenerse.

Alise cuidadosamente el adhesivo (F) pasando
los dedos por encima para asegurar un contacto
perfecto con la piel.

los clips laterales del tapón de soporte azul (M) y
retírelo del cuerpo de la cánula (G).
