1 Sicherheitshinweise
VORSICHT
Nichtbeachtung der Sicherheitshinweise. Eine Nichtbeachtung
der nachstehenden Sicherheitshinweise kann zu Fehlfunktionen des
Drehadapters 4R57 und einem daraus resultierenden Verletzungsrisi-
ko für den Patienten sowie zu Zerstörungen des Drehadapters 4R57
führen�
Beachten Sie die Sicherheitshinweise dieser Gebrauchsanweisung
und informieren Sie Ihren Patienten�
INFORMATION
Der Drehadapter 4R57 ist nicht mit den Adaptern 2R49, 2R50, 4R69,
4R98, 4R95 kombinierbar, da die Anschlüsse unterschiedliche Abma-
ße haben�

VORSICHT
Umgebungs- bzw. Einsatzbedingungen. Je nach Umgebungs- bzw�
Einsatzbrdingungen kann die Funktion des Drehadapters 4R57 beein-
trächtigt werden� Es kann zu Defekten und daraus resultierenden Fehl-
funktionen bis hin zu Funktionsverlust durch Strukturversagen kom-
men� Das kann einen Sturz des Patienten zur Folge haben�
• Bitte vermeiden Sie es, Prothesenpassteile Umgebungen auszuset-
zen, die Korrosionen an den Metallteilen auslösen, z�B� Süßwasser,
Salzwasser und Säuren�
• Achten Sie darauf, dass weder feste Teilchen noch Flüssigkeit in
den Drehadapter eindringen können�
• Der Drehadapter sollte keinem intensiven Rauch oder Staub, keinen
mechanischen Vibrationen oder Stößen und keiner großen Hitze
ausgesetzt werden�
VORSICHT
Manipulationen an Systemkomponenten. Selbstständig vorge-
nommene Veränderungen bzw� Modifikationen an Systemkomponen-
ten können zu Defekten und daraus resultierenden Fehlfunktionen des
Drehadapters 4R57 bis hin zu Funktionsverlust durch Strukturversa-
gen führen� Das kann einen Sturz des Patienten zur Folge haben�
• Jegliche Änderungen oder Modifikationen am Drehadapter 4R57
können zu Nutzungseinschränkungen führen�
• Das Öffnen und Reparieren des Drehadapters bzw� das Instand-
setzen beschädigter Komponenten darf nur durch den zertifizierten
Ottobock Service vorgenommen werden�
VORSICHT
Mechanische Belastungen. Mechanische Einwirkungen bzw� Belas-
tungen von außen, wie z�B� Stöße und Vibrationen, können zu Defek-
ten und daraus resultierenden Fehlfunktionen des Drehadapters 4R57

einen Sturz des Patienten zur Folge haben�

• Setzen Sie Systemkomponenten keinen mechanischen Vibrationen
oder Stößen aus�
• Vermeiden Sie Schläge oder andere Gewalteinwirkungen auf
den Auslöseknopf, denn dadurch kann die Rotationsfunktion des
Drehadapters beeinträchtigt werden�
INFORMATION
Falls Sie eine Beschädigung entdecken, senden Sie den Drehadapter
bitte an eine Ottobock Servicestelle�

