Introduction
Intended Use
The Hudson RCI® Neptune® (also referred to herein as ‘humidifier’) is a respiratory humidifier designed
to heat and humidify respiratory gases delivered via endotracheal tubes, nasal cannula or face masks to
adult, pediatric and infant/neonatal patients. This system may be used with either conventional
(non-heated wire) breathing circuits or compatible (21-volt) Hudson RCI heated-wire circuits.
The Hudson RCI Neptune can be used with ventilators, continuous flow systems, oxygen diluters and
blenders, adjustable nebulizer adapters for aerosol therapy, or non-flammable anesthesia gases to help
maintain patient body temperature.
The Hudson RCI Neptune is intended for use in professional healthcare environments by qualified
trained personnel and within the limits of its stated technical specifications.

General Warnings, Cautions, and Contraindications
Warning: The Hudson RCI Neptune humidifier has been designed and tested for use with Hudson RCI
Branded accessories including 21-Volt Heated Wire and conventional breathing circuits. For
safe operation, only use Hudson RCI Branded accessories and breathing circuits. Use of
non-approved circuits or accessories with this unit may cause degraded performance and
an unsafe condition. See the current issue of the Hudson RCI Product Catalog or go to the
Medline Website at (www.medline.com) for compatible circuits and accessories validated for
use with the Hudson RCI Neptune Heated Humidifiers.
Warning: Fire Hazard. Do not use the humidifier in the presence of flammable anesthetic gases.
Warning: Burn Hazard. The heating element of the humidifier is hot. Allow the humidifier to cool before
handling.
Warning: Burn Hazard. The Concha-Column is hot (as high as 88°C) and may contain liquid at
temperatures up to 81°C. Allow the column to cool before handling.
Warning: Electrical Shock Hazard. No modification of the humidifier is allowed. Refer all servicing to
Medline or authorized repair center. Use of the RS-232 service connector by anyone other than
qualified, factory-trained personnel may damage the instrument and result in harm to the
patient. No connection to this port is to be made when device is operational and connected to
a patient. Patient injury may occur.
Warning: Electrical Shock Hazard. To avoid the risk of electric shock, the humidifier must only be
connected to a supply mains with protective earth.
Warning: Electrical Short Hazard. Do not immerse this device in water or other fluids for cleaning. Do
not autoclave, gas sterilize or pasteurize. Severe damage will result.
Warning: This humidifier requires 100-120V ~, (nominal) 60 Hz sine wave output. If used in a transport
vehicle requiring an inverter, do not use square or pulse width modulated sine wave output. To
do so may result in overheating.
Warning: Grounding reliability can only be achieved when the humidifier is connected to a grounded
power receptacle with the Hospital Grade power cord supplied in the original packaging. This
device has been developed to operate over the voltage ranges noted on the name plate label
for use in foreign countries.
Warning: The temperature probes must be properly placed in the breathing circuit before operating the
humidifier. Failure to properly connect the temperature probes will cause the unit to alarm.
The probe alarm is not intended to replace routine physical inspection of the temperature
probe placement. Be sure both probes are in place during operation of humidifier.
Warning: Always verify airway temperature and that there is regulated gas flow through the system
before connecting the humidifier to the patient. Failure to do so may result in heat buildup,
causing a bolus of hot air to be delivered to the patient.
• Do not turn the humidifier ON without gas flow.
• Do not leave the humidifier ON without gas flow.
• Do not exit the pause mode without gas flow.
• Turn the humidifier OFF before stopping gas flow.
• When using pause, place in pause mode before stopping gas flow.
• Allow the system to cool before stopping gas flow.
• Use in accordance with recommended flow rates.
Warning: When using the ConchaSmart® column with a Hudson RCI Neptune that does not have a
low water sensor, the low water notification will not work. Also, when using columns other

