Itispossibletouseeithersingleplateneutralreferenceelectrodesor
electrodeswithsplitconductivezonesotowatchtheplatetopatient
contactduringthesurgicalintervention.
Controloftheunitsisviafrontpanelkeysanddisplay;mainsinletis
locatedontherearpanel.
Theunitshaveautomaticcontrolsystemsthat,monitoringtheinternal
parameters,signalthepossibledamages/errorsthatarefound.
Theoperationalparametersthatareusedareconstantlystoredsothat,
everytimetheunitisswitchedonortheoperativemethodischanged,the
lastutilizedparametersarerecalled.
Theleveloftheemissionsoundcanvary;everyoperatorcanchoosehis
ownlevelaccordingtothenoiseofhisworkingambient.
Poweroutputcanbeactiveeitherthroughholder‐handleswith
pushbuttonsorthroughsingleordoublefootswitchcommand.Moreover,
applyingaspecialoptionaladapteritispossibletheunitconnectionto

causeaslowheatingofintra‐cellularliquid,thewatercontainedinthecell
evaporatesandanactionofcoagulumisobtained,sothebloodflowis
stopped.

Temperatureover100ºCintheareaaroundtheactiveelectrode
determinestheevaporationoftheintracellularliquidandthecell

Thepulsedcurrentcausestheneuro‐muscularstimulation,originatedby

MonopolarCut
Monopolarcutisthesectioningofthebiologicaltissueachievedbythe
high‐densitypassageofHFcurrent,whichisconcentratedatpointofthe
activeelectrode.TheHFcurrent,whenitisappliedtothetissue,through
thepointoftheactiveelectrode,itcreatesintensemolecularheatinthe
cellssohighthatexplosionofitiscaused.Thecuteffectisachievedby
movingtheelectrodethroughthetissueanddestroyingthecellsoneafter
theother.Themovementoftheelectrodepreventsthepropagationofthe
sideheatinthetissue,thuslimitingtoasinglelinethecells’destruction.
ThebestHFcurrentforcuttingispuresinewavewithoutanymodulation
thatcutsverysmoothlyandprovidestheleastthermaleffectwithpoor
haemostasiswhilecutting.Becauseitseffectscanbepreciselycontrolled,
itcanbeusedsafelywithoutdamagetothebone,butsincegood
coagulationwhilecuttingisoneofprincipalbenefitsofusingelectro
surgeryacurrentwithacertainamountofmodulationisdesirable.

BipolarCoagulation

obtained,withoutdeeppenetrationanditsconsequentcoagulation.These

Electrosurgeryisnotrecommendedinthefollowingsubjects:
havingpacemaker;
withstimulatingelectrodes;
withmetalprosthesisplant;
withimportantarterialpressureunbalance;
withimportantnervousdisorders;
withrenalinsufficiency;
instateofpregnancy.
BurnsarethemostconsequencesoftheHFelectrosurgeryforthepatient,
evenifthesearenottheonlyone.Infactnecrosisbycompression,allergic
reactionstothedisinfectant,gasorinflammableliquidsignition.
Someimportantcausesofburnsareby:
insufficientmedicalequipetrainingaboutallmodalitiestoavoidor
reducetherisksofburnsbyusingHFelectrosurgicalunits;
useofdisinfectantswithhighalcolcontent;
incorrectpositionofthepatientduringtheelectrosurgical
operation;
contactbetweenactiveelectrodeandtheskin;
contactwithliquid;

Thewholesurfaceofthepatientplatemustbeplacedonawell‐
vascularizedmuscleasnextaspossibletosurgicalarea.Avoid
connectingthepatientplatetobonyprotrusions,prosthesis,cicatricial
tissues,andpartsofthebodysubjectedtoliquidaccumulationorthat
presentsubcutaneousadiposetissue.Thepartofthebodymustbe
withouthair,dryandclean.Donotusealcoholtocleantheskin.Unless
forveterinaryuse,theuseofgelatinoidssubstancesfortheelectrodes
isnotadvised.
Byusingthedisposableneutralelectrodesrespectthedateofexpire.
By usingthereusableelectrodesascertainthatthefixingsystemsgive
warrantyofstability.
Whenyouapplytheneutralelectrodeavoidthetransversalcourse

ForsurgicalprocedureswheretheHFcurrentcouldflowthroughparts
ofthebodyhavingarelativelysmallcross‐sectionalarea;theuseof
bipolartechniquesmaybedesiderableinordertoavoidunwanted
coagulation.
Thepowerlevelshouldbethelowestusefultotheworktodo.
Alwayscheckthereturnplatewheneverelectrosurgicalunitfailsto
producethedesiredeffect.Reasonforalowoutputpowerlevel,orfor
anincorrectfunctioningoftheelectrosurgicalunitwhenarrangedfor
anormaloutput,maybelackofconnectionofthereturnplateorits
imperfectplacement.
Theuseofflammableanesthetics,ofoxygenandofnitrogenprotoxide
shouldbeavoidedinthecaseofoperationattheheadoratchest
levelexceptthepossibilityofevacuatinggas.Flammablematerials
usedtoclean,ortodisinfect,shouldbelettoevaporatebeforethe
useoftheelectrosurgicalunit.Thereisriskofstagnationofflammable
solutionsunderthepatientorinbodycavitiesastheumbilicusandthe
vagina.Thefluidthatdepositsintheseareasshouldberemoved
beforetheequipmentuse.Thedangerofendogenousgasignitionhas
tobeconsidered.Somematerialslikecottonwoolorgauze,when
saturatedwithoxygen,mayburstintoflamesbecauseofthesparks
producedbytheequipmentinthenormaluse.
