Device Descripti n and Intended Use:
All ADC®Neurological Percussion Hammers are medical instruments used by ealt care practitioners
to test deep tendon reflexes as part of a neurological p ysical examination in order to detect abnor-
malities in t e central or perip eral nervous system.
All ADC®Tuning Forks are medical instruments used by ealt care practitioners to conduct auditory
and neurological testing to identify underlying medical conditions. T e Rinne and Webber tests can
be conducted wit t ese devices.
C ntraindicati ns:
Neurological Instruments s ould not be used on damaged or injured locations of a patient’s body
w ere t e use of suc a device may cause greater arm or injury.
Warnings and Cauti ns:
WARNING: Neurological ammers may contain s arp points or components.
Do not leave t is device unattended around c ildren.
CAUTION: Some neurological ammers may ave pointed tips t at can cause serious injuries
if used improperly. Care s ould be taken w en andling t ese devices.
CAUTION: No form of sterilization as been validated for t is device. Do not autoclave
t is device or expose it to extreme environmental conditions as damage to t e device may result.
Operati n (F r all Neur l gical Hammers):
T e Taylor Hammer (3693) is typically eld at t e end and t en swung in an arc-like motion onto
t e tendon. T e Queen’s Square (3699) and Babinski Hammers (3697) are usually eld perpendi-
cular to t e tendon and passively swung onto t e tendon wit t e assistance of gravity. T e Jen-
drassik maneuver can also be used to accentuate reflexes.
Select models (Buck Hammer - 3691 and Babinski Hammer - 3697) ave concealed needles
and/or brus es for additional reflex and neurological testing. To use t ese components, unscrew
t e base of t e ammer’s andle. For Buck Hammers, t e concealed needle can be accessed by
unscrewing t e top of t e device and pulling t e needle free.
Operati n (F r all Tuning F rks):
To operate your tuning fork, old it as close as possible to t e end. For best results, strike t e tuning
fork gently against a ard surface near w ere t e forks start. It is not necessary to strike t e object
very ard and care s ould be taken not to strike an object t at can be easily damaged, suc as a
glass table top or ot er similar object.
Alternately, t e tuning fork may be activated by ‘snapping’ t e tips of t e forks. Place your t umb
and index figure on t e outside of eac fork and t en slide your fingers off t e tip of eac fork
w ile moving t em towards eac ot er, as if you were snapping. T e motion will set t e two
forks vibrating.

Helpful Tips when H lding the Tuning F rk:
• Keep your wrist relaxed and flexible.
• Keep your fingers firm, but don’t press ard against t e stem of t e tuning fork.
• Your elbow s ould be bent and relaxed, and your arm s ould not be tense.
• Ensure t at you are olding t e tuning fork as close to t e end as possible.
• Avoid touc ing t e tines of t e tuning fork during use.
• Avoid grasping t e tuning fork tig tly.
• Avoid aving your arm in a stiff or locked position.
• Avoid activating t e tuning fork by itting it against your and.
W en using a tuning fork to examine a patient, t e device s ould be eld in suc a way t at it is in
t e same orientation as t e patient. T e tines of t e tuning fork will give off more vibrations t at t e
patient can ear w en oriented like t is. See t e image below for more details.
Cleaning and Disinfecti n:
Your neurological instrument may be cleaned wit a soft clot or cotton swab. A low-level disinfec-
tant comprised of 70% isopropyl alco ol may be used as necessary during t e cleaning process.
C emical cleaners, excessive eat, cold, or oils may damage t e instrument and reduce its lifespan.
T is device may not be sterilized or autoclaved.
Imp rtant N te: Excessive cleaning solutions may leave a residue on t e device w ic will impact
t e sound quality of tuning forks. T ese devices must be t oroug ly cleaned and dried prior to use.
LIMITED WARRANTY:
American Diagnostic Corporation (ADC®) warrants its products against defects in materials and workman-
s ip under normal use and service as follows:
1. Warranty service extends to t e original retail purc aser only and commences wit t e date of delivery.
2. Your ADC Neurological Instrument is warranted for one year from date of purc ase (all parts).
What is C vered: Replacement of parts, and labor.
What is N t C vered: Transportation c arges to and from ADC®. Damages caused by abuse, misuse,
accident, or negligence. Incidental, special, or consequential damages. Some states do not allow t e exclu-
sion or limitation of incidental, special, or consequential damages, so t is limitation may not apply to you.

Dr., Hauppauge, NY 11788. Please include your name and address, p one no., proof of purc ase, and a
brief note explaining t e problem.
Implied Warranty: Any implied warranty s all be limited in duration to t e terms of t is warranty and in