than a ConchaSmart column with a Hudson RCI Neptune that has a low water sensor, the low
water notification will not work.
Warning: Some ventilators produce elevated gas temperatures at the patient inhalation port which can
reduce the amount of absolute humidity in the circuit. Visually inspect for condensation
droplets in the inspiratory circuit after one hour of operation, and periodically thereafter.
Warning: Do not use the humidifier at an altitude above 700 hPa (3000 m) or outside a temperature of
20 to 26°C (68 to 79°F). Using the humidifier outside of this temperature range or above this
altitude can affect the quality of the therapy or injure the patient.
Warning: When using Hudson RCI Heated-Wire Circuits:
• Observe the minimum minute volumes recommended in this manual..
• Do not place or hang foreign objects from the circuit tubing.
• Do not apply excessive tension to the heated-wire harness - DO NOT “MILK” TUBING.
• Do not allow the circuit to rest on the patient’s bare skin.
Warning: Covering breathing tubes with sheets, blankets, towels, clothing or other materials or heating
them in an incubator or with an overhead heater may soften circuit tubing, affect the quality
of the therapy, or injure the patient.
Warning: Maintain adequate gas flow through the column and breathing circuit. This will prevent
overheating the column and breathing-circuit.
Warning: Do not use a Low-Compliance Concha-Column in continuous flow CPAP applications with gas
flow greater than 50 LPM. There is potential for flooding of the breathing circuit and airway.
Warning: The Comfort Flo Column is designed exclusively for use with continuous high flow oxygen
applications with the Comfort Flo Humidification System. Only the ConchaSmart Column or
Comfort Flo Column should be used with the Hudson RCI Neptune Humidifier for continuous
high flow oxygen therapy. Use of any other column during high flow oxygen applications may
cause flooding of the circuit. Do not exceed 60 LPM in this mode because there is potential for
flooding of the breathing circuit and airway.
Warning: Sudden changes in circuit pressure may cause the water in the column to rise temporarily and
may affect compressible volume.
Warning: Common radio frequency emitters including RFID and security systems may interfere with
normal Hudson RCI Neptune Humidifier operation. The Neptune has been tested and conforms
to testing of radiated immunity down to 80 MHz. Frequencies below this limit may lead to
temporary loss of function or degradation of performance which ceases after the disturbance
ceases, and from which the Neptune recovers. Operator intervention may be required
including power cycling of the heater and or increasing the distance between the device and
the radio frequency source.
Warning: Use of this equipment and it’s accessories adjacent to or stacked with other equipment should
be avoided because it could result in improper operation. If such use is necessary, this
equipment and the other equipment should be observed to verify they are operating
normally.
Warning: Use of accessories, transducers and cables other than those specified or provided by the
manufacturer of this equipment could result in increased electromagnetic emissions or
decreased electromagnetic immunity of this equipment and result in improper operation.
Warning: Portable RF communications equipment (including peripherals such as antenna cables and
external antennas) should be used no closer than 30 cm (12 inches) to any part of the Hudson
RCI Neptune, including cables specified by the manufacturer. Otherwise, degradation of the
performance of this equipment could result.

Warning: Do not add any attachments or accessories to the humidifier that are not listed in the
instruction for use of the humidifier or accessory, or the humidifier might not function
correctly affecting the quality of the therapy or injuring the patient.
Warning: Use of the humidifier with equipment other than defined as part of intended use should be
avoided because it could result in improper operation. If such use is necessary, the humidifier
and the other equipment should be observed to verify that they are operating normally.
Warning: To prevent disconnection of the tubing or tubing system during use, only tubes in compliance
with ISO 5367 or ISO 80601-2-74 should be used.
Warning: The humidifier is intended for use in an electromagnetic environment typically found in
hospitals and the professional healthcare environment. The humidifier is not intended to be
used in hospitals near active HF surgical equipment and the RF shielded room of an MRI for
magnetic resonance imaging, where the intensity of electromagnetic disturbances is high and
can result in improper operation.
Warning: The humidifier performance may be negatively affected by the simultaneous use with a
nebulizer.
Warning: Hudson RCI Branded circuits and accessories are Single Use, with the exception of the
Re-usable Temperature Probes, which are cleanable and reusable. Accessories should be
changed when noticeably contaminated by the patient, or they are damaged through use.
Reusing or not changing single use accessories creates a potential risk of serious injury and/or
infection.
Caution: Federal law (USA) restricts this device to sale, distribution, and use by, or on the order of.
a physician.
Caution: Always operate the humidifier in a vertical position with the water reservoir properly
mounted.
Caution: Temperature patterns within the system associated with normal operation will be disrupted if
there is insufficient water to pass through the system, which may cause a high temperature
alarm.
Caution: Do not operate the humidifier with a dry or disconnected reservoir or without a column
installed. Severe damage may result.
Caution: Failure to open upper reservoir clamp on the Comfort Flo Column may result in water from the
column entering circuit tubing.
Caution: Failure to open the upper reservoir clamp on a ConchaSmart Column may result in no water
entering the column.
Caution: Placing a temperature probe in an environment which is at or near the temperature of the gas,
such as an incubator or radiant warmer, may mask a dislodged probe, may cause an alarm, or
cause the heating elements to shut off, thus reducing the humidity and temperature to the
patient.
Caution: Replace the power fuse only with one of the same rating.
Caution: In the Invasive, or Subglottic Mode, the humidifier is designed to deliver a minimum of
33 mg H2O/L at 37°C, for the proximal airway temperatures at zero gradient and with the
recommended Flow Rates listed in the Specification Section of this manual.
• Use of Positive or “Sun” gradient settings in low patient airway temperature conditions
can result in humidity outputs at the patient of less than 33mg H2O/L.
• Visually inspect for condensation droplets in the inspiratory circuit after one hour of
operation, and periodically thereafter.