4 Beschreibung
4.1 Verwendungszweck
Die Drehadapter 4R57 und 4R57=ST sind ausschließlich zur exopro-
thetischen Versorgung der unteren Extremität oberhalb des Prothesen-
kniegelenks einzusetzen�
Amputationsniveau: Oberschenkelamputation, Hüftexartikulation�
4.2 Einsatzgebiet
Einsatzgebiet nach dem Ottobock Mobilitätssystem MOBIS:
4R57, 4R57=ST:
Zugelassen bis 150 kg Patientengewicht.
Die Drehadapter wurden für Alltagsaktivitäten entwickelt und dürfen
nicht für außergewöhnliche Tätigkeiten wie zum Beispiel Extremsportar-
ten (Freiklettern, Paragleiten, etc�) eingesetzt werden�
Die Versorgung eines Patienten mit den Drehadaptern 4R57 und
4R57=ST darf nur vom Orthopädie-Techniker vorgenommen werden�
4.3 Funktion
Im alltäglichen Umgang mit der Oberschenkelprothese sind „Kombina-
tionsbewegungen“, z�B� beim „Schneidersitz“, nur eingeschränkt mög-
lich� Ebenso werden Positionsänderungen der Sitzhaltung zur Erhöhung
des Tragekomforts von Patienten erwünscht� Die Drehadapter 4R57
und 4R57=ST erfüllen diese Zusatzfunktion� Sie müssen oberhalb des
Prothesenkniegelenks eingebaut werden, so dass der gebeugte Unter-
schenkel gegen den Schaft rotiert, d�h� nach innen oder nach außen
gedreht werden kann� Beim Autofahren oder zum Wechsel des Schuhs
werden die Vorteile deutlich (Abb� 2 und Abb� 3)
Zur Freigabe der Drehung wird der Auslöseknopf durch Drücken betä-
tigt� Die Arretierung in die Ausgangsposition erfolgt selbsttätig�

4.3.1 Konstruktive Besonderheiten:
Das Drehgelenk des Drehadapters besteht aus zwei zueinander durch
Lager drehbaren Teilen und einer Verriegelungseinrichtung�

Die beiden Lager nehmen die hohen Kippkräfte, die in Folge des Ge-
wichts des Prothesenträgers entstehen, spielfrei auf und benötigen nur
ein geringes Verdrehmoment von 0,5 Nm im eingebauten Zustand� In
Nullstellung erfolgt das Einrasten selbsttätig�

6 Handhabung
6.1 Drehadapter mit Justierkern 4R57 (Abb� 4)
Der Drehadapter besitzt einen speziellen Justierkern zur Aufnahme in ei-
nen Schaftansatz mit Justierkernaufnahme oder einen Doppeladapter mit
Justierkernaufnahme� Zum Austausch oder bei der Demontage müssen
alle vier Gewindestifte des Adapters (Abb� 4, Pos� 1) herausgedreht wer-
den� Die Justiermöglichkeiten bei statischen Korrekturen sind in diesem
Bereich geringfügig� Zum Anziehen der Gewindestifte einen Drehmo-
mentschlüssel benutzen (z� B� Ottobock Art� Nr� 710D4)�

INFORMATION
Das Anzugmoment für Gewindestifte des proximal anschließenden Ad-
apters beträgt abweichend von der Vorgabe in der entsprechenden
Gebrauchsanweisung 10 Nm (Abb� 4)�
Zur Anprobe: Die Gewindestifte im Uhrzeigersinn auf 10 Nm anzie-
hen (Abb� 4)�
Zur Fertigstellung: Die Gewindestifte mit Loctite 636K13 sichern und
im Uhrzeigersinn auf 10 Nm bzw� 15 Nm anziehen (Abb�4)�
6.2 Drehadapter mit Gewindeanschluss 4R57=ST (Abb� 5)
Über den Gewindeanschluss des Drehadapters erfolgt die Verbindung
mit den Eingussankern 4R43 und 4R111=N�
INFORMATION
Für beide Eingussanker ist die Laminierhilfe 4X46=ST zu verwenden
(sie ist nicht im Lieferumfang enthalten)�
Vorgang: Drehadapter bis zum Anschlag einschrauben� Danach eine
Umdrehung, maximal zwei Umdrehungen zurückdrehen, bis
• der Auslöseknopf medial ausgerichtet ist und
• sich das Gehäuse des Drehadapters nach Betätigung des Auslöse-
knopfes 360° durchdrehen lässt, ohne den Eingussanker zu berühren�
Den Drehadapter im Eingussanker durch Festziehen der Klemmschrau-
be (Abb� 5, Pos� 2) fixieren�
Zum Anziehen der Klemmschraube einen Drehmomentschlüssel benut-
zen (z�B� Ottobock Art� Nr� 710D4)�
INFORMATION
Das Anzugmoment für die Klemmschraube des 4R111=N und des
4R43 beträgt 10 Nm (Abb� 5)�

Zur Anprobe: Die Gewindestifte im Uhrzeigersinn auf 10 Nm anzie-
hen�
Zur Fertigstellung: Die Gewindestifte mit Loctite 636K13 sichern und
im Uhrzeigersinn auf 10 Nm vorziehen, anschließend Montageanzugs-
moment 15 Nm.
6.3 Justierkernaufnahme
Die Justierkernaufnahme der Drehadapter dient zur distalen Verbindung
mit einem Justierkern� Über die vier Gewindestifte sind statische Korrek-
turen während des Aufbaus, der Anprobe und nach Fertigstellung der
Prothese jederzeit übersichtlich durchführbar�
Zum Austausch eines Moduls oder bei der Demontage wird die vorher
justierte Position beibehalten, wenn nur zwei nebeneinander angeordne-
te Gewindestifte, und zwar die am tiefsten eingeschraubten, herausge-
dreht werden�
Gewindestifte, die bei der Justierposition zu lang (Schaumstoffbeschä-
digung) oder zu kurz (geringe Festigkeit) erscheinen, sollten durch ent-
sprechend passende ersetzt werden:

6.4 Wartungshinweise
Grundsätzlich werden alle modularen Adapter von Ottobock mit drei Mil-
lionen Belastungszyklen geprüft� Dies entspricht, je nach Aktivitätsgrad
des Amputierten, einer Nutzungsdauer von drei bis fünf Jahren�
Wir empfehlen grundsätzlich regelmäßig jährliche Sicherheitskontrollen
durchzuführen�

7 Haftung
Der Hersteller empfiehlt, das Produkt nur unter den vorgegebenen Be-
dingungen und zu den vorgesehenen Zwecken, sowie mit den für die
Prothese geprüften Modular-Bauteil-Kombinationen, entsprechend des
Ottobock Mobilitätssystems MOBIS zu verwenden und es entsprechend
der Gebrauchsanweisung zu pflegen� Für Schäden, die durch Passteile
verursacht werden, die nicht vom Hersteller im Rahmen der Verwendung
des Produktes freigegeben wurden, haftet der Hersteller nicht�
8 CE-Konformität
Das Produkt erfüllt die Anforderungen der Richtlinie 93/42/EWG für
Medizinprodukte� Aufgrund der Klassifizierungskriterien für Medizinpro-
dukte nach Anhang IX der Richtlinie wurde das Produkt in die Klasse I
eingestuft� Die Konformitätserklärung wurde deshalb von Ottobock in
alleiniger Verantwortung gemäß Anhang VII der Richtlinie erstellt�

INFORMATION
The 4R57 Rotation Adapter cannot be combined with Adapters 2R49,
2R50, 4R69, 4R98 and 4R95, because the connectors have different
dimensions�
CAUTION
Environment and usage conditions.The environment and usage
conditions may compromise the functionality of the 4R57 Rotation Ad-
apter� This may lead to defects and resulting malfunction, including
loss of function due to structural failure� This can cause the patient to
fall�
• Please avoid exposing prosthetic components to corrosive elements
such as fresh water, salt water and acids�
• Do not let foreign particles or liquids get into the rotation adapter�

structural failure� This can cause the patient to fall�

4 Description
4.1 Intended Use
The 4R57 and 4R57=ST Rotation Adapters are intended solely for the
exoprosthetic fitting of amputations of the lower limb above the prosthetic
knee joint�
Amputation level: transfemoral amputation, hip disarticulation�
4.2 Field of Application
Field of application according to the Ottobock MOBIS Mobility Sys-
tem:
4R57, 4R57=ST:
Approved for a patient weight of up to 150 kg (330 lbs).
The Rotation Adapters have been developed for everyday use and must
not be used for unusual activities such as extreme sports (free climbing,
paragliding, etc�)�
The fitting of patients with the 4R57 and 4R57=ST Rotation Adapters may
only be carried out by a prosthetist�
4.3 Function
During daily life with a transfemoral prosthesis “combination move-
ments”, such as for sitting with the knees crossed, are possible only to
a limited extent� Position changes of the sitting posture, in order to in-
crease the wearing comfort of the patients, are desired as well� The 4R57
and 4R57=ST Rotation Adapters comply with these additional require-
ments� They must be installed above the prosthetic knee joint so that the
lower leg rotates against the socket when flexed, i�e� so that it can be
rotated inward and outward� When driving a car or when changing shoes
the advantages become obvious (Fig� 2 and 3)�
The release button need only be pressed to allow the unit to rotate� The
unit will automatically lock in the neutral position�

Structural Features:
The rotation joint of the Rotation Adapter consists of two parts that rotate
on bearings, as well as a locking device�
The high forces that are generated by the weight of the prosthesis wear-
er, are absorbed without play by the two bearings� These bearings re-
quire a torque moment of only 0�5 Nm/0�37 ft�lbs�/4�42 lbs�in� when in-
stalled� In the neutral position the unit locks automatically�

6 Handling
6.1 4R57 Rotation Adapter with Adjustment Pyramid (Fig� 4)
The Rotation Adapter has a special adjustment pyramid for connection to
a socket attachment block with pyramid receiver or double adapter with
pyramid receiver� In the case of replacement or disassembly all four set
screws of the Adapter must be removed (Fig� 4, item 1)� For static cor-
rections, adjustment possibilities are minimal� Use a torque wrench to
tighten the set screws (such as Ottobock art� no� 710D4)�

INFORMATION
The torque of the adjustment screws of the proximal adapter connec-
tion is 10 Nm (Fig� 4)� This value differs from the torque specified in
the relevant user’s manual�
For trial fitting: Turn the adjustment screws clockwise to a torque of
10 Nm (Fig� 4)�
For finished prosthesis: Secure the set screws with 636K13 Loctite
and tighten them clockwise to 10 Nm/7.4 ft.lbs./88.5 lbs.in. and 15
Nm/11 ft.lbs./133 lbs.in. respectively (Fig� 4)�
6.2 4R57=ST Rotation Adapter with Threaded Connector (Fig�5)
The threaded connector of the Rotation Adapter provides the connection
to the lamination anchors 4R43 and 4R111=N�
INFORMATION
For both lamination anchors the 4×46=ST Lamination Aid is to be used
(not included in the delivery)�
Procedure: Screw the Rotation Adapter in completely� Then unscrew the
Rotation Adapter by one turn, maximally two turns, until
• the release button is positioned medially and
• after pressing the release button, the housing of the Rotation Adapter
can be rotated 360° without contacting the lamination anchor�
Fix the Rotation Adapter in the lamination anchor by tightening the clamp-
ing screw (Fig� 5, item 2)�
Use a torque wrench to tighten the clamping screw (such as Ottobock
art� no� 710D4)�
INFORMATION
The torque for the clamping screw of the 4R111=N and of the 4R43 is
10 Nm/7.4 ft.lbs./88.5 lbs.in. (Fig� 5)�

The torque for the set screws is 15 Nm/11 ft.lbs./133 lbs.in. (Fig� 5)�
For trial fitting: Tighten the set screws clockwise to 10 Nm/7.4 ft.
lbs./88.5 lbs.in.
For finished prosthesis: Secure the set screws with 636K13 Loctite
and pre-tighten to 10 Nm/7.4 ft.lbs./88.5 lbs.in. Then complete as-
sembly by tightening to 15 Nm/11 ft.lbs./133 lbs.in.
6.3 Pyramid Receiver
The pyramid receiver of the Rotation Adapters provides the distal con-
nection to the knee joint� Static corrections are possible at any time dur-
ing alignment and trial walking and after completion of the prosthesis us-
ing the four set screws� To exchange a module or during disassembly the
previous position may be retained by loosening only the two most deeply
screwed in, adjacent set screws�
Replace set screws that appear too long (to prevent foam cover damage)
or too short (which can fail) with the appropriate screw length:

6.4 Maintenance Instructions
As a basic principle, all Ottobock modular adapters are subjected to
tests involving three million load cycles� Depending on the amputee’s
activity this corresponds to a service life of three to five years�
We recommend carrying out regular safety checks once a year�
7 Liability
The manufacturer recommends using the device only under the speci-
fied conditions and for the intended purposes� The device must be main-
tained according to the Instructions for Use� The device must only be op-
erated with tested modular components in accordance with the Ottobock

Mobility System� The manufacturer is not liable for damage caused by
component combinations that were not authorized by the manufacturer�
8 CE conformity
This product meets the requirements of the 93/42/EWG guidelines for
medical products� This product has been classified as a class I prod-
uct according to the classification criteria outlined in appendix IX of the
guidelines� The declaration of conformity was therefore created by Otto-

1 Consignes de sécurité
PRUDENCE
Non-respect des consignes de sécurité. Le non-respect des con-
signes de sécurité indiquées ci-après peut conduire à des dysfoncti-
onnements de l‘adaptateur de rotation 4R57 et, par conséquent, à un
risque de blessure pour le patient ainsi qu‘à des endommagements de
l‘adaptateur de rotation 4R57�
Respectez les consignes de sécurité des présentes instructions
d‘utilisation et informez votre patient�

INFORMATION
L’adaptateur de rotation 4R57 n’est pas compatible avec les adapta-
teurs 2R49, 2R50, 4R69, 4R98 et 4R95, les raccords présentant diffé-
rentes dimensions�
PRUDENCE
Conditions d‘environnement et d‘utilisation. Les conditions
d‘environnement et d‘utilisation sont susceptibles de nuire au fonc-
tionnement de l‘adaptateur de rotation 4R57� Cela peut entraîner des
pannes et, par conséquent, des dysfonctionnements, voire une perte
de fonctionnalité due à une défaillance structurelle� Cela peut provo-
quer la chute du patient�
• Évitez de placer les composants de prothèse dans des milieux pou-
vant provoquer la corrosion des parties métalliques, par ex� l’eau
douce, l’eau salée et les acides�
• Veillez à ce qu’aucune particule solide ni aucun liquide ne puissent
pénétrer dans l’adaptateur de rotation�
• N‘exposez pas l’adaptateur de rotation à une fumée ou à une pous-
sière excessives, à des vibrations mécaniques ou des chocs et à
une forte chaleur�
PRUDENCE
Manipulations des composants du système. Tout changement ou
modification apportés de votre propre chef aux composants du sy-
stème sont susceptibles d’entraîner des pannes et donc des dysfonc-
tionnements de l‘adaptateur de rotation 4R57, voire même une perte
de fonctionnalité causée par une défaillance structurelle� Cela peut
provoquer la chute du patient�
• Tous les changements ou toutes les modifications effectués sur
l‘adaptateur de rotation 4R57 peuvent entraîner des restrictions
d’utilisation�
• Seul un SAV certifié par OttoBock est autorisé à ouvrir et à répa-
rer l‘adaptateur de rotation ou à remettre en état les composants
endommagés�

Charges mécaniques. Les effets ou les charges mécaniques exter-
nes tels que les chocs et les vibrations peuvent entraîner des pannes
et, par conséquent, des dysfonctionnements de l‘adaptateur de rotati-
on 4R57, voire une perte de fonctionnalité causée par une défaillance
structurelle� Cela peut provoquer la chute du patient�
• Protégez les composants du système des vibrations mécaniques
et des chocs�
• Évitez tout choc ou toute autre manipulation brutale du bouton de
déclenchement pouvant endommager définitivement la fonction ro-
tative de l’adaptateur�
INFORMATION
Veuillez envoyer votre adaptateur de rotation à un SAV Ottobock, si
celui-ci présente des dégradations�
PRUDENCE
Manipulation incorrecte de l‘adaptateur de rotation 4R57.Toute
sollicitation excessive ou surcharge peut entraîner des pannes et des
dysfonctionnements de l‘adaptateur de rotation 4R57, voire une perte