Caution: Monitor the inspiratory and expiratory limbs of the circuit periodically for excessive
condensation (rainout). Use water traps to collect rainout when using non-heated wire
breathing circuits.
Caution: Routine checks as described in this User Manual are essential for proper use of the Neptune
Humidification System. The low water notification feature is intended as a convenience feature
and not a replacement of those routine checks.
Caution: Electrically powered medical devices may generate artifacts in patient monitors under some
conditions. Should artifacts be observed, it is recommended to follow the monitor, EKG Pad,
and Lead Wire manufacturer’s recommendations for elimination of the artifact.
Contraindication: ConchaSmart humidification column is contraindicated for use with the Comfort Flo
Humidification System circuits for continuous flow oxygen therapy.
NOTE: The Neptune was designed to shorten start up time by maintaining power to the control circuit
boards anytime the unit is plugged into an active electrical outlet. This design generates a small
amount of heat and it is normal for the Neptune housing to be warm to the touch and for the
cooling fan to cycle on and off periodically when the unit is in “Stand-By” (Off) mode.
NOTE: The Emissions characteristics of this equipment make it suitable for use in industrial areas
and hospitals (CISPR 11 class A). If it is used in a residential environment (for which CISPR 11
class B is normally required) this equipment might not offer adequate protection to
radio-frequency communication services. The user might need to take mitigation measures, such
as relocating or re-orienting the equipment.
NOTE: Dispose of shipping and packaging materials included with the Neptune and its accessories in
accordance with local, state, and federal regulations and facility protocols.

facility protocols.
NOTE: The facility should ensure the compatibility of the humidifier and all of the parts and accessories
used to connect to the patient or other equipment before use.

prefilled with sterile water and are not refillable. See the current issue of the Hudson RCI Product
Catalog or go to the Medline website (www.medline.com) for water reservoirs validated for use
with the Hudson RCI Neptune Heated Humidifiers.

Principles of Operation
Neptune Basics
The Hudson RCI Neptune heated humidification system consists of:
• Hudson RCI Neptune heated humidifier
• Concha® Column humidifier cartridge (also referred to as ‘column’)
• Concha sterile water reservoir (Available in 1650ml)
• Hudson RCI Neptune dual-thermistor temperature probe
• Hudson RCI Breathing circuit (non-heated wire breathing circuit or
21-volt heated-wire breathing circuit)
The heated humidifier continuously monitors the proximal airway temperature and regulates the heat
supplied to the column by the heating element, and if used, the heated-wire circuit.
The Concha-Column humidification cartridge is inserted into the humidifier where it is surrounded by
a heating element. Sterile water flows via a gravity-feed system from the reservoir into the Concha-
Column, wetting an internal absorbent wick. The heating element conducts heat through the column’s
metal wall, heating the wick and vaporizing the sterile water. Respiratory gas passes through the column
where it picks up the moisture in molecular form. The heated, humidified gas is delivered to the patient
through a breathing circuit attached to the top of the column.
Concha-Columns are available in 4 configurations: ConchaSmart, Standard, Low-Compliance, and
Comfort Flo. All columns are labeled for easy identification and operate on a reliable, gravity-feed
system for uninterrupted water flow. In addition, numerous breathing circuit options are available based
on patient demographic and type of respiratory therapy required.
Note: The amount of fluid transferred from the reservoir varies slightly depending on ventilator settings.
Several factors increase the amount of fluid transfer: high peak pressure, lower breath rates,
reduced water volume in reservoir, I/E (Inspiratory/Expiratory) ratios approaching 1:1.4, and
square pressure waveforms.
Contraindication: ConchaSmart humidification column is contraindicated for use with the Comfort Flo
Humidification System circuits for continuous flow oxygen therapy.
Note: The Hudson RCI Neptune requires the use of a dual temperature probe at all times.
Note: Failure to open the reservoir clamps on Concha Columns that have them may result in no water in
the column.
Concha-Column Description
The Standard Concha-Column (see Figure 3) is designed for use with ventilator parameters most
commonly used with adults. The water in the column will be at the same level as that in the reservoir.
The water level in both the reservoir and the column drops as water is consumed. As the water level in
the column drops, the compressible volume of the system increases and the system compliance changes.

The Low-Compliance Concha-Column is recommended for applications requiring a more constant
compressible volume and compliance, (see Figure 4). The Low-Compliance Concha-Column maintains
a constant water level in the column resulting in a more stable compressible volume. The system’s
compliance equals the column’s compliance plus the amount of fluid transferred to the reservoir during
each inspiratory phase. Some fluid, in the form of either water vapor or water, always transfers during
each cycle. In addition, because of the smaller mass of water contained inside the column, the
Low-Compliance Column will heat and cool faster as compared to the Standard Concha-Column with a
full water reservoir.

The Comfort Flo Concha-Column is for use with continuous high flow oxygen applications with the
Comfort Flo Humidification System. This column is also equipped with the ability to notify clinicians of
low water conditions when used with a Neptune equipped with a low water notification sensor.
The ConchaSmart Column is for use in all heated humidification applications except with Comfort Flo
Humidification System. It maintains a constant compressible volume like the low-compliance column.
This column is also equipped with the ability to notify clinicians of low water conditions when used with
a Neptune equipped with a low water notification sensor.

• *The Invasive Mode of the Hudson RCI Neptune Heated Humidifier was designed to provide 100% RH, as evidenced by
moisture condensation in the circuit at the recommended flow rates over the settable proximal airway temperature
settings at zero gradient. In some cases, the use of positive gradient settings “ “or flow rates outside the
recommended ranges may result in increased warm-up times or lower than recommended humidity outputs.
Additionally, some environmental conditions, such as warm ventilator outlet gas temperatures and/or high ambient
temperatures, may require the use of the negative gradient setting “ ” to reach 100% RH, or visible moisture droplets
in the circuit.
Humidity Output(Essential Performance):
Invasive Mode > 33 mg H2O/L at 37°C proximal airway temperatures when set at zero gradient
at the recommended Flow Rates listed above.

Warning: Operating the humidifier at the extremes of the operating temperature may affect the

Front Panel Indicators
1. Non-Invasive Mode - Icon illuminates when the humidifier is in the SUPRAGLOTTIC or
Non-Invasive Ventilation mode.
2. Invasive Mode - Icon illuminates when the humidifier is in the SUBGLOTTIC or Invasive or
Intubated mode.
3. LED Display - Displays the temperature within tenths of a degree centigrade resolution between
25°C and 45°C. Above 45°C, the display will read ‘HI’. During normal operation, the LED displays actual
proximal patient airway temperature. To display the column temperature, press and hold the console
button. The column temperature will be displayed as long as the key is pressed.
4. Airway Temperature - Icon illuminates green during normal operation. This indicator flashes red when
there is a temperature probe problem or an over temperature condition at the patient airway.
5. Column Temperature - Icon illuminates green when the console button is pressed and the column
temperature is displayed on the temperature display LED. This indicator flashes red when there is a
temperature probe problem or high temperature condition at the column.
6. Inspiratory Wire Indicator - Arrow icon illuminates green when the inspiratory wire of a heated-wire
circuit is connected to the humidifier. This arrow will flash red when the inspiratory wire status has
changed (connected or disconnected) during operation. This arrow will flash green if the status
(connected/disconnected) is changed during “Confirmation of Settings or Set-up”.
7. Expiratory Wire Indicator - Arrow icon illuminates green when the expiratory wire of a
heated-wire circuit is connected to the humidifier. This arrow will flash red when the expiratory wire
status has changed (connected or disconnected) during operation. This arrow will flash green if the
status (connected/disconnected) is changed during “Confirmation of Settings or Set-up”.

8. Pause Indicator - When the humidifier has been placed in pause mode, the icon will illuminate and
the LED display will count down the remaining pause time.
9. Service and Auto Settings Mode Indicator - If the humidifier detects an internal problem during
operation, the icon will flash, the Yellow Warning Indicator will illuminate, and an audible alarm will
sound. If this happens, turn the humidifier off and let it cool. If the condition persists, take the unit out
of service and return to Medline for servicing. A solid red wrench will display in the set-up state if the
unit is in Auto Settings mode, this indicates that the settings cannot be adjusted until the unit is taken
out of Auto Settings mode.
10. Temperature Gradient Indicator - Indicates the temperature gradient set point. Indicator is active
when using a heated-wire circuit and will be illuminated during “Set-up” and “Check Settings” modes.
11. Low Water notification - Indicates that water in column is low.
Status Indicators
1. GREEN - Green icons indicate the device is functioning normally.
2. YELLOW - A solid yellow icon in conjunction with a low volume, 2-tone audio signal indicates
a low priority alarm condition (see also “Alarm Conditions” in the “Troubleshooting” section of this
manual).
3. YELLOW - A yellow flashing icon in conjunction with a low volume, 3-tone audio signal indicates
a cautionary alarm condition (also see “Alarm Conditions” in the “Troubleshooting” section of this
manual).
4. RED - A red flashing icon in conjunction with a high volume, 5-tone audio signal indicates an
emergency alarm condition requiring immediate action by the clinician (see “Alarm Conditions” in the
“Troubleshooting” section of this manual).

Front Panel Controls
a. Power ON/OFF - The button controls power to the humidifier. Press and hold to turn unit ON.
Press and hold to turn unit OFF. When the unit is plugged in, but not turned on the button will
fade on and off to indicate that the unit is in standby mode.
b. Mode Selector - This key selects the operating mode (Invasive or Non-Invasive).
c. Temperature and Pause Time Control - The Increase and Decrease buttons allow the user to
adjust the proximal patient airway temperature and pause time settings.
d. Temperature Gradient - Sun and Raindrop - These buttons increase or decrease the
temperature gradient between the column and the patient end of the heated-wire circuit.
e. Gradient Display - Blue LED between the Sun and Raindrop indicates the actual temperature gradient
setting. The example below shows a setting of one degree towards the raindrop. This setting would
increase humidity in the circuit.
Example:
f. Accept Button - Used to check settings, access adjustable settings and accept the displayed
parameters and store settings into memory.
g. Pause Button - Used to enter the Pause Mode for a user adjustable period of 5-60 minutes.
h. Alarm Mute Button - Used to silence an Alarm or low water notification alert for two minutes.
i. Console Button - When pressed, the LED displays the temperature of the gas at the outlet of the
column. The column temperature will remain displayed as long as the key is pressed.

Directions for Use
Read this entire section, including all warnings and cautions, before operating the Hudson RCI Neptune.
See the “Applications” section for tips on using the Hudson RCI Neptune in specific clinical situations.
System Setup
1. Installation:
Mount the humidifier to a pole, ventilator or rail system using the appropriate mounting bracket.
Contact your Medline representative for available mounting brackets. Mounting instructions are
supplied with the bracket.
Warning: Fire Hazard: Do not use humidifier in the presence of flammable anesthetic gases.
Caution: Always operate the humidifier in a vertical position with the water reservoir properly mounted.
2. Connect the humidifier to power:
a. Connect the supplied hospital grade power cord to the rear of the unit.
b. Plug the humidifier into an appropriate 120 V ~ three-prong, grounded electrical outlet. Verify
power at the outlet.
c. When unit is plugged in, but not turned on (standby mode), the power button will pulse as an
indication that power is being supplied.
Note: Mains power can be removed from the humidifier by removing the power cord from the wall
outlet or power supply input. Do not position the Neptune so that it is difficult to perform this
disconnection.
3. Install the Concha Column and Concha Water Reservoir:
a. Install reservoir bracket on the side of the Neptune.
b. Insert the column into the humidifier. Be sure that the tubing clamp(s) are closed.
c. Place the water reservoir into the reservoir bracket.
Note: Ensure the reservoir bracket and water reservoir are installed in the correct manner as shown
below for proper operation.
4. Connect the ventilator circuit to the Concha-Column:
a. Remove the yellow cap from the top of the column.
b. Connect the humidifier line from the ventilator to one of the ports on the column.
c. Connect the inspiratory tubing elbow to the remaining port on the column.
d. Connect the expiratory tubing elbow to the expiratory port on the ventilator.
e. If using a column with 1 vertical and 1 right angle port, connect the humidifier limb to the vertical

port and inspiratory limb of the breathing circuit to the right angled port.
5. Connect the dual thermistor probe:
a. Insert the temperature probe plug into the jack at the lower front of the humidifier.
b. Place the short-cabled probe at the column outlet and the long-cabled probe into the inspiratory
side of the patient wye.
Warning: The temperature probes must be properly placed in the breathing circuit before operating the
humidifier. Failure to properly connect the temperature probes may cause the unit to alarm
and shut down.
6. Connect the heated-wire pigtails (not applicable when using conventional circuits):
a. Connect the inspiratory limb heated-wire (blue) pigtail to the blue heated-wire connector on the
humidifier cable. Connect the expiratory limb heated-wire (yellow) pigtail to the yellow heated-wire
connector on the humidifier cable.
b. Verify heated-wires are connected to the humidifier before turning the unit on.
Note: The humidifier will automatically enter heated-wire operation when it senses the inspiratory
heated-wire from a Hudson RCI circuit.
Warning: If using a heated-wire circuit with this humidifier, use only Hudson RCI 21-volt circuits.
Warning: When using Hudson RCI Heated-Wire Circuits:
• Observe the minimum minute volumes recommended in this manual.
• Do not place or hang foreign objects from the circuit tubing.
• Do not apply excessive tension to the heated-wire harness – DO NOT “MILK” TUBING.
• Do not allow the circuit to rest on the patient’s bare skin.
Warning: Covering breathing tubes with sheets, blankets, towels, clothing or other materials or heating
them in an incubator or with an overhead heater may soften circuit tubing, affect the quality
of the therapy, or injure the patient.
7. Connect the water reservoir to the column:
• Remove the cap from the column’s bottom puncture pin. Push and twist the pin through the
puncture site at the bottom of the reservoir. Insert the pin all the way. Repeat this procedure for
the top puncture pin and puncture site.
• Open the tubing clamp(s) and gently squeeze the reservoir bottle.

this manual.

Automatic Operation
1. Set up and turn ON the ventilator or gas source (see the Ventilator Manufacturer Instructions for Use
to determine ventilator operation and monitoring requirements).
Warning: Always verify airway temperature and that there is regulated gas flow through the system
before connecting the humidifier to the patient. Failure to do so may result in heat buildup,
causing a bolus of hot air to be delivered to the patient.
• Do not turn the humidifier ON without gas flow.
• Do not leave the humidifier ON without gas flow.
• Do not exit the pause mode without gas flow.
• Turn the humidifier OFF before stopping gas flow.
• When using pause, place in pause mode before stopping gas flow.
• Allow the system to cool before stopping gas flow.
• Use in accordance with recommended flow rates.
2. Press and hold the “ ” button to bring the unit out of “Stand-By” mode.
If user holds down the power button while bringing unit out of “Stand-By” mode, the software
revision will display on the LED display until the power button is released. Once activated, the
humidifier will perform a self-check (audio tones followed by a scrolling dash will be seen on the LED
display). During the self-check, confirm the unit issues the audio tones. If no tones are heard, remove
the unit from service. When the self-check is complete the set airway temperature will be displayed in
the LED display. This indicates the humidifier is ready for operation. At this point the user can choose
to confirm or change the settings.
Note: If the humidifier detects an internal fault during the self-check, the icon will flash and an
audible alarm will sound. If this happens, take the unit out of service and return to Medline for
servicing.
Note: The humidifier will begin to heat once settings are confirmed, or after 30 seconds if user does not
confirm settings.
Note: The humidifier will automatically detect Hudson RCI adult, pediatric and neonatal heated-wire
circuits.
Note: Confirm the heated-wire icons illuminate when the appropriate heated-wires are connected to the
unit.
